While backing up roof bolter, cable reel struck employee in the right elbow. Resulted in a pulled muscle and chipped elbow.

Employee received flash burns to both eyes while welding on a furnace.

Employee was welding and a piece of metal flew into his right eye. Pain and welding.

Employee was roof bolting when he caught his left thumb between the mine top and the drill chuck on the roof bolter.

A loose bolt came loose from the dewatering vibrator and hit employee on the left thumb. Thumb was swollen.

Ee is a dozer operator. He back over a rock that was not in his eyesight causing a jolt of the machine. This jolt caused him to be jarred severely causing a hurt back. He missed two days and is not back to work with very little problem.

Ee was bolting. A piece of drill steel got loose and hit him in the mouth, knocking out three teeth. Other than needing a partial denture, Ee is in good shape. The dentist allowed him one day off.

While drilling a hole for a roof bolt, the drill steel hit EE in mouth. This cut his lip and chipped his teeth. Stitches fixed his lip and a dentist took care of his teeth. Missed one day of work. Back to full duty with no problems.

EE operates a roof bolter. His hand got caught in canopy when it came down. He simply had his hand in the wrong place at the wrong time, and he knew it. It mashed his thumb so much that it broke it. Will be off a two or three weeks.

While operating the roof bolter, Ee got his hand in between boom arm and drill pot. This pinched off the tip of his middle finger on his right hand.

EE caught his finger in between auger and auger barrel and pinched his finger to the point of cutting it. No bone broken. Six stitches required. He did not miss any work.

Ee operates a roof bolter. He caught his finger in between head and arm of the bolter. This pinched his skin to the point of looking like a cut. Three stitches required. No lasting problems.

Ee was operating a grader. He was going up hill when the engine stopped running. The grader then took off backwards down the hill. Ee jumped off. He received only minor injuries, in fact only very minor brusies.

Ee was replacing a section of 6" pipe when a section of it fell on his foot.

A spring rod assembly came apart and a piece of it hit Ee on the ankle.

Ee was using a cutting torch when a hot piece of slag rolled into his boot. (He was wearing an open type boot.) By the time he got it off, his leg was burned badly.

Ee was using a cutting torch. He was overcome with fumes. Apparently the area was not well enough ventilated.

EE was operating a lathe, when his emory cloth got caught in the lathe and pulled his hand into the turning lathe. This broke one finger and dislocated two others.

Ee slipped while roof bolting and twisted his knee.

Ee pinched his finger between the drill steel and chuck on a roof bolter.

Employee was tramming up to get last 2 bolts on left side of cut when the pinner wedged his hand to the rib.

Employee was putting up roof bolts. The bolt bent and mashed his thumb.

EMPLOYEE WAS HELPING THE REPAIRMAN TO WORK ON A BUCKET. EE BEGAN WELDING ON THE BUCKET BEFORE HE GOT HIS HOOD INTO THE CORRECT POSITION CAUSING "WELDERS FLASH BURN TO THE EYES".

EMPLOYEE WAS DRILLING INTO SIDE OF WASHER WHEN DEBRIS BLEW INTO HIS EYE. EMPLOYEE DID NOT WEAR SAFETY GLASSES BUT GLASSES WERE PROVIDED. ACCIDENT WAS REPORTED TO OUR COMPANY ON 10/22/04.

EMPLOYEE WAS BOLTING THE TOP WHEN A PIECE OF STEEL FELL OUT OF THE HOLE AND HIT HIS LEFT HAND CAUSING A BRUISED HAND AND A SMALL CUT WHICH TOOK A FEW STITCHES.

EE IN MINER OPERATOR. HIS LEFT HAND WAS RESTING ON THE FRAME OF THE MINER WHEN A PIECE OF ROCK FELL FROM BETWEEN ROOF BOLTS AND HIT IT. EE WAS CUT AND HAD TWO BONES CRACKED. EE WAS NEVER IN DI STRESS. HE DROVE HIMSELF TO THE ER FOR TREATMENT.

WHILE REMOVING ROCK THAT HAD FALLEN ON CONTINUOUS MINER, A PIECE FELL & HIT EE ON RIGHT HAND. THE MIDDLE FINGER ON RIGHT HAND WAS DISLOCATED & THE LAST DIGIT WAS BROKEN. EE CONSIDERS THIS A MI NOR INJURY. HE TOOK ONE DAY OFF & PLANS TO BE BACK AFTER VACATION.

EE WAS OPERATING A ROOF BOLT MACHINE AND CAUGHT HIS HAND BETWEEN THE ROOF BOLT PLATE AND ROOF OF MINE.

EMPLOYEE WAS OPERATING THE ROOF BOLT MACHINE & STRAINED HIS BACK.

EE TWISTED HIS LEFT WRIST WHILE DRILLING WITH ROOG BOLTER.

WHILE DRILLING BOLT HOLE, EMPLOYEE GRABBED HOLD OF DRILL STEEL TWISTING HIS LEFT WRIST.

EMPLOYEE WAS IN THE PROCESS OF BOLTING THE ROOF TOP WHEN A PIECE OF DRAW ROCK FELL FROM ROOF ONTO EMPLOYEE'S LEFT LEG.

EE WAS IN PROCESSOF ROLLING CONVEYOR BELT WHEN THE BELT CHAIN SLIPPED AND HIT EE ON HIS RIGHT KNEE.

EE WAS IN PROCESS OF DRILLING A ROOF BOLT HOLE WHEN A PIECE OF DRAW ROCK FELL FROM ROOF STRIKING EE ON RIGHT HAND.

WHILE BOLTING THE TOP, THE EMPLOYEE WAS LETTING THE JACKS DOWN ON THE MACHINE AND CAUGHT HIS HAND BETWEEN THE BLOCK AND LEVER ON THE ROOF BOLT MACHINE.

THE INJURED WAS SPREADING ROCK ON THE SLATE DUMP WITH A PIECE OF HEAVY EQUIPMENT WHEN HE BACKED INTO A ROCK/DIRT BANK AND PULLED A MUSCLE IN HISNECK.

A ROOF JACK FELL FROM THE ATRS ON THE ROOF BOLYTER STRIKING THE INJ ON THE RIGHT SHOULDER, FRACTURING THE SHOULDER

THE INJ WAS STANDING ON THE RIGHT RIB HELPING THE MINER OPERATOR WHEN THE RIB ROLLED AND CAUGHT HIS LEFT FOOT, BREAKING A BONE IN THE FOOT

WHILE INSTALLING 3/4" CHUCK ROOF BOLTS THE INJURED STRAINED HIS RIGHT SHOULDER.

A ROCK FELL BETWEEN THE CAMOPY AND TIE BAR, STRIKING THE INJURED ON TOP OF THE RIGHT FOOT, CUTTING THE FOOT.

THE EE WAS OPERATING THE SCOOP WHEN A COAL BUSRT OCCURRED, STRIKING HIM.

THE EE WAS OPERATING THE CONTINUOUS MINER WHEN A COAL BURST OCCURRED & STRUCK HIM.

THE INJURED PUT A PIECE OF STEEL INTO THE TOP WHILE ROOF BOLTING AND A PIECE OF ROCK FELL, HITTING HIM ON TOP OF THE RIGHT FOOT. CRUSHING MUSCLE AND TISSUE.

INJURED WAS BOLTING THE TOP WHEN A DRILL BARREL FELL OUT OF THE HOLE, STRIKING HIM ON THE RIGHT HAND AND CUTTING HIS THUMB.

WHILE BOLTING THE TOP, THE INJURED DEVELOPED PAIN IN HIS RIGHT SIDE.

WHILE ROOF BOLTING, A ROOF BOLT FELL OFF THE BOLT MACHINE, STRIKING HIM ON THE RIGHT FOOT.

WHILE BOLTING THE TOP, A SMALL PIECE OF DRAW ROCK FELL FROM BETWEEN THE STRAPS, STRIKING EE ON THE NOSE AND RIGHT INDEX FINGER.

WHILE INSTALLING A ROOF BOLT, THE BOLT BENT, CATCHING HIS RIGHT LITTLE FINGER BETWEEN THE BOLT AND PLATE.

THE EMPLOYEE WAS RUNNING THE ROOF BOLTER AND HE MASHED THE TIP OF HIS FINGER OF HIS RING FINGER, ON THE LEFT HAND, ON THE FRAME WORK OF THE ROOF BOLTER.
