APPARENTLY EMP WAS ATTEMPTING TO STRAIGHTEN A BULL DOZER TILT ADJUSTING BRACE BY USING A 200 TONPRESS WHEN THE PRESSING TOOL SLIPPED OFF THE BRACE AND STRUCK EMP IN LEFT SIDE OF FACE.
APPARENTLY THE EMPLOYEE WAS OPERATING A SCRAPER ON A STEEP GRADE WHEN HE HIT A HOLE IN THE HAUL ROAD CAUSING HIS SEAT TO COLLAPSE RESULTING IN STRAINED CHEST MUSCLE.

APPARENTLY THE EE HIT HIS HEAD ON THE FIARLEADS OF THE DRAGLINE HURTING HIS NECK. HE DID NOT RECEIVE MEDICAL TREATMENT UNTIL NOV 20.
EMPLOYEE SAID HE WAS GRINDING OVER HEAD AND A PIECE OF METAL FELL IN HIS EYE. THE EMPLOYEE WAS WEARING SAFETY GLASSES AND A FACE SHIELD.
THE EMPLOYEE SAID HE WAS BACKING A DOZER & THE TRACTOR SLID OFF A ROCK WHICH CAUSED SOME PAIN IN H IS NECK I NAME REPORTED THIS INCIDENT ON 02-04-92 BUT WAS NOT TREATED UNTIL 02-14-92 FOR THE INJURY AT WHICH TIME IT BECAME A MEDICAL TREATMENT
EMPLOYEE SAID HE WAS MACHINGING A METAL PIN ON THE LATHE & REACHED UP TO PULL OFF WHAT HE THOUGHT WERE LOOSE METAL SHAVINGS THE SHAVINGS WERE NOT LOOSE & AS THE LATHE TURNED THE SHAVCINGS CUT HIS THUMB
EMPLOYEE SAID THAT HE WAS CHANGING TEETH ON A DRAGLINE BUCKET WHEN ANOTHER EMPLOYEE USING AN AIR CHISEL SMASHED HIS THUMB BETWEEN THE TOOTH AND THE CHISEL.
THE EMPLOYEE SAID HE WAS TAKING A BOLT OUT OF IDLERS ON A D9L DOZER WITH AN IMPACT WRENCH. THE PIN CAME OUT OF THE SOCKET ON THE IMPACT WRENCH AND CAUGHT THE PALM OF THE LEFT HAND. TWO LACERAT IONS RESULTED, REQUIRING TREATMENT AND STITCHES BY MEDICAL PERSONNEL.
APPARENTLY I.NAME WAS USING A HUDRAULIC PUMP & BEARING PRESS TO INSTALL A PIN THE PRESS BROKE STRIKING I.NAME IN THE JAW & NOSE
INAME APPARENTLY RAISED UP UNDER A METAL BEAM STRIKING THE POINT OF HIS SHOULDER ON THE BEAM.
EMPLOYEE CLAIMED HE STRAINED HIS LOWER BACK WHILE WELDING GROUSER BARS ON DOZER TRACKS.
IT APPEARS THAT I.NAME CRAWLED ON THE DRAGLINE SHOE FROM THE INSIDE OF THE SHOE AND WAS PINCHED BETWEEN THE BEAM SUPPORTING THE SHOE GUIDE AND THE SHOE OF THE DRAGLINE
