As part of a routine inspection of a cyclone, the overflow pipe was removed & being reinstalled. The pipe was suspended by an overhead crane; another mechanic was pushing on the pipe to align the bolts. The pipe hung up, causing it to bounce. When it bounced, the nylon strap broke, & pipe fell, striking the mechanic's right hand between a pry bar & the overflow pipe flange.

Tire service tech, operating a IMT tire service crane/manipulator. Pick up a 3300-51 haul tire in a scrap pile. Lifted tire approximately 6'ft, tire slipped out of tire clamp, impacting the ground and rolled onto service tech operating controls. Pinned by weight of tire, causing several broken bones. Wet weather..

Employees were replacing teeter tubes inside the hydrosizer. There were filings left inside tubes from drilling out nozzles. While lowering teeter tubes filings fell onto employee getting into left eye. Employee flushed eye and thought debris was cleaned out. On 5/31/18 employee was taken to the doctor and a small fragment of metal was removed.

Employee reported to work on 6/23/16, knee was swollen so employee left to see doctor. Employee was admitted into the hospital with a staph infection. Employee had some old welding burns on arm and possibly one on leg. It's unclear as to what caused the infection and waiting for doctors report.

Employee was using a hand-held 9" grinder to reduce thickness of a metal plate. During the activity, the grinder "kicked" back, w/ the rotating wheel striking the employee on top of the left knee. Employee suffered a laceration/cut approximately 3" long and approximately 0.25" deep. Employee was taken to Urgent Care facility where wound was treated and closed w/ seven stitches.

Employee was Using an air arc, had face shield and safety glasses on. Employee lifted shield, had not pushed the air release button on gouging torch which blew debris into the air. Employee did not feel anything in the eye until that night and did not seek medical attention or report until the next day. The eye doctor removed a small particle of rust from the eye.

Employee was using a saws-all to cut a 2x6 stud with a plywood sheet attached to the back. The saw kicked back when it hit a knot or a screw in the wood, cutting left 5th digit and requiring stitches.

EE was arc gouging 09/05/2009. He said about 9:00PM his right eye felt like there was something it. About Midnight he woke with tearing and pain in his right eye and went to the ER. He did not contact his supervisor because he didn't think it was work related until the ER Dr. told him his eye suffered from arc flash. EE reported the incident by phone 09/06/2009.

Employee was using a drill press to drill 1-1/2 inch hole, bit was slipping so employee was using a chuck to tighten. Employee stepped to the right to get a better position and hit the foot actuator, turning the drill bit and chuck. Employee had two stitches in hand as a result.

Employee was using a pressurized grease system on 2085 lube truck to get a small amount of grease to put on the o-ring on a hyd. filter while servicing a D10 dozer. The grease system injected grease through the gloves and into the right hand, 3rd joint of middle finger.

Employee was using a hydro-blaster to unplug a sanded hydrosizer. The hydro-blaster removed most of the sand that plugged the discharge. Employee pulled the hydro-blaster hose out to realign the nozzle with the discharge line. As he repositioned his hand on the hose, the discharge struck the outside edge of his left forefinger cutting his finger.

On 05-11-2008 employee reported to his supervisor that he noticed an irritation in his left eye, he thought it was from the work he was doing the day before (05-10-2008). Employee stated he had been grinding and when he removed his grinding goggles he felt something get into his left eye. Employee was taken to see a doctor.

On 3/10/08 employee reported to his supervisor that he noticed an irritation in his right eye 3/6/08. 3/7/08, he went to see a Dr. Dr removed a piece of metal from his right eye. Employee stated that he doesn't know for sure when incident occurred he thinks it was related to the work he was doing on 3/5/08. Employee was working in the mine shop area using a torch and a grinder.

On 12/31/07 Employee was pulling and pushing a head mechanism on a drill press across discharge manifold flanges. This was done so that the flange faces would be the same length; he worked at this task for about 3 hrs. Employee reported he did not have any pain or soreness until the next day. On 01/02/08 he came to work and reported that he felt like he needed to see a doctor.

Employee became entangled in the rotating drill steel on the air track drill when he left the cab for some unknown reason.

Mechanic was using a wire feed welder when he missed and the wire went through the seam of his welding glove and hit his finger. The finger later became infected and the individual had to have his finger lanced and be treated with IV antibiotics on 12/26/2006.

Employee claimed he was driving motor grader and hit a rock. He did not report accident until next day after he went and saw a chiropractor for neck and shoulder discomfort. A Workers Compensation claim was made 8/7/07.

Employee was being loaded by an excavator when a large rock rolled in the bed striking the side of the truck bed. Employee was bounced around in the cab causing possible whiplash.

EMPLOYEE WAS WALKING BACK TO LOADER WHEN MAINTENANCE EMPLOYEE ASKED HIM A QUESTION. EMPLOYEE TURNED TO ANSWER WHEN PORTABLE LIGHT PLANT FELL ONTO HIS NECK/SHOULDER. EMPLOYEE WORKED A FULL DAY ON 3/17/06 AND 6 1/2 HRS. ON 3/18/06 BEFORE GOING TO DOCTOR.

Employee was sitting in cab of haul truck being loaded by excavator when a large rock rolled off the bucket, hitting headache rack on the truck. Jarred their back.

EE was prepairing metal for welding. The grinding wheel caught on the edge and sprayed paricles in the left eye. The EE was wearing a facesheild and safety glasses.

Employee removed the top cover on the radiator housing to see why the cap was leaking. When the Employee reached to remove the cap it blew off spraying the employee with hot antifreeze.

EMPLOYEE WAS CHANGING TRACK PINS ON A CAT D-11 DOZER. A PIN WAS PRESSED OUT OF THE LINK. THE RAIL HALVES WERE STUCK TOGETHER BY A SMALL LIP ON THE INSIDE BORE OF THE RAILS. EE WAS HOLDING A SMALL HYDRAUULIC RAM BETWEEN THE RAILS TO SPREAD THEM APART. WHEN THE LINKS SEPARATED EE'S THUMB WAS PINCHED BETWEEN THE TRACK & A TRACK ROLLER.

EE was operating a dozer in rough terrain all day, causing stiffness in lower neck and upper back.

Employee was removing snow from the mine haul road with a motor grader and hit a rock jarring lower back.

EE HIT A LARGE ROCK BURIED IN MUD WHILE BLADING HAUL ROAD. WHEN HE HIT THE ROCK, HE SUFFERED AN INJURY TO THE LOWER BACK.

AIR COMPRESSOR WAS BEING CHECKED FOR FUEL LEAKS.EE HAD LEFT HAND ON ENGINE FRAME WHILE CLIMBING.HE SLIPPED & LEFT HAND WENT INTO FAN CUTTING RING FINGER. SURGERY REQUIRED TO REPAIR TENDON.

AFTER WELDING ON A PIECE OF EQUIPMENT, INDIVBIDUAL WENT TO HAVE LUNCH AND NOTICED SOMETHING IN EYE. A FOREIGN OBJECT WAS REMOVED FROM EE'S RIGHT EYE AT HOSPITAL.

EE WAS CUTTING SCRAP STEEL. A PIECE OF STEEL PLATE THAT HE HAD CUT LOOSE FELL ONTO HIS RIGHT FOOT.

EE WAS STEAM CLEANING CRANE AND A COVER WAS LEANING UP AGAINST CRANE, WHEN HE TOUCHED COVER, THE BOTTOM WAS HINGED AND FELL ON HIS LEFT SHIN.

EMPLOYEE WAS REPLACING A DOOR ON 21210. IT REQUIRED GRINDING A RIVET OFF. A LIVER OF ALUMINUM BECAME EMBEDDED IN HIS RIGHT EYE.

GRINDING A WELD - RESULTED IN FOREIGN OBJECT IN RIGHT EYE.

REMOVING BALLMILL LIFTERS WITH A 1" IMPACT WRENCH

EE WAS BEING LOADED BY 11260 SHOVEL, A ROCK FELL OUT OF SHOVEL BUCKET & HIT ON THE BACK OF TRUCK BED, CAUSING TRUCK TO BOUNCE & JAR THE DRIVER.

EE WAS OPERATING #15830 WHEEL DOZER WITH THE DOOR OPEN. HE HAD HIS HAND ON THE DOOR JAM, THE DOOR SWUNG SHUT AND SMASHED HIS LEFT THUMB.

RUNNING MANLIFT FOR ANOTHER EMPLOYEE WHO WAS WELDING ON AN OVERHEAD. WELDING FLASH BURN TO THE EYES.

SLAG IN RIGHT EYE. EE WAS WELDING ON A TRUCK IN A CONFINED SPACE.

HE WAS USING A 6" GRINDER TO GRIND A WELD OFF OF AN "I" BEAM. THE GRINDING WHEEL CAUGHT AND TWISTED THE GRINDER OUT OF HIS ELFT HAND. THE WHEEL CUT THE UNDER SIDE OF HIS LEFT WRIST.

WHILE FUSING SCRUBBER LINE PIPE, HE HIT HIS LEFT HAND WITH CHIPPING HAMMER BETWEEN THUMB FOREFINGER.

CLEANING UP IN WAREHOUSE, LIFTING A BEARNING ASSEMBLY WITH THE OVERHEAD CRANE. BEARNING ASSEMBLY SLIPPED AND PINCHED RIGHT HAND BETWEEN ASSEMBLY AND MOTOR RESULTING IN A SMASH RIGHT RING FINGE R.

WORKING WITH GRINDER

GRINDING A WELD ON NEW HAND RAIL WHEN PARTICLE OF METAL WENT IN RIGHT EYE.

CLEANING HOT ORE OUT OF TOWER, HOT ORE BROKE LOOSE AND SLID DOWN CHUTE, CAUSING FIRST AND SECOND DEGREE BURNS TO BOTH LEGS FROM THE KNEE TO FOOT.

WORKING ON ED-33 THE SOCKET ON THE AIR IMPACT WRENCH CAME OFF AND HIT LEFT FRONT TEETH.

SEAT KEEPS BOTTEMING OUT LOWER & UPPER BACK.

JARRING OF SHOVEL RESULTED IN SPRAINED BACK
