The roof bolter stood on the head of bolter to insert resin and bolt in a 9 ft high area. He asked another operator to raise wrench up with head to push the bolt into roof. After pushing the bolt up, the assisting operator began lowering the head. The victim's foot which was still on the drill head frame became pinched between the frame and hydraulic fitting of the machine.

The miner's right forearm was injured by broken glass and he received a total of four sutures to close the two lacerations.

The man was drilling a bolt hole into the roof near the rib to install a roof bolt. While lowering the drill head, a corner of the drill head struck the man on the upper leg.

The man was lowering the roof bolter drill head to remove drill steels from the roof. The man reached for the drill steels before they had stopped spinning. His glove became caught on the drill steel coupler and twisted his hand.

The man was inserting a roof bolt in the roof. The bolt was bent to be able to insert into the roof. While pushing on the bolt to straighten it, the bolt spun around causing the man's wrist to twist.

The man was bending a roof bolt and installing the bolt in the roof bolter chuck. While straightening the bolt, the plate slid down the bolt and pinched his finger between the plate and chuck.

The man was roof bolting when a drill steel stuck in the roof. He put his hand over the drill head when the steel fell out of the roof and struck him on the hand.

The man was lifting belt structure with a lifting jack. While lowering the jack, the man's hand slipped off the handle and the handle struck him on the lip. Report filed for medical treatment only(Stitches)

The man was pushing a 6 ft resin bolt into the drill hole when he felt a pain in his shoulder.

The man was cleaning coal off the bottom with the continuous miner just outby the cut that was partially mined. As he was cleaning and positioning to enter the left side of the cut, the right side of the tail contacted him in the hip area.

While working as a roof bolter operator, the man stated that he developed pain in his back.

The man was attempting to separate 2 drill steels that were stuck together. As he was raising and lowering the drill head to loosen the drill steels, he twisted his wrist.

The man was roof bolting when a drill steel became lodged in the roof. As he was attempting to dislodge it, the drill steel fell from the drill steel hole and struck the man on the thumb. Report filed for medical treatment only.

The man had his hand on the boom arm of the roof bolter and as the boom raised he caught he finger between the boom arm and canopy.

The man was tramming the roof bolter out of a working place from the operator's compartment. The man had his arm resting on the corner of the compartment. As he turned the machine, it moved close to the rib and he pinched his arm between the machine and the rib.

The man attempted to remove by hand a drill steel stuck in the mine roof without success. While preparing to add a second steel into the bolter chuck to raise into the stuck steel, the man inadvertently raised the drill head and caught his finger between the chuck and the drill steel.

The man was tramming the continuous miner to prepare to mine coal. He went to side ofminer to move the miner cable. While moving the cable, the machine moved slightly as he moved the tram controls. The man pinched his hand between the remote box and rib as miner contacted remote box.

The man was lying under a feeder burning with a torch when a piece of slag went into his ear. The man worked in regular job until 12-1-09 when he reported off.

The man was putting the drill steel in the roof bolter chuck when the drill steel broke and struck the man on the hand.

EE started to walk along side of the roof bolter while the machine was being trammed and the EE got his leg caught between the rear of the machine and the rib.

Employee was burning with a torch when a sprak went in his ear. He continued working until 7/24/09 when he missed a day of work.

The man was operating a Hilti-Drill to drill a hole to set a belt head. His finger struck the drill bit, lacerating his finger. The man received stitches in his finger. Report filed for medical treatment only.

A bolter drill steel popped out of the drill head, bounced back, striking him in his face.

the man was installing a roof bolt near the rib. As he lower the head of the bolter, it struck his right thigh.

The man was attempting to remove a drill steel from the roof when he pinched his finger between the drill head and the roof.

The man was knocking a used drill bit off of a drill steel when the bit bounced off the canopy and down his shirt collar.

The man was standing behind the roof bolter. When the cable reel was started, the cable struck the man on the leg and knocked him down.

The continuous miner was loading into a shuttle car. A piece of rock bounced off the shuttle car and into the crosscut and struck the man on the leg. The man continued to work until 10/15/08 which was his first day of work.

A bolter drill steel broke striking the employee on his left temple requiring a couple sutures.

He was sitting in the shuttle car while it was being loaded. A piece of rock flew from the tail of the miner and struck the man on the chest.

The man was drilling a hole in the roof at an angle when the drill steel coupler broke. The drill steel struck the man on the lip.

The man was cutting bottom with the continuous miner, when a piece of material flew back and struck the man on the safety glasses. The material damaged the glasses and embedded in his right eye.

The man was installing a roof bolt and while turning and reaching to activate the rotation control, he felt a pain in his lower back.

The man was installing a roof bolt when it bent, spun around, and struck the man on the right hand.

The man was lowering the stab jack when his left foot slipped and was struck by the jack. Medical treatment only.

When taking out drill steels from the roof, two 3" steels fell out striking the employee's hand against the bolter.

When installing a 6' roof bolt, the bolt spun around striking the employee's tooth.

Injured EE was holding onto drill steel with up pressure on head. He was attempting to drop head to change bit end. Steel broke in 2 pieces. Top part remained in roof & other part in drill head. Upward pressure abruptly raised to roof injuring thumb.

EE was reaching into the automatic dump. The flap fell shut on his fingers, crushing them.

Employee got his finger caught between boom and frame of bolter when he raised the drill head to the bolter. Requiring 12 stitches.

DUE TO LOW CEILING HEIGHTS, INJURED EE HAD 2 BENDS IN 4' RESIN GROUTED ROOFBOLTER. UPON UTILIZING DRILL HEAD UP PRESSURE TO ASSIST BOLT INSTALLATION, ROOF BOLT BEARING SLIPPED OFF DRILL HEAD STRIKING EE'S GLOVED RIGHT HAND.

EMPLOYEE WAS STRUCK ON HIS FACE BY DRILL STEEL THAT WAS STUCK IN DRILL HOLE. STEEL SUDDENLY DISLODGED, STRIKING EE & CAUSING A LACERATION TO HIS LEFT CHEEK, REQUIRING 7 SUTURES FOR CLOSURE.

INJURED PERSON WAS INSTALLING A RESIN GROUTED ROOF BOLT WHEN HE INADVERTENTLY PINCHED TWO FINGERS OF HIS LEFT HAND BETWEEN THE ROOF BOLT BEARING PLATE AND THE ROOF. HE SUSTAINED OPEN FRACTURES OF THE MIDDLE AND RING FINGERS ON HIS LEFT HAND WHICH WILL REQUIRE HIM TO LOSE TIME FROM WORK.

While operating roof bolter employee dropped drill boom on right knee causing large bruise with contusion.

WHILE REACHING BEHIND ROOF BOLTING MACHINE TO MOVE TRAILING CABLE. HIS LEFT HAND WAS PINCHED BETWEEN BOLTING MACHINE AND RIB. HE RECIEVED INJURY REQUIRING 4 STITCHES

EMPLOYEE WAS LOWERING CANOPY JACK TO FACILITATE SLIDING OF METAL ROOF MAT ONTO DRILL HEAD WHEN HIS FINGER WAS PINCHED BY TELESCOPING CANOPY JACK SEGMENTS.

Continous Miner was tramming in reverse direction. Injured employee reached to move cable/h20 line from rear of machine. Finger was caught between miner and line post.

While drilling hole for roof bolt EE got piece of foreign object in eye. Had to get medical treatment with prescription eye drops.

Ee was fitting pipe, while moving with torch hose became tangled causing lit end to enter glove burning wrist/arm

When drilling a bolt hole a piece of rock split and fell from the roof striking the victim's right thumb on the bolter head.
