Employee reported while filling an emulsion tank, EE's left foot slipped. EE landed on EE's left side and experienced soreness in EE's left hand/wrist and shoulder. Shoulder MRI results obtained on 04/22/25 indicated multiple areas of tendinosis and advanced AC joint arthropathy.

Employee reported pain in EE's neck and top of shoulder after turning EE's head to look at a berm while operating a dozer.

Employee was assigned to change a stem on an inner dual of a Komatsu haul truck. In the process of completing this task, EE reported experiencing pain in EE's upper back.

Employee reported that EE was attempting to assist a mechanic pulling a rock knocker out of the rear of a truck bed. EE tried to catch the rock knocker with EE's hand after being instructed not to do so. EE's hand was crushed into the back deck of the truck.

Employee reported shoulder pain after operating equipment. Employee has had two prior surgeries on same shoulder (Occupational Illnesses: 02/19/11 and 05/09/15).

While training a Journeyman Mechanic to perform a strut PM on a Komatsu 830E, lost footing and landed on the concrete floor on right hip/leg.

Employee reported that right shoulder was bothering them approximately two months after running a Loader. Employee was sent to a physician who prescribed physical therapy.

Pinchpoint smash of Right Index Finger. Requiring stitches. No permanent damage. Work to resume 5/10/2022

Employee reported stepping off a beam to a walkway inadvertently landing on the edge of the walking surface.

Employee reported that EE was navigating a side hill when EE felt something burn in EE's left shoulder.

Employee reported while climbing on a dozer, employee experienced pain in knee. MRI revealed medial extrusion of the body of the medial meniscus and a tear of the root of the medial meniscus. These conditions are associated with osteoarthritis and degenerative changes.

Employee reported discomfort in shoulder at the end of shift. The employee reported using a hammer to assist in removal of a pin earlier in the day. MRI received on 2/3/21 indicated presence of a ganglion cyst, advanced AC joint arthrosis and mild bursitis presence.

Employee reported while climbing off a motor grader, EE slipped off the last step. EE's lower back landed on the back side of the moldboard.

Employee reported that when cleaning out a drain pan in the lube bay, EE tipped the pan forward and it became unstable. EE felt pain in EE's left shoulder immediately following the incident.

Employee reported EE had reached under EE's safety glasses to itch left eye. EE immediately noticed a painful sensation in the eye. EE was treated by an optometrist who removed a foreign body.

Employee reported while assisting a mechanic, they felt some discomfort in right shoulder. MRI results given to the employee on 04/28/20 indicated moderate tendinosis, bursitis and degenerative changes.

Employee reported while exiting a pickup, EE slipped and fell. EE reported pain in the left arm/shoulder immediately following the incident.

Employee removing hydraulic pump from tire truck when it slipped and pinched a finger. X-rays subsequently revealed a non-displaced terminal tuft fracture.

At approximately 4:05 p.m. a small brush/grass fire was ignited by lightning. There was no mining activity underway near the point of ignition. This fire is being documented with a 7000-1 report as it was not fully extinguished within 30 minutes of discovery and MSHA was contacted immediately.

Walking up short ladder when the ladder moved causing foot to hit floor. Fracture of metatarsal bones in left foot.

Employee lifted microwave. Felt strain in left bicep and later than evening noticed bruising and some swelling. MRI reviewed and physical therapy prescribed on 6/17/19. MRI indicates degenerative tearing of nearly the entire labrum, synovitis in the rotator interval and severe acromioclavicular osteoarthrosis.

Employee reported dismounting a haul truck and straining EE's shoulder. MRI reviewed and physical therapy prescribed on December 10. MRI indicated multiple degenerative conditions suggesting tendinopathy, arthritis and bursitis. No work restrictions or days of work missed.

Employee was welding overhead on a dragline when a piece of slag fell into right eye. EE was treated by an optometrist who removed the foreign body.

Two small grass/bush fires were started by lightning near the shop and office complex at Trapper the evening of 8/31/18. There were no mining activities near the points of ignition. These fires are being documented with a 7000-1 report as they were not fully extinguished within 30 minutes of discovery and MSHA was contacted immediately.

Employee reported while stacking cases of water in change room, felt a pain in the left side of abdomen.

Acetylene tank that was being stored on the shop floor caught fire during an off shift. The tank was not in use at the time of the fire. There were no employees working in the shop at this time.

Employee reported when moving a rock knocker that had fallen off a haul truck, EE felt a pop and pain in EE's left arm. Surgery performed 3/19 for a left elbow distal tendon rupture repair.

Employee moving dragline cable experienced lower back strain. Work restriction issued 12/19/17.

Employee was getting off a dozer when their foot slipped on tracks. Employee was holding the grab rail with right arm and strained the right shoulder. Reported the injury on 10/9. Surgery was determined necessary on 11/3. Employee had surgery on 11/14.

Employee was discovered deceased in the active mining area. There were no indications of traumatic injury event. Employee is believed to have been engaged in routine surveying activity at the time of collapse.

Heavy equipment operator restricted from work duty as of February 7 following report of progressively worsening lower back pain. MRI indicates degenerative changes including multilevel facet joint arthropathy in the lumbar spine.

Employee reported dismounting dragline at end of shift and reaching up to get employee's lunch box when employee felt a twinge of pain in employee's shoulder that became increasingly worse. Physical therapy prescribed. Subsequent MRI indicates multiple degenerative changes that will require surgical repair.

Employee moving dragline cable stepped into a dragline shoe track and EE's left knee buckled. Pain and swelling developed over the remainder of EE's shift and persisted over the weekend. Employee referred to doctor on 5/23 for evaluation. MRI completed 6/2.

Employee reported after dismounting haul truck, ee was walking away when ee slipped and fell on left side. Physical therapy was prescribed on 2/5/16.

While seated, co-worker patted employee's knee. The employee reacted by reaching to push the co-worker's hand away. Hands made contact and a loud "pop" occurred. X-rays revealed fracture of the 4th metacarpal.

The operator allowed haul truck to gain too much momentum and failed to make left hand turn causing extensive damage to truck. No reportable injury occurred.

Employee reported while positioning a heavy metal stand, the stand toppled injuring two fingers on the employee's left hand. The injuries required medical treatment and resulted in time off work.

Employee reported numbness in both hands on 6/25/15. The employee underwent surgery on 9/8/15.

Employee reported shoulder pain after operating haul truck. Employee has had prior surgery on same shoulder (occupational illness, 2/19/11).

Employee reported carrying part in shop when left foot twisted (ankle rolled) and he went down. Left knee became sore over weekend and employee went to ER for evaluation. MRI results received 4/7 indicate torn ligament.

Maintenance employee reported he knelt down on a bolt while working on an overburden drill. His knee later swelled up and started to hurt. Medical treatment provided at follow-up appointment 4/8/15.

Employee reported after stepping off a dozer, he felt a locking sensation and knee pain. Employee had surgery on 4/23.

Employee repositioned right leg while conducting maintenance work, stepped backwards and felt a pop in right knee. Diagnosed with partial dislocation and restricted from all work 3/31.

Employee was stepping out of the operator's cab to clean the rear window on bulldozer when he slipped and fell from the equipment deck to the ground. This injury resulted in an ear laceration requiring sutures.

Employee was cutting a piece of lath toward his thumb with a knife when it slipped, cutting his left thumb. This injury resulted in a laceration requiring sutures.

Employee reported noticing numbness in both hands. Workers' compensation claim filed. Determined non-work related. Employee subsequently appealed denial and obtained approval for carpal tunnel release surgery. Employer informed of surgery and verified work-related compensability decision on 7/6/15.

On April 30, employee reported shoulder pain after slipping and catching himself dismounting haul truck. On Friday, November 14, employee confirmed plans to undergo surgery to repair torn tendon. He has not missed work or been restricted from normal work activity to date. Surgery to be scheduled for December or January.

Employee reported lower back pain after operating a dozer. The medical evaluation identified degenerative conditions in the lumbar spine. Diagnostic results were obtained 5/7/14.

On April 13, 2014, employee reported lower back strain after jarring back while operating bulldozer. Physician issued lifting restriction on April 21 that prevents employee from fulfilling normal duties.

Employee diagnosed with bilateral carpal tunnel syndrome. Notified employer on 12/10/13 of surgery scheduled for 1/7/14.

Employee was checking a fuel leak repair on a haul truck when a loose particle blew into his left eye. He was treated by an optometrist who removed the foreign body.

Employee was assisting with a cable change on a boom winch. His left hand was pinched when the weight of the cable shifted while getting the cable back on the sheave. This injury resulted in a laceration requiring sutures.

Employee reported back pain after knocking wedges out of a tooth adaptor with a sledge hammer. Notified employer on 12/18/13 of surgery scheduled for 1/3/14.

Employee was installing track pin when the track press failed causing an object to strike him in the forehead.

Employee was pulling jack stand into place when he felt pain in wrist and noted immediate swelling. Left wrist sprain. Provided with splint.

Employee injured when operating motor grader that was struck by haul truck.

Employee was cutting outer jacket off electrical cable with a utility knife. The knife slipped, cutting his left hand. This injury resulted in a laceration requiring sutures.

Employee was cleaning a weld on the lip of an excavator bucket with an air chipper when a piece of debris made contact with his eye. He was treated by an optometrist who removed the foreign body and applied antibiotics.

Employee was assisting in replacement of a bulldozer ripper tooth when a metal fragment from keeper pin chipped off and struck him in the left forearm. Treatment for this injury included an incision requiring stitches.

EE investigating oil leak under 777 haul truck during pre-op inspection on March 18. Reached up with rag to wipe some dirt from hose. His light went out. Dirt fell on his face behind safety glasses. Blinking, couldn't see, stood up and hit his head under the truck. May 9 employer was informed of a bulging disc from this incident. Work restriction imposed May 6. No missed work days.

Employee slipped and fell on 2/26/13. Surgery to repair torn rotator cuff took place on 8/29/13.

Employee reported slipping on ice, falling and injuring right shoulder. Employer notified of the incident on 1/2/13. Referred to a physician who prescribed physical therapy on 1/15/13. No restrictions or days away from work as a result of the injury.

Employee pinched his left hand ring finger in the door stop on the rear door of the dragline. This injury resulted in a laceration requiring sutures. The employee did not miss any work time or was restricted as a result of the injury

Employee reported pain in groin area while operating dozer. Diagnosed with a hernia. Compensability not yet determined.

Employee pinched the end of his right hand ring finger when lifting a piece of grouser material up and onto a work bench. This injury resulted in a laceration that required sutures. The employee did not miss any work time or was restricted as a result of the injury.

Employee reported while operating a dozer, he ran over a rock and hit his head inside the cab. Two days after the incident (8/9), he began complaining about neck pain and clumsiness of the left arm.

Ee was diagnosed with bilateral carpal tunnel syndrome on 1/27/12. Reported this condition to Trapper on 2/1/12. Diagnosis confirmed and made available to Trapper on 2/7/12.

Employee was replacing a cross-link on a motor grader tire chain. While tightening the pin on a shackle with a crescent wrench the wrench slipped, hit the employee in the mouth, and chipped a tooth.

Employee reported continuing knee pain after his foot slipped on a clump of dirt while walking on blast pattern. He was referred to a specialist who diagnosed him with an extensively torn and degenerated meniscus and mild to moderate chondromalacia. Surgery was performed on 10/11/11.

Employee was closing his toolbox door on his service truck when he smashed his thumb. Employee received treatment in the form of sutures. Employee did not miss any work time or incur any restrictions as a result of this injury.

Ee reported his right shoulder had been bothering him. No specific incident noted. Employee was diagnosed with osteoarthritis. Ee was not restricted or missed any time as a result of the illness until surgery was performed on 10/13/11.

Employee was working to remove stuck rock lodged between dozer push arm and track frame. Rock shifted and crushed tip of left ring finger requiring medical treatment.

A bulldozer operator on a D10R Caterpillar dozer went over an approximately four foot tall steeply sloped bank. As the machine proceeded over this bank, the front end cantilevered and dropped suddenly and struck the ground, jarring the operator.

Employee diagnosed (06/16/2011) with carpal tunnel syndrome right hand. Surgery scheduled for 07/25/2011. Employer informed 07/08/2011.

Employee had surgery on his right knee on 7/28/11 for treatment of chronic knee pain. Employee first reported pain and swelling on 2/28/11.

EE using pry bar to realign center pin felt "something kinda gave in my left shoulder." Became painful over time as EE continued working. Shoulder surgery recommended; completed 04/04/11. Documented as work-related occupational illness on 5/12/11.

Employee was using an air chipper for a welding project. He reported putting the air chipper down when the trigger engaged blowing accumulated slag particles into his eyes. Employee was treated by an optometrist who removed a corneal foreign body from his left eye. Employee has not missed any work time or been restricted as a result of this injury.

Employee pulled rock from drill hole and felt pain in bicep. Referred to Orthopedic who prescribed surgery for several chronic degenerative conditions involving bicep tendons and rotator cuff. Surgery performed on 3/24. Treating physician indicates employee's shoulder condition was due to chronic degeneration rather than a single traumatic event.

Employee reported while driving down pit area in his service truck, he drove across of spot of loose coal causing his truck to jump. He hit his head on the roof of the truck causing neck pain. Employee was referred by a physician for chiropractic treatment. He did not miss any work time or incur any restrictions a result of the incident.

Employee reported while reaching over to close the door on the loader, he felt pain in his lower back. Pain developed into severe muscle spasm requiring medical evaluation. He was restricted from full work duties as a result of this injury.

Employee reported while loading dragline bucket teeth into a truck he strained his shoulder. Reported to employer on 10/21/10. He was restricted from full work duties as a result of this injury on 10/22/10.

Employee reported a large rock was loaded into the haul truck bed causing truck to bounce and seat to bottom out. This resulted in a broken tooth which required treatment from a dentist. The employee did not miss any work time or incur any restrictions as a result of the incident.

Employee was changing an oil filter when his hand slipped causing his hand to hit the heat shield exhaust. Employee received treatment in the form of sutures. Employee did not miss any work time or incur any restrictions as a result of this injury.

Employee reported while servicing a Komatsu haul truck, he lost his footing on the ladder and fell, striking the ladder with his chest. Reported to employer on 6/21/10. He was restricted from full work duties as a result of this injury on 6/22/10.

Employee reported while washing the rear window of dozer, his foot fell through a section of the open cat walk. His right calf struck the cat walk. Employee was restricted from all work duties on 6/17/10.

Employee reported slipping while washing the drill deck, twisting his left knee. On 6/16, physician reported small fractures in the tibial plateau and left fibular head. Employee was restricted from full work duties. Referred to an orthopaedic specialist who sustained the work restrictions.

An employee of our Electrical Subcontractor was using a razor knife to cut the tape that was binding a bundle of pipe. The knife slipped and cut the index finger on the employee's left hand, resulting in the need for three stitches.

Employee worked his scheduled shift and was then taken to the emergency room after complaining of neck/chest pain. Employee was hospitalized and subsequently suffered a heart attack requiring quadruple bypass surgery two days later.

Employee slipped and fell from the back of a flatbed truck. He landed on his left side causing pain in his hip, elbow, and shoulder. Employee was restricted from all work activities.

EE reported climbing ladder on equipment when he felt his left big toe "pop". Nothing unusual about the climb - no slip, trip, fall, no imbalance, no awkward positioning, nothing being carried. When EE removed boot after work, big toe droopy. Diagnosed with extensor tendon rupture; surgery required. EE reported same surgery on same toe from chainsaw accident 40 years ago.

Employee reported he was driving a haul truck when he hit a pot hole jarring his back causing severe immediate pain. As a precautionary measure, the treating physician restricted the employee from all work activity during the initial diagnostic phase of evaluation and treatment. At a subsequent appt, the work restriction was sustained.

Employee was dismantling rear drive train on a Ford F600 2-ton truck when the rear differential unexpectedly dropped out of its housing onto his hand. Employee received treatment in the form of sutures. Employee did not miss any work time or was restricted as a result of this injury.

Employee tripped over a phone cord and fell, dislocating her shoulder. Surgery date and days away from work are pending at this time.

EE with history of chronic lower back pain, back surgery (see E-Doc 118619), lifted large wrench and felt pain in lower right side. Then swung 20# sledge hammer several times to tighten nut. Reported injury 08/03/09 and referred to physician same day. Physician removed employee from work for one week.

EE knelt down on a rock with left knee. Worsening pain and stiffness in knee developed over next few days. EE reported injury and was referred to physician on 07/06/09. Work restrictions limiting activity were imposed by treating physician.

Employee was exiting a scraper when he reportedly got his boot caught on bottom rung of ladder. When leaning forward to grab the ladder, the ladder swung back pinching his left index finger causing a laceration. Employee received treatment in the form of sutures. Employee did not miss any work time or been restricted as a result of this injury.

EE was stepping down from rubber tired dozer. Stepped on a rock or chunk of coal and twisted knee. Reported injury to employer on 04/09/2009. Arthroscopic surgery completed on 08/14/2009 with subsequent time away from work.

On 3/20/09, EE reported left knee felt "funny" after crawling around under truck on knees. Referred to orthopedic specialist and diagnosed with numerous degenerative conditions requiring arthroscopic treatment. EE underwent surgery on 7/2/09.

Employee was pulling a tire chock from a CAT 777 haul truck injuring his right shoulder. Employee did not miss any work time or been restricted as a result of this injury. Shoulder surgery was performed on 05/06/09.

Surveyor measuring coal seam thicknesses on sloping pit floor slipped and fell, landing on and fracturing left wrist.

EE reported shoulder had been bothering him for 3-4 months. No specific incident noted. Physician determined that an impingement was not created by work activities but welding activity was aggravating condition. EE was not restricted or missed any time as a result of the illness. Surgery performed on 6/11/09.

Employee reported on 12/22/08, that he had stepped down from a dozer and rolled his ankle, feeling it "pop". Xrays indicated a fractured bone. Employee has been restricted from walking on uneven ground as a result of the injury.

Employee was working in the house structure of the dragline, when he felt pulling at back of left foot. Employee was referrred to a physician on 12/2/08 and was restricted from all work activities.

Employee reported back pain on 9/22/08. Referred to a physician for evaluation who referred employee for chiropractic treatment on 10/8/08. Employee has not missed any work time or been restricted as a result of this injury.

Employee reported back pain on 10/31/08. Referred to a physician for evaluation. Physician referred employee for continuing chiropractic treatment on 11/17/08. Employee has not missed any work time or been restricted as a result of this injury.

On 06/04/09, a hearing loss first reported by the employee on 10/30/08, was determined compensable.

Employee was performing mechanical work on a drill. He reportedly reached out to grab handle of the door and shut door on left thumb. Employee received treatment in the form of sutures at the Emergency Room. Employee did not miss any work time or been restricted as a result of this injury.

EE rept'd pain in rt. shoulder devel. over past 4 months (after seeking treatment on own). No specific incident/activity rept'd as cause. Work restrictions issued on 7/8/08 limiting activities and EE missed work time. Diagnosis: ?right shoulder rotator cuff tendinitis? On 9/19/08, Comp. provider notified TMI-claim accepted and EE compensated for time missed.

Employee reported lower and middle back pain after handling dragline power cable. Referred to a physician for evaluation. Xrays showed degenerated intervertebral discs and arthritis. Referred to Chiropractor.

Employee reported he was climbing onto a dozer when his knee gave out.

Employee was removing a tire and rim assembly from a trailer when he felt a pop on the top of his right shoulder. MRI completed 4/14/08 showed severe degenerative changes in the AC joint requiring surgery.

Employee slipped and fell on 2/14/08 injuring his right shoulder. Employee did not miss any work time or been restricted as a result of this injury. Shoulder surgery was performed on 5/13/08.

Employee reported that he was driving a haul truck through rough section of pit floor resulting in discomfort in his left shoulder and neck. He was referred to a physician who referred him to a chiropractor for treatment (10/11/07). This employee has not missed any work time or been restricted as a result of this injury.

Employee reported lower back pain after operating forklift in outdoor storage area. A lifting restriction limiting normal job duties was issued 6/25/07.

EE working under pickup when debris fell into right eye. EE treated by a Physician who noted foreign body and rustering in the right corneal rim. Antibiotic ointment was instilled and will be administered for the next several days by EE. He reported injury on 1/22, doctor notes received on 1/30. He has not missed any work time or been restricted as a result of this injury.

EE reported noticing pain in right shoulder becoming worse over approximate five week period. Referred to physician and then orthopedic specialist and diagnosed with "right shoulder rotator cuff tendinitis." Notified on 09/21/06 EE had received medical treatment in the form of an injection. EE has not missed any work time or been restricted as a result of this illness.

Employee reported that he had been experiencing pain in his left elbow while operating the dragline. Referred to a physician, and then orthopaedic specialist who diagnosed with Lateral Epicondylitis left. Notified by doctor on 4/19/06 that he had received treatment in the form of an injection. Employee has not missed any work time or been restricted as a result of this illness.

EE states that over the past several months, he has had increasing pain and swelling in his right hand. His right arm also goes to sleep while operating equipment. Referred to an orthopaedic physician and diagnosed with right carpal tunnel syndrome. Employee has not missed any work time or been restricted as a result of this illness to date. Notified of diagnosis 2/2/06.

EE reported sharp pain in lower back after welding in awkward position. EE diagnosed with "degenerative disc disease lumbar spine" after previous injury (11/05/02). MRI indicates this occurrence is directly related to that condition.

A sales and product support representative tripped as he was walking off a drill pattern. He attempted to regain his balance by extending his leg to catch himself. As he planted his foot, he heard and felt a loud pop as his hip dislocated. Subsequent x-rays indicated dislocation and fractured hip.

Employee has been diagnosed with "degenerative changes AC joint" in the right shoulder. No days away from work or restricted duty days have occurred to date due to this degenerative shoulder condition. Surgery is pending.

EE reported pain in middle back after operating a bulldozer to rip and push a layer of rock. Referred to physician who prescribed physical therapy for treatment. No work restrictions or lost workdays occurred as a result of this injury.

Employee was operating a dozer (341) on a dozer push for 1-1/2 weeks. The machine was reported as riding rough causing the employee pain in his lower back (for the past three weeks). He was referred to physician who prescribed physical therapy for treatment (3/29/05). This employee has not missed any work time or been restricted as a result of this injury.

Employee reported that he was driving over rough roads when he hit two holes in the road, resulting in jarring of his back and neck. He was referred to a physician who referred him to a chiropractor for treatment. This employee has not missed any work time or been restricted as a result of this injury.

On 03/04/2005 a hearing loss first reported by the employee on 12/13/2004 was determined to be compensable and is therefore reportable under Part 50. This hearing loss does not meet the reportable criteria specified under Part 62.

While lifting dragline power cable, employee pulled something in his lower back. Past medical history of chronic lower back pain and has had previous back surgery. The employee was released to full work duty without restrictions upon initial doctor's visit. Subsequent appointment resulted in a lifting restriction. Employer was notified of lifting restriction on 12/6/04.

Strained lower back when lifting drill bit.

Tim states that his hands have been falling asleep, especially the right one. Tim indicates that this is most noticeable after he has been sleeping. Referred to an orthopaedic physician where he was diagnosed with bilateral carpal tunnel syndrome. Employee has not missed any work time or been restricted as a result of this illness to date. Notified of diagnosis 2/22/05.

John reported that he had been experiencing pain in his left elbow while steam cleaning. Referred to a physician and diagnosed with left lateral epicondylitis (tennis elbow). This condition is caused by repetitive twisting of the wrist/forearm. Employee has not missed any work time or been restricted as a result of this illness.

Employee reported pain in both hands and wrists. Referred to physician and diagnosed as "carpal tunnel syndrome, tendonitis of wrist." Doctor identified work as an aggravating factor. Employee has not missed any work time or been restricted as a result of this illness to date.

Employee reported both wrists tingle and ache while working. Referred to physician and diagnosed as: "wrist pain due to arthritis vs. tendonitis." Doctor identified work as an aggravating factor. Employee has not missed any work time or been restricted as a result of this illness to date.

Employee suffered heart failure due to natural causes. Investigation showed no exertion prior to the event. Employee was discussing project assignments with other crew members when collapse occured. No tools or equipment were in use at the time of the collapse. The employee did not survive.

Employee was operating a dozer when a rock came off the highwall (right side). The rock came through the door glass over right leg striking the employee in the left ankle. This employee notified employer on 11/18/04 that he was pursuing physical therapy (treatment option).

HE WAS USING A GRINDER WEARING HIS SAFETY GLASSES AND A FACE SHIELD. AFTERWARDS HE RUBBED HIS EYES WITH HIS HANDS, HE BELIEVES HE INADVERTANTLY RUBBED SOME GRINDING DUST INTO HIS EYES.

EE was driving a pin out of the dozer lift cyl. using an 8lb hammer, he struck a glancing blow causing his left wrist to twist. Subsequent diagnosis of: 1. Bilateral wrist ulna impaction syndrome. 2. Bilateral carpal tunnel syndrome, left greater than right.

WASHING BEHIND THE SEAT OF EQUIPMENT, WITH HIS LEFT LEG BEHIND THE SEAT & HIS RIGHT LEG ON THE RIGHT SIDE OF SEAT, HIS KNEE WAS IN CONTACT WITHTHE BACK OF THE SEAT. HE MOVED JUST A LITTLE & FELT HIS RIGHT KNEE CAP BEING SHOVED DOWN

Employee indicated during a wellness physical that he had been experiencing numbness and tingling.

WHEN EE WAS STEPPING OFF THE DRAGLINE, HE MUST HAVE STEPPED ON A ROCK IN THE SNOW.

AFTER UNLOADING A DRILL BIT FROM THE UTILITY TRUCK, EE WAS GETTING OUT OF THE BED WITH A 3 POINT CONTACT, WHEN HIS LEFT FOOT LANDED ON A BIT AND ROLLED OFF CAUSING HIS RT. LEG TO CATCH HIS WEI GHT AND HIS KNEE GOT BENT TO THE SIDE.

EE WAS REMOVING A HOSE REEL FROM THE REAR OF THE SERVICE TRUCK, SPRAYED THE BOLT WITH 6 IN 1 LUBRICATING OIL TO LOOSEN THE BOLT. HE WAS PULLING ON THE BOLT HE FELT A TWINGE IN HIS LOWER RIGHT ABDOMINAL AREA. HE DID NOT HURT VERY MUCH & CONTINUED TO WORK, LATER WHILE EATING LUNCH IT WAS UNCOMFORTABLE TO SIT DOWN,M AND WASN'T FEELING WELL. HE CONTACTED THE LEADMAN AND WAS TAKEN TO TH

EE STATED HE WAS GETTING OFF THE DOZER AND WHEN HE STEPPED DOWN, HE SOMEHOW TWISTED HIS LOWER BACK.

EE SAID HIS BACK WAS SORE BETWEEN HIS SHOULDER BLADES AFTER MOVING CABLE FOR ABOUT AN HOUR. HE ALSO SAID HE HAD SOME NUMBNESS IN HIS RIGHT ARM. THIS WAS DIAGNOSED AS A CHRONIC DEGENERATIVE CON DITION.

WHILE EE WAS STANDING ON BLADE CLEANING WINDOWS, WHEN HE REACHED TO CLEAN THE BACK WINDOW HE FELT A TWINGE INHIS LEFT HIP.

EMPLOYEE SAID HE SLIPPED ON ICE HITTING OUTSIDE OF RIGHT KNEE, HEAD AND LEFT SHOULDER.

EMPLOYEE SAID HE WAS OPERATING A SCRAPER WHEN ONE WHEEL WAS COMING OUT OF A HOLE AS THE OTHER RAN OVER A ROCK, CAUSING A SIDE-TO-SIDE MOTION. DURING THIS, HE BUMPED THE INSIDE OF HIS LEFT KNEE ON THE STEERING COLUMN.

EE REPORTED INCREASING SORENESS IN HIS RIGHT HAND.

EE WAS PINNED BETWEEN A CREWCAB PICKUP AND A LUBE TRUCK, RESULTING IN A SEPARATED PELVIS.

EE REPORTED THAT HE SLIPPED WHILE CLEANING GREASE INSIDE THE HOUSE OF A DRAGLINE, JAMMING HIS SHOULDER.

EE COMPLAINED OF PAIN INHIS RIGHT SHOULDER WHEN OPERATING A SCRAPER. THIS WAS AN AGGRAVATION TO A PREVIOUS INJURY SUFFERED IN FEBRUARY AT ANOTHER JOB. HE SLIPPED ON SOME ICE AND HURT HIS ELBOW AND SHOULDER.

EE REPORTED THAT HE BUMPED HIS HEAD, JAMMING HIS NECK WHILE CLEANING THE DRAGLINE TUB. AFTER SEVERAL WEEKS, HE WAS PRESCRIBED FOUR WEEKS PHYSICAL THERAPY, PRESCRIPTION DATED 9/6/01.

EE SAID HE HAD BEEN WELDING AND LEANING ON HIS LEFT ELBOW, AND IT STARTED BOTHERING HIM. (BURSITIS)

EE SAID HE STEPPED OFF THE BOTTOM STEP OF THE STEAM TRUCK ONTO UNEVEN GROUND AND TWISTED HIS LEFT ANKLE. X-RAYS VERIFIED A CHIPPED BONE.

EE REPORTED INCREASING PAIN & DISCOMFORT IN HIS LOWER BACK AS A RESULT OF REPEATED TRAUMA. HE HAS BEEN DIAGNOSED WITH SEVERE DEGENERATIVE DISK DISEASE & STENOSIS.

EE SAID HE WAS CHANGING A GREASE REEL ONT HE REAR OF A SERVICE TRUCK. THE REEL GOT HUNG UP, AND WHEN HE REACHED AROUND TO FREE UP A LINE, THE REEL SLID OFF AND LANDED ON THE INSTEP OF HIS RIGH T FOOT.

EE SAID THAT SHE HAS BEEN EXPERIENCING INCREASING DISCOMFORT NUMBNESS, COLD PAIN IN HER RIGHT HAND AND WRIST SOMEWHAT IN HER LEFT HAND/WRIST ALSO.

EE SAID HE HAS BEEN EXPERIENCING INCREASING RIGHT SHOULDER PAIN AND ASKED TO SEE A DOCTOR. PRELIMINARY DIAGNOSIS INDICATES IMPINGEMENT.

EE HAS REPORTED NUMEROUS INCIDENTS RELATING TO SHOULDER PAIN. A VISIT TO AN ORTHEPEDIC SURGEON RESULTED IN REQUEST FOR MRI

EE WAS REMOVING A BOLT FROM A FINAL DRIVE WHEN THE LOAD SHIFTED, PINCHING HIS LITTLE FINGER. SUBSEQUENT X-RAY SHOWED THE FINGER TO BE BROKEN.

EE SAID HE WAS LEAVING THE DRAGLINE, SLIPPED OFFTHE TOP OF THE SHOE, & FELL TO THE GROUND. HE HAD PUT HIS LUNCHBOX DOWN & TURNED AROUND TO GO DOWN THE SHOE STEPS WHEN HE FELL. HE SAID THAT HIS BOOTS WERE WET & MUDDY WHICH CONTRIBUTED TO THE INCIDENT

EE SAID HE STARTED EXPERIENCING A SORENESS IN HIS RIGHT ELBOW AFTER WORKING UNDER A TRUCK, HAMMERING ON A U-JOINT, FOR AN EXTENDED PERIOD OF TIME. HE THOUGHT IT WAS JUST A SORE MUSCLE, BUT THE SORENESS PERSISTED. THE DR DIAGNOSED TENDENITUSOF THE RIGHT ELBOW.

EMPLOYEE SAID THAT HE WAS TAKING THE WATER TRUCK THROUGH THE PIT WHEN HE HIT A BAD BUMP, JOLTING HIS NECK AND BACK.

HE SAID HIS SHOULDERS ARE SORE; SEEMS TO BE CHRONIC, PROGRESSIVE.

EE SAID THAT HE WAS OPERATING THE RUBBER-TIRED DOZER ON LOOSE MATERIAL. WHEN HE WAS TRYING TO BACK UP, THE MACHINE STARTED BOUNCING AND THE SEAT BOTTOMED OUT. HE HEARD SOMETHING POP IN HIS BAC K AND HIS LEFT LEG FELT KIND OF NUMB.

EE SAID THAT ON 6/23, HE WAS WORKING IN THE DRAGLINE WALK BOX. HE WAS TIGHTENING BOLTS IN A PLACE THAT REQUIRED HIM TO WORK THROUGH SPOKES IN THE GEAR - A TIGHT POSITION PLUS WORKING WITH ARMS OUTREACHED, HOLDING A 1" IMPACT WRENCH. HE DEVELOPED PAIN IN HIS ARM, & WHEN HE WAS STILL EXPERIENCING THE PAIN ON 6/26, HE FILED AN INCIDENT REPORT. BY 6/30 HE STILL HAD PAIN & REQUESTED DR.

EMPLOYEE SAID HE INJURED HIS LEFT THUMB WHILE USING A SHOVEL TO PUNCH A HOLE IN A BARREL OF DUST SUPPRESANT, AND ALSO WHILE RUNNING THE 0954 LOADER (THUMB GOT CAUGHT IN THE STEERING WHEEL). TH E INCIDENT OCCURRED JUNE 8, BUT IT WASN'T UNTIL JUNE 27 THAT HE ASKED TO SEE A DOCTOR. DIAGNOSISLEFT THUMB TENDONITIS.

EE SAID HIS LEFT KNEE SWELLED UP AFTER HE HAD BEEN KNEELING ON A GRATE TO WELD A HANDRAIL. DIAGNOSIS AND TREATMENT HAVE BEEN FOR BURSITIS.

EE SAID THAT A LIGHT PLANT THAT HE WAS SETTING UP WAS BLOWN TOWARD HIM AND THAT THE DOOR OF THE PLANT HIT HIS RIGHT KNEE. THIS ALLEGEDLY TORE HIS MENISCUS AND HE HAS BEEN SCHEDULED FOR SURGERY 8-22-00. THIS IS THE FIRST MEDICAL TREATMENT EE WILL HAVE RECEIVED RELATIVE TO THIS INCIDENT.

EE SAID THAT WHEN HE WAS GETTING DOWN OFF THE DRAGLINE (SHOE), HE FELL, LANDING IN A SINK HOLE. HE HURT HIS RIBS ON HIS RIGHT SIDE, AND HIS LEFT KNEE.

EMPLOYEE SAID HE WAS WORKING ON A DOZER TRACK FRAME REBUILD AND USING A HAMMER AND SWINGING LATERALLY. ON A BACKSWING, HE FELT SOMETHING POP IN HIS LOWER BACK.

EE SAID THE HE WAS CARRYING A TRUNION PIN & SOMETHING POPPED IN HIS RIGHT SHOULDER HE SAID THAT HIS SHOULDER WAS WEAK AT THE TIME THIS OCCURRED IT IS LIKELY THAT REPEATED USE OVER TIME CAUSED THE MUSCLE/TENDON TO BECOME FRAYED RATHER THAN A SINGLE INCIDENT BEING ATTRBUTABLE TO THE INJURY

EMPLOYEE SAID HE HAD BEEN EXPERIENCING TINGLING AND NUMBNESS IN HIS HANDS, AND HAD BEGUN TO LOSE STRENGTH IN ONE HAND.
