As part of a routine inspection of a cyclone, the overflow pipe was removed & being reinstalled. The pipe was suspended by an overhead crane; another mechanic was pushing on the pipe to align the bolts. The pipe hung up, causing it to bounce. When it bounced, the nylon strap broke, & pipe fell, striking the mechanic's right hand between a pry bar & the overflow pipe flange.

Mechanic was working overhead installing a starter on a dozer when the screwdriver slipped from the mechanic's grip and fell striking the mechanic in the tooth. When the mechanic went into the dentist on 10/1/24 it was determined that the tooth was cracked and needed repair.

Operator was dismounting a D-11 dozer and stepped on a rock causing ankle to roll resulting in a sprained ankle.

While standing and tightening the brake bleeder valve on a haul truck, the tech lost balance. The tech reactively stuck a hand out to catch self from falling and hit a knuckle on the accumulator bracket which caused a laceration

Employee set down trusses EE moved w/telehandler & was removing the slings when felt a pop in EE' arm. The next day examined by PA who set lifting restrictions. Employee was off work for holidays and returned to work on 1/5-1/8 w/restrictions. On 1/9 MRI showed torn muscle requiring surgery. Surgery was performed 1/11/24 @ which time injury changed from Restricted Duty to Lost Tim.

Employee was removing an air starter motor from a 785 CAT haul truck, when oil started pouring out of the starter mount. Employee quickly leaned to the left, while still holding the air starter, and smashed left ring finger between the starter motor and the truck.

Employee was lifting rubber mill liner segments off forklift & placing on liner machine to install inside the mill. Employee felt EE's forearm pop. Employee notified supervisor & was taken to ER. Employee was released to return to work w/further exams scheduled later. MRI results showed torn bicep requiring surgery. Employee missed first scheduled shift on 6/1/23

When employee reached down to open door to MCC the door handle broke off in employee's hand lacerating the palm of the right hand. The cut to the hand required 2 stitches.

Mill Maintenance Mechanic was rebuilding a 12" Knife Valve. While re-aligning the valve plate, the piston slipped down and impacted EE's left thumb causing a laceration that required 4 stitches.

A mechanic disconnected a hydraulic hose from the final drive on a CAT 785 haul truck and placed one end up on the frame. As EE was putting plastic in place to cover the final drive, EE bumped the hose. It came off the frame and struck EE in the mouth. The impact from the end of the hydraulic hose cracked EE's upper right canine tooth.

While repairing A/C on a 777 CAT Water Truck a mechanic reached down between seats to check air flow, as EE was reaching down EE stumbled and EE's fingers entered the A/C intake and contacted the fan. Mechanic was taken to ER where finger tip was stitched up.

Maintenance was replacing pads on a dozer track. While trying to position the track, one worker struck the track with a hammer. A piece broke off & struck another mechanic in the leg. A metal shard embedded in the leg above the knee. After attempting to remove the shard, MDs left it in place, mechanic was treated w/antibiotics & returned to work on next shift.

On 1/15/2020 a mine mechanic slipped on the ice as the mechanic stepped out of a pickup. Initial medical diagnosis was a slight knee strain and the mechanic was allowed to return to work without restrictions. On 1/22/2020 the mechanic was seen by another doctor and it was determined that an MRI was needed for possible surgery and the mechanic was not allowed to return to work.

On 12/26/19 warehouse-person picked up a box and was putting it on a cart and felt a pain in back. This was reported to supervisor 12/30/19 because pain continued. Upon reporting employee was taken to local clinic. Diagnosis was a back strain & restricted for 2 weeks to lifting <10 lbs. Employee was placed on restricted duty.

While working to replace roof structure a rafter failed causing an employee to fall resulting in a broken arm.

Employee was removing an 8' section of 8' poly pipe from an elevated position on the reject stacker at the SAG mill. The section of pipe dropped a short distance while EE was trying to support it with the arm overextending EE's right arm tearing/detaching the bicep tendon. Results of 6/7/18 MRI revealed the surgery would be necessary.

Employee developed a skin rash/irritation after contacting epoxy on the skin of chin and wrist. Initial contact to the skin occurred on 5/11/18, employee noticed rash a couple of days later. Eight days after the initial contact the employee reported the illness to the supervisor on 5/19/18 and sought medical attention.

Employee had just climbed off of the bulk truck and slipped and tripped over some round grinding balls and fell to the ground injuring EE's right shoulder.

Employee was changing bit on blast hole drill. When EE reached to straighten the bit to connect it to the drill steel EE hit right little finger on a sharp edge of the breakout wrench. The sharp edge cut through the glove and lacerated EE's finger. The laceration required 4 stitches.

Employee needed to reposition the bit basket in the drill deck. Employee bent over and grabbed the bit basket with the left hand and pulled up on the bit basket. Employee felt a pop and an "electrical shock" sensation go up the left forearm. Injury to tendon in left forearm.

Employee was lifting a pipe fitting with a come-along when the pipe shifted causing handle to spine striking employee in the nose. Nose was lacerated and broken.

Contract employee was swinging a sledge hammer to drive in a pin when they smashed a finger between the hammer handle and a hydraulic hose. Finger required 27 stitches to close.

Mill Mechanic was moving a small (30 pound) electric hoist to another pick point. As ee lifted the hoist ee felt pain in back.

Employee was assisting a mechanic in moving final planetary out of a press. Employee held the hook on the lifting sling while the mechanic took up the slack in the sling with overhead crane. When the slack came out of sling employee's left index finger was caught between the hook assembly & planetary housing. This resulted in lacerating of left index finger requiring 5 stitches

Employee tripped on pallet and forklift forks falling to the ground and landing on right arm/wrist, fracturing right wrist.

Employee was working in the SAG Mill changing lifters and liners. Employee was lifting a Sag Mill lifter and felt sharp pain in EE's lower back. Employee was taken to receive medical care/evaluation 01/14/2016. Received notice 01/20/2016 that employee is unable to return to work at this time.

Employee had finished lowering boosters down a blast hole and turned to walk to the next blast hole when he tripped over a rock and fell. The workers compensation insurance has been investigating this case to determine if the injury is work related. On 02/09/2015 we received notice that this incident is determined to be work related.

As the employee was moving two 12 inch pipe elbows by hand he got his left ring finger between two pipe flanges and the end of his finger was cut. Employee was wearing leather gloves.

Employee stepped back off of a 2 foot step ladder and lost his balance causing him to fall backwards. The employee instinctively stuck out his right hand to break the fall and struck a tool cart that was placed in the area. The impact caused a laceration to the palm of the right hand. Employee did not fall all the way to the floor.

Employee was helping replace a section of 14" diameter by 55 long Drisco pipe that ran out through a wall and down into a surge tank. One bolt had been installed in the flange. The pipe was being rotated to make alignment of the holes. The pipe turned and rolled around and pinched employee's right middle finger between a 2" steel water line and the Drisco pipe.

Employee needed to repair a broken wire on a D-11 dozer that was located under cab floor. As employee was in this area he pushed against a hydraulic line, this caused dirt to fall from the hydraulic line onto his forehead and down on the inside of his safety glasses. Some foreign material got into the employee's right eye.

Two employees were working between two buildings. While facing one building Ice slid off the roof behind and struck one of the employees on the back of right shoulder. The uninjured employee stated they had looked at the roof edge and saw no overhanging ice or water dripping and they made the determination that the area was safe to work in.

Employee, an SOS contract employee working at Simplot Phosphates, was raising the feed pipe. She put her left hand on the block of the crane to steady it. As the crane block was coming up through the chute it pinched employee's left ring finger between the feed chute and the crane block.

Employee was helping to change lifters in a grinding mill. Nuts, sealers, washers and cones were removed from bolts. Employee was driving the old lifter out using a sledge hammer hitting the end of the bolts. As she hit the end of a bolt the hammer glanced off the bolt, her finger was caught between the hammer handle and the bolt.

Employee was positioning the SAG Mill liner machine to perform maintenance in the SAG Mill. His right thumb was caught between the SAG Mill liner machine and the edge of the SAG Mill entrance.

Employee was helping perform maintenance on a conveyor belt. He put some tools in a service truck and was returning to get some more tools to return them to the tool room. As he was returning to the conveyor, he slipped and fell. Employee did not report the incident until the next day 05/20/2010.

Employee was switching a sealed sulfuric acid injection system, a rupture occurred in the line exposing her to a sulfuric spray. First aid was given by a safety shower for 20 min. Employee was transported to the hospital where first aid treatment was administered by a Dr. This injury is FIRST AID TREATMENT ONLY. Should medical treatment be needed later, report will be amended.

EE was arc gouging 09/05/2009. He said about 9:00PM his right eye felt like there was something it. About Midnight he woke with tearing and pain in his right eye and went to the ER. He did not contact his supervisor because he didn't think it was work related until the ER Dr. told him his eye suffered from arc flash. EE reported the incident by phone 09/06/2009.

Employee was holding a box of clean rags and was going to cut the side of the box with utility knife to open it so the rags could be removed from the box and placed into a "Clean Rag" barrel. The box was on its edge and employee was holding the box with her left hand and cutting with her right hand, when the knife slipped and cut the backside of her left hand.

Employee was replacing the A frame on a pickup truck. He was attempting to remove a bolt which was so tight that an extension was needed. As employee was pulling with both hands on the extension the bolt came free suddenly and the employee felt a pain in his left shoulder/neck area. The location of the bolt prohibits the use of an air wrench or longer extension.

Employee was changing picks on the Feeder Breaker; after he climbed out of the hopper (using the ladder) as he stepped from the ladder to the platform he felt a pain in his left leg just behind the knee. ER doctor released employee to work no restrictions 10/08/2008. Follow-up by another Dr. on 10/13/2008 he was placed on restricted duty.

Employee was in the process of installing a one inch air line to a new compressor. He was kneeling on his right knee tightening a fitting with a pipe wrench. As he pulled on the pipe wrench he felt pain in his right knee.

Employee was using a hydro-blaster to unplug a sanded hydrosizer. The hydro-blaster removed most of the sand that plugged the discharge. Employee pulled the hydro-blaster hose out to realign the nozzle with the discharge line. As he repositioned his hand on the hose, the discharge struck the outside edge of his left forefinger cutting his finger.

Taking bucket teeth off 992 loader, mechanic hit pin with hammer and a piece came off and hit wheeler mechanic on left forearm. The piece cut one vein and nicked the tendon.

On 05-11-2008 employee reported to his supervisor that he noticed an irritation in his left eye, he thought it was from the work he was doing the day before (05-10-2008). Employee stated he had been grinding and when he removed his grinding goggles he felt something get into his left eye. Employee was taken to see a doctor.

Originally believed to be off site injury and became reportable upon acceptance of workers comp insurance. Became reportable 5-19-08 as per notification form workers comp insurance 09-03-08. Accommodations have been made for permanent lifting restrictions.

On 3/10/08 employee reported to his supervisor that he noticed an irritation in his right eye 3/6/08. 3/7/08, he went to see a Dr. Dr removed a piece of metal from his right eye. Employee stated that he doesn't know for sure when incident occurred he thinks it was related to the work he was doing on 3/5/08. Employee was working in the mine shop area using a torch and a grinder.

On 12/31/07 Employee was pulling and pushing a head mechanism on a drill press across discharge manifold flanges. This was done so that the flange faces would be the same length; he worked at this task for about 3 hrs. Employee reported he did not have any pain or soreness until the next day. On 01/02/08 he came to work and reported that he felt like he needed to see a doctor.

Employee was changing the teeth on the shovel bucket, teeth keeper pins had been removed. He was using a 10 pound sledge hammer to drive the old teeth off of the adapters. When he hit one of the teeth with the sledge hammer a small metal fragment (approximately 7 mm by 4 mm paper thin) chipped off of the tooth and lodged in his left forearm about six inches below the elbow.

Employee was attempting to repair a rubber water hose. As he was attempting to trim the rubber hose with a utility knife with snap off type blades the blade of the knife broke off. The part of the blade remaining in the knife housing penetrated employee's left hand between the thumb and index finger.

EE was running MgO and soluble iron assays on mill samples in the lab at the AA machine when a sample bottle he was holding in his left hand slipped loose. As he attempted to catch it, the sample and bottle struck the lab bench's counter-top, broke and cut emplooyee's lower left hand.

Employee was pulling on forks of a forklift when he stepped backwards onto an uneven surface & twisted his right knee. He did not report any problems until 11/6/05. At that time he had an infection that required surgery. The surgeon would not comment on whether the infection was associated with the sprain. Workers Comp had another doctor review the case.

Two employees were getting a 4" steel pipe from a storage rack. There were several sizes of pipe mixed together on the same rack. One employee lifted the pipe with the forklift. When the other employee rolled a 4" pipe off the tip of the forks, a 10" pipe rolled with it. His finger was pinched betweem the two pipes.

EE COMPLAINED OF WEAKNESS/PAIN IN HIS RIGHT ARM & WRIST. MEDICAL EVALUATION DETERMINED THE PROBLEM TO BE PINCHED NERVE IN THE R. ELBOW AREA, RELATED TO REPETITIVE MOTION OVER A NUMBER OF YEARS - RELATED TO JOB/WORK ACTIVITIES.

EMPLOYEE WAS CHANGING TRACK PINS ON A CAT D-11 DOZER. A PIN WAS PRESSED OUT OF THE LINK. THE RAIL HALVES WERE STUCK TOGETHER BY A SMALL LIP ON THE INSIDE BORE OF THE RAILS. EE WAS HOLDING A SMALL HYDRAUULIC RAM BETWEEN THE RAILS TO SPREAD THEM APART. WHEN THE LINKS SEPARATED EE'S THUMB WAS PINCHED BETWEEN THE TRACK & A TRACK ROLLER.

after ee had completed changing worn picks int he feeder breaker he positioned himself on top of the breaker and was in the process of removing the 12' extension ladder. while pulling the ladder out of the feeder breaker he felt a sharp pain in the upper right side of his back. after an examination by a physician the ee was placed on five days light duty work.

EMPLOYEE WAS CLEANING SOME LAUNDERS. SOME OF THE FLOOR GRATED HAD BEEN REMOVED TO FACILITATE CLEANING. AFTER THE LAUNDER WAS CLEANED HE BENT DOWN TO REPLACE A FLOOR GRATE. AS HE BEGAN TO MOVE THE GRATE BACK INTO PLACE HE FELT PAIN IN HIS LOWER BACK.

EE SUPERVISOR WENT TO TALK WITH EE AT 1030 AND NOTICED THAT EE WAS LIMPING. WHEN ASKED ABOUT THE LIMP EE STATED THAT EE TWISTED KNEE WHEN EE WAS CLIMBING ONTO A D-11 DOZER. EE WAS TAKEN TO DOCTOR AND RELEASED FOR FULL DUTY. ADDITIONAL FOLLOW-UP INDICATED THAT AN ORTHOSCOPE IS NEEDED (12/15/04) TO DETERMINE THE EXTENT OF INJURY.

Employee was attempting to separate two pieces of channel iron that were stuck together. As he shook the iron, it suddenly separated and one of the pieces struck him on the right thumb. As a result of the incident, the employee suffered an abrasion and contusion to his thumb.

EE WAS REMOVING A CONVEYOR BELT ROLLER FROM CONVEYOR BELT. HE CONNECTED 1 OF A CABLE HOIST-PULLER TO THE CONVEYOR FRAME THE OTHER END TO THE ROLLER. AS HE BEGAN TO LOWER ROLLER, CABLE SLIPPED EE REACHED OUT & GRABBED THE ROLLER FELT SHARP PAIN IN RIGHT SHOULDER. 11/10/04 INFORMED BY OPERATOR EE HAS INCURRED AN INGUINAL HERNIA ON LEFT SIDE

BELLY DUMP GATES DIDN'T OPEN. TOLD DRIVER TO GO TO YARD. HE DID NOT GO TO YARD BUT TRIED TO OPEN GATES HIMSELF BY BUMPING THEM. DRIVER PUT TRUCK ON UNLEVEL GROUND, TIPPED TRUCK ON IT'S SIDE. POOR ACTION ON PART OF DRIVER.

EMPLOYEE WAS CHANGING A DRILL BIT. HE WAS USING A HAMMER TO LOCK THE PIPE WRENCH TEETH ON THE DRILL BIT. AS HE WAS SWINGING THE HAMMER THE HAMMER HIT THE SIDE OF THE DRILL CAB, THIS CAUSED THE HAMMER TO GLANCE. THE HAMMER HIT THE EMPLOYEE'S FINGER.

EE WAS USING A TORCH TO CUT PART OF A BLDG. STRUCTURE FOR REMOVAL PURPOSES. AS HE CUT THE FIRST END OF A BEAM, THE BEAM MOVED, WHICH CAUSED EE TO LOSE HIS BALANCE. THIS CAUSED HIM TO FALL & STRIKE HIS KNEE INTO A SECTION OF HANDRAIL. EE HAD HIS SAFETY HARNESS ON WHICH PREVENTED HIM FROM FALLING FURTHER.

EE WAS MOVING A BARREL OF OIL WITH A BARREL CART. THE BARREL SLID OFF THE FEET OF THE CART & PARTLY TIPPED OVER SO AS TO BE LEANING ON AN ANGLE OF ABOUT 45 DEGREES. HE TOOK HOLD OF THE BARREL & STOOD IT BACK UP. IN DOING SO HE FELT A STRAIN IN HIS GROIN. HE WENT TO THE DOCTOR WHO TOLD HIM HE HAD A MINOR HERNIA. HE WAS RELEASED FOR LIGHT DUTY.

EMPLOYEE WAS REMOVING THE MAST A FRAME SUPPORT FROM A DRILL. HE HOOKED A DOUBLE CHAIN SLING TO THE MAST A FRAME SUPPORT AND THE OTHER END TO THE SHOP CRANE. HE WAS HOLDING THE CHAIN WITH HIS L EFT HAND TO KEEP THE SLACK OUT OF THE SLING. AS THE CRANE WAS BEING HOISTED THE SLING TWISTED AND CAUGHT THE EMPLOYEE'S LEFT MIDDLE FINGER BETWEEN THE SLING LEGS.

INJURED AND OTHER EE WERE REPLACING TROUGH ROLLERS ON THE SAG MILL FEED BELT. EE WAS ON THE EAST SIDE OF THE BELT. THEY WERE REMOVING A WORN ROLLER ASSENBLY, THE BELT HAD LIFTED WITH COME-A-LO NG. WHEN THE WORN ROLLER WAS TIPPED OVER INJURED'S RIGHT INDEX FINGER WA CAUGHT BETWEEN THE BELT ROLLER ASSEMBLY AND THE BELT FRAME WORK.

EE WENT OUTSIDE OF THE BUILDING TO START HIS DAILY CLEAN UP, WHEN HE BENT OVER TO PICK UP A WATER HOSE HE FELT PAIN IN HIS LOW BACK. ON 8-30-02 THE EE WENT TO A DR, THE DR TOLD EE TO RETURN TO WORK, THE PAIN WAS A SPASM, THE DR DID NOT DO AN EXAMANATION. ON 8-31-02 THE EE RETURNED TO THE DR, THE DR PLACED EE ON LIGHT DUTY FOR ONE WEEK.

EE WAS CHANGING OUT A BALL CHIP DRUM, THE MOTOR ON THE TROLLY WAS NOT WORKING, THIS MADE IT NECCESSARY TO PUSH THE DRUM TO THE END OF THE BOOM BY HAND. AFTER THE DRUM WAS CHANGED SHE STARTED T O WASH SOME PRODUCT SPILL OFF THE FLOOR. SHE WASHED OFF AND ON APPROXIMATELY TWO HOURS. AFTER WASHING SHE EXPERIENCED PAIN IN HER RIGHT HIP AND SHOULDER. IT IS NOT KNOWN WHICH CAUSED THE PAIN

EE WAS REPAIRING THE CHUTE ON 4896 BELT LINE. WELDING A PLATE ALONG THE BOTTOM OF CHUTE. HE WELDED ONE END OF PLATE TO THE CHUTE, THEN USED A WELD ON CLAMP TO PULL THE GAP OUT OF THE OTHER END . THE WELD HOLDING THE CLAMP BROKE, RELEASING THE GREE END OF THE PLATE FORCING HIS HAND BACK AGAINST THE OPPOSITE SIDE OF THE CHUTE.

DURING SWING SHIFT, EE OVER-EXERTED HIMSELF FOR SEVERAL DAYS BY SHOVELING ROCK & CONCRETE. EE ALSO HAS DEGENERATIVE DISC DISEASE, WHICH COULD CONTRIBUTE TO HIS BACK PROBLEMS.

NOTICE OF POSSIBLE HEARING STS. A RETEST IS REQUIRED BUT NOT POSSABLE IN THIS CASE. THIS EE HAS DECEASED SINCE THE TIME OF THE TEST AND THE NOTIFICATION.

0N 1/5/02 EE WAS WORKING IN FEEDER BREAKER. APPOROX 10:30AM HE BREATHED IN SOME SMOKE THAT CAME FROM HIS SHOE AFTER HE HAD STEPPED ON A HOT PIECE OF METAL. THE HOT METAL WAS SLAG/SCRAP FROM AN OXYGEN/ACETYLENE CUTTING JOB HE HAD JUST PERFORMED. SUPERVISOR WENT TO PICK UP INJ EE FOR LUNCH ABOUT NOON & SAW THAT EE LOOKED ILL SUPERVISOR ASKED EE IF HE WAS OK: EE ANSWERED IT WAS JUST

EE, A MINE OPERATOR WAS ATTEMPTING TO REMOVE A USED RIPPER TOOTH FROM A DOZER RIPPER SHANK BY HITTING IT WITH A SLEDGEHAMMER. A SMALL SLIVER CAME OFF OF THE RIPPER TOOTH AND HIT THE EE IN THE LT. SHIN AND FOOT CAUSING A LACERATION. THE ATTENDING DR. APPLIED SOME SUTURES TO CLOSE THE CUT.

EE WAS HELPING THE ELECTRICAL DEPT. PULL WIRES THRU A CONDUIT ALONG THE SIDE OF #4894 CONVEYOR BELT. THE WIRES HUNG UP THEN CAME LOOSE. AS THE WIRES CAME LOOSE EE STEPPED BACK TO MAINTAIN HIS BALANCE. EE S FOOT HIT A BELT SUPPORT BEAM ON THE GROUND CAUSING HIM TO FALL BACKWASRD. THE SUDDEN SLACK INT HE ROPE THAT WAS ATTACHED TO THE WIRES THAT WERE BEING PULLED THRU THE CONDUIT CAUS

EE WAS CLEANING OUT SOME DIRT FROM UNDER ONE THE METAL PURLIN. AS HE WAS CLEANING, HIS LEFT MIDDLE FINGER WAS CUT BY THE METAL. HE WRAPPED HIS FINGER WITH A RAG AND SOME TAPE AND CONTINUED TO WORK. HIS SUPERVISOR WENT TO CHECK ON HIM ABOUT 1630 AND NOTICED THE BLOOD ON HIS HAND AND ASKED WHAT HE HAD DONE. HE EXPLAINED AND WENT HOME. WHEN HE GOT THERE HE NOTICED HIS FINGER WAS STILL

EE WAS CLIMBING OUT OF A FEEDER BREAKER WHEN HIS KNEE GAVE AWAY CAUSING PAIN IN THE BACK OF HIS KNEE.

AT THE END OF THE WORK SHIFT EE BEGAN TO CLEAN THE DIRT & SWEAT OFF OF HIS FACE WITH A RAG HE HAD PICKED UP IN THE RAG BOX. THERE MUST HAVE BEENSOMETHING ON THE RAG BECAUSE HE SCRATCHED HIS LE FT EYE. THE NEXT DAY HIS EYE WAS IRRITATED & HE WENT TO THE E.R. & GOT MEDICINE FOR HIS EYE.

AS EE WAS WASHING A PHILLIPS BEAKER LIP CRACKED AND CUT TOP OF RIGHT INDEX FINGER AT TIME OF OCCURRENCE SHE WAS WEARING A LATEX GLOVE. EE RMSED CUT WITH WATER, DRIED WITH PAPER TOWEL AND SUPER VISOR APPLIED TWO BANDAIDS.

EE WAS WASHING DOWN THE DRAIN/SIDEWALK ON THE SLOUTH SIDE OF THE MILL BUILDING. BUILDUP WAS HEAVY AND NEEDED LOTS OF WATER. EE WAS GOING TO SET THE HOSE ON THE GROUND AND LET IT FLOW. AS HE BE NT OVER HE FELT A "POP" IN HIS LOWER BACK. HE COULD NOT BEND OVER. WAS TAKEN TO AVMC WHERE HE WAS SEEN BY DR. LIGHT DUTY
