The steels bowed under pressure then released striking the ee's thumb. This caused a dislocation. The ee went to the hospital where dislocation was reset. He return to work and completed his shift.

The hand grinder jumped across the piece of steel striking the employee's left thumb. The grinding wheel lacerated the employee's lower thumb. The cut required 5 stitches. The guards on the grinder were in place as required.
The EE was prepping steel with a hand held grinder during contstruction of the wash-plant. A piece of steel/rust entered the EE's eye behind his safety glasses. The incident was reported. The EE thought the object was out of the eye. He began to experience irritation the following day. EE went for treatment on 11/24/06 to have object removed. No lost time is expected.

OPERATOR HAD JUST SET MINER OVER ON LEFT SIDE TOCLEAN WHEN LOWERED CUTTING HEAD, THE BITS PICKEDUP A PIECE OF ROCK FROM RIGHT END OF DRUM AND FLIPPED BACK ALONG SIDE OF MINER LANDING ON TOP OF EE'S RIGHT FOOT.
I NAME WAS SITTING ON A FIVE GALLON BUCKET WHILE ROOF BOLTING WHEN BENDING A ROOF BOLT HE SLIPPED OFF THE BUCKET AS HE CAUGHT HIMSELF ON HIS LEFT KNEE HE FELT A SHARP PAIN IN HIS KNEE
THE MAN WAS PULLING A DRILL STEEL OUT OF THE ROOF AND CAUGHT HIS FINGER BETWEEN THE END OF TEH DRILL STEEL AND THE DRILL HEAD.
INJURED WAS STARTING TO DRILL A BOLT HOLE HOLDING THE DRILL STEEL WITH HIS LEFT HAND. WHEN THE STEEL CAME IN CONTACT WITH THE ROOF A SLAB OF ROCK FELL AND MASHED HIS LEFT INDEX FINGER AGAINST THE DRILL POT CAUSING A CONTUSION AND FRACTURE.
HE WAS BEGINNING TO START A DRILL HOLE WITH A ROOF BOLTER. WHEN HE APPLIED THRUST TO THE DRILL STEEL, A SMALL PIECE OF SLATE FELL STRIKING HIS GLASSES, BREAKING THEM AND CAUSING A SMALL LACERA TION BESIDE HIS LEFT EYE.
WHILE LOADING COAL INTO A SHUTTLE CAR THE BOOM OF THE CM14 MINER WAS RAISED INTO THE ROOF, CAUSING COAL TO FLY BACK AND HIT THE OPERATOR IN THE LEFT EYE.
EMPL WAS INSTALLING MULTIPLE PIECE COMBINATION BOLTS IN #1 ENTRY OF 1ST NORTH PANEL ON 001 UNIT . HE HAD 2 SECTION OF BOLT IN ROOF & WAS READY TO THREAD ON 3RD SECTION. HE WAS HOLDING COUPLER OF 2ND SECTION WITH RIGHT HAND & REACHED DOWN WITH LEFT HAND TO ROTATE NEXT SECTION. HE HIT BOOM LEVER INSTEAD OF ROTATION LEVER. WHEN HE DID BOLT JUMPED UP CUTTING THUMB & FOREFINGER REQUIRNG
OPERATOR WAS BOLTING IN FACE OF #2 MAIN NORTH ENTRY DRILL STEEL WAS STUCK IN HOLE OPERATOR SLID FORWARD TO REACH DRILL STEEL TO PULL IT FROM HOLE. WHEN HE LOWERED BOLTER BOOM IT CAUGHT LEFT KN EE CAUSING CONTUSION.
CAUGHT HAND BETWEEN CANOPY AND ROOF WHILE TRAMMING BOLTER
EMPLOYEE WAS INSTAALING A ROOF BOLT IN #8 ENTRY OF 1ST SOUTH MAINS EMPLOYEE WAS PUTTING THE BOLT HEAD ON THE ROTATION POTT TO RAISE BOLT TO THE ROOF WITH THE BOLTER BOOM,WHEN THE BOLT SLIPPED OFF CATCHING EMPLOYEES FOOT BETWEEN THE BOLTER BOOM AND ROOF BOLT.
I NAME WAS PUTTING IN 4 SECTION POINT ANCHOR ROOF BOLTS HE WAS PUSHING UP THE FIRST 2 SECTIONS WITH HIS R HAND WHEN THE COUPLER ON THE BOLT CAUGHT ON THE ROOF THE IMMEDIATE STOP STRAINED A TEN DON IN HIS WRIST RESULTING IN TENDONITIS
I,NAME, THE BOL7ER OPERATOR HADJUST FINISHED INSTALLING A ROOF BOLT AND LAID HIS DRILL STEELS IN THE STORAGE TRAY. HE STEPPED TO THE SIDE OF THE BOLTER TO LOWER THE CANOPY. WHEN HE LOWER THE C ANOPY IT CAUGHT3A DRILL STEEL HANGING OVER THE END OF THE TRAY. THE DRILL STEEL FLIPPED UP STRIKING HIM IN THE RIGHT TEMPLE. THE CUT REQUIRED 5 STITCHES.
I NAME WAS INST5LLING MULTIPLE SECTION CONVENTIONAL BOLTS IN THE FACE OF THE #4 ENTRY HE WS HOLDING THE TOP SECTION OF THE BOLT IN HIS LEFT HAND THE BOTTOM SECTION IN THE CHUCK WHEN HE REACHED FOR THE ROTATIO3 LEVER HE MISSEDIT AND HIT THE BOOM LIFT WHEN THE BOOM RAISED IS SMASHED HIS INDEX FINGER THE INJURY REQUIRED 3 STITCHES
