On September 1, 2020 the employee had a follow up visit with Doctor for a previous minor burn. On a follow up visit the burn had some infection and the doctor prescribed an antibiotic and two days off work. Then employee received a return to work with no restrictions on September 3rd.

Mostrando los 18A tire technician was removing the inside dual lug nuts of a Komatsu 100 ton truck (T331). While positioning the 1' spline gun the technician twisted wrong causing employee to strain employee's back below the left shoulder blade.
An experienced miner was blading an area. While working the second pass, EE leaned forward to get a better look at the material coming out from the blade. EE came in contact with a large, unseen, rock that was part of the fill material. In doing so the equipment came to an abrupt stop causing head to come forward and the tooth coming into contact with the steering wheel.
A haul truck was having trouble getting backed into the shovel for loading. To assist the truck into position due to a slight grade and loose material, the shovel operator put the bucket in the bed of the truck for weight/traction and in doing so the bed of the truck raised and then returned to its lowered position causing a jarring effect in the cab.
Employee was changing the tire of a Caterpillar 777 end dump using a hydraulic operated bead breaker to loosen the inner ring. After the ring was loosened he grabbed the bead breaker to keep it from falling to the ground and inadvertently placed his index right index finger between the slide rails while the hydraulic ram was in motion, resulting in smashing his finger.
THE EMPLOYEE HAD ORIGINALLY REPORTED ON 11-8-95 HAT AFTER MANY EYARS OF PERATING VARIOUS PIECES OF EQUIPMENT THATHE HAD EXPERIENCED PAIN AND NUMBNESS IN HIS RIGHT WRSIT AND ARM. ON 3-19-96 WH ILE WORKIN GAT ANOTHER MINESITE HE LEFT WORK EARLY BECAUSE OF THE RECURRING PROBLEM. HE FAILED TO RETURN TO WORK THE NEXT DAY. HE WAS ADVISED TO SEEK MEDICAL ATTENTION.
THE EMPLOYEE WAS PREPPING THE BED OF A HAUL TRUCK WITH A WIRE WHEEL FOR AIR ARCING. WHILE DOING SO HE FELT AN IRRITATION IN HIS LEFT EYE. THE EMPLOYEE REPORTED THE ACCIDENT AND SEEKED MEDICA L ATTENTION ON 8-21-95.
THE EMPLOYEE HAD BEEN WELDING ON A LOADER BUCKET (CAT 992C) AFTER PERFORMING THIS JOB HE FELT AN IRRITAITON I HIS LEFT EYE EVANTUALLY HESEEKED MEDICAL TREATMENT TH4E DOC REMOVED A FOREIGN BODY FROM THE SURFACE OF HIS EYE & REMAINDED THE EMPLOYEE TO FULL DUTY
TOWARD THE END OF HIS SHIFT ON 06-01-95 THE EMPLOYEE NOTICED A SLIGHT IRRITATION IN HIS RT EYE THINKING IT MIGHT BE ONLY A SLIGHT WELDING FLASH HE WENT HOME THE FOLLOWING DAY 06-02-95 THE INTE NSIFION & THE EMPLOYEE REPOTED TO HIS SUPERVISORHE STATED THAT HE WAS NOT AWARE OF ANY PARTICULAR INCIDENT THAT CAUSED THE IRRITATION
THE EMPLOYEE WAS OPERATING A CAT DOZER (DION). HE BACKED AWAY FROM THE LOADER HE WAS WORKING WITH OVER A ROCK. THE DOZER PASSED THE PIVOT POINT COMING DOWN ON THE BACK OF THE TRACKS IN TURN W HIPPING THE EMPLOYEES HEAD & NECK.
WHILE REMOVING THE ENGINE COOLING FAN FROM CAT LOADER (992C) THE FAN SLIPPED AND PINNED THE EMPLOYEES FINGER (MIDDLE RIGHT0 BETWEEN IT ANDTHE AIR CONDITIONER CONDENSER COILS. THE FAN BLADE CU T HIS FINGER.
THE EMPLOYEE WAS CHANGING A TIRE AT THE READY LINE. WHILE TIGHTENING THE LUG NUTS WITH AN AIR WRENCH HE WAS STRUCK IN THE RIGHT EYE WITH A FOREIGN OBJECT. HE DID NOT GO TO HAVE IT CHECKED UNTI L 8-21-93.
WELDER WAS CUTRTING SHEET METAL FROM SIDE OF HITACHI 1800 SHOVEL THE METAL CAME LOOSE AT THE END HE WAS WORKING ON & PINCHED HIS FINGER (MIDDLE EFT) BETWEEN IT & THE SHOVEL TRACK
THE EE WAS CUTTING A HEEL PLATE FROM A LOADER BUCKET 992 C HE MADE HIS 4TH CUT AND THE PLATE FELL OFF STRIKING HIS RIGHT ELBOW AND PUSHING HIS ARM AGAINST A SAFETY BRACE PLATE WAS INITIALLY HE LD BY 6 WELDS. WELDS 5 & 6 WERE BROKEN 4 TH CUT CAUSED PLATE TO FALL.
THE EE WAS RUNNING A PUSH CAT TO THE LOADER HIS BLADE HOOKED A ROCK WHEN HE CAME OFF THE ROCK THE CAT WAS JARRED. AT THIS TIME HE FELT A POP IN HIS GROIN.
54736YEE WAS PULLING ON THE HOIST CABLE WITH HIS HAND WHEN THE BRAKE SUDDENLY RELEASED & CAUGHT HIS MIDDLE FINGER OF THE RT HAND RESULTING IN A LIGHT FRACTURE
FOREIGN MATERIAL IN RIGHT EYE.
TWO EMPLOYEES WERE REPAIRING LINER IN BUCKET ON 992C CATERPILLAR FRONT END LOADER. THEY WERE USING TWO 400 AMP HOBART WELDERS TO COMPLETE THE JOB. SOMETIME DURING THE WELDING EMPLOYEE STATES H IS EYES WERE FLASHED BY THE WELDERS.
