While changing the cutter drums on a continuous mining machine, the employee got hand caught between the impact gun and a crib block bruising the left index finger.
EE was using a torch to cut bracket to change water hose,accidently cut hydraulic hose and threw oil in his face and hands (burns).
EE was bolting top and dropped the head of the pinner down to remove the drill steel, when the drill steel came back and struck the EE on the right index finger. The finger was broken in two places.
He was installing curtain boards and when he was removing drill steel, he rotated the steel and the drill steel hit his forearm.
EE was rock dusting Friday, Aug 31, 2007. He injured his left shoulder draggng the rock dust hoses. He finished his shift. After Labor Day weekend, he returned to work Tuesday. His shoulder still hurt. He did not work Thurs or Friday. When he reported Monday, he reported incident & sent to Dr. He was told he had a pulled muscle in shoulder & returned to work 9/11
While he was loading supplies, the roof bolter advanced to the nex row of bolts and trammed over left foot with rear tire.
Miner boom contacted shuttle car, lifting it up and then the shuttle car slammed to the mine floor. It occurred in sub mains, break 27, approximant location.
Two employees were operating twin-head roof bolter when injured's hand became caught between coal rib and metal strap being installed. Strap was being raised to mine roof by other operator.
While bolting top, employee was drilling the top with a 4'drill steel. The drill steel bowed in the middle and spun around hitting the EE in the right arm.
While operating roof bolt machine, injured raised canopy against mine roof (uneven top) with too much pressure applied, causing part of canopy to break loose, fall, striking victim, causing injuries.
Spot bolting the top with a single head bolter when his left hand was caught between the jack & the arm of the bolter. The end of the left index finger was pinched off.
Employee was operating a drill with jack leg attachment, jack leg failed to release, employee was attempting to release jack leg when employee slipped, employees (right) foot was between two (2) rocks, when employee fell and injured ankle.
The injured employee pulled roof bolter up then swung head of bolter out to put up outside bolts. swung head back in, drilled hole, put bolt & glue in hole. Employee set the head of the bolt in the chuck, hit the rotation control to set bolt down in head. Employee's glove had went in hole with bolt, finger was twisted with bolt.
Victim finished bolting (corner bolt). He reached over to let canopy down but hit swing lever by mistake. Bolter moved left hitting his left knee - hyper-extending his knee.
WHILE OPERATING ROOF BOLTER THE EMPLOYEE GOT HISLEFT HAND HUNG BETWEEN CANOPY & DRILL STEEL. THEDRILL POT SMASHED THE DRILL STEEL DOWN ON HIS HAND AGAINST THE CANOPY.
EE WAS PINNING TOP ON 3-20-00 AND A PIECE OF DRAW ROCK FELL AND HIT HIM ACROSS SHOULDERS AND BACK. EE CONTINUED TO WORK THROUGHOUT THIS PARTICULAR SHIFT AND UP TO 4/13/00 WHICH WAS HIS LAST SH IFT WORKED. ON 4-14-00 EE WENT TO THE DR. BECAUSE OF CONTINUED PAIN FROM THE ROCK THAT STRUCK HIM ON 3-20-00.
