Employee parked EE's equipment and was walking to EE's light duty truck. Employee tripped and fell landing on EE's right hand. Employee was taken to the doctor for evaluation and X-ray. Employee was diagnosed with an Acute nondisplaced transverse distal radial metaphyseal fracture.

A mechanic was moving 55-gallon barrels by hand. The barrels were loaded with solid materials. When EE rolled a barrel into place, EE lacerated and broke EE's left middle finger at the distal phalange when it came into contact with another 55-gallon barrel.

On September 1, 2020 the employee had a follow up visit with Doctor for a previous minor burn. On a follow up visit the burn had some infection and the doctor prescribed an antibiotic and two days off work. Then employee received a return to work with no restrictions on September 3rd.

While walking EE stepped on some loose gravel and lost balance and fell on a knee. EE landed on a sharp rock that lacerated the right knee below the knee cap. EE received three stitches at the hospital.

Mechanics were removing a hub assembly on a piece of equipment. Employee slipped and EE's hand got in the path of the striking hammer. The hammer contacted the tip of EE's middle finger on EE's right hand.

Employee was starting a piece of equipment, EE reached around to unlatch door the point of the antenna struck employee on the side of the nose and slid up under EE's safety glasses and poked EE in the corner of the eye. Employee was prescribed an antibiotic eye drop to prevent infection.

Employee was preparing to change a roller on a 345 Track hoe by removing dried mud from the roller and bracket. A dead blow hammer was used to knock off the dried mud. In doing so, the hammer came in contact with the roller and a metal shard come off the roller and punctured the mechanics stomach.

A mechanic was working in the shop on a CAT triple 7 end dump. A mechanic was removing plastic zip ties from a transmission cooler hose with a folding knife with a locking blade. As the mechanic cut the last zip tie the pressured applied carried the knife toward EE's other hand, cutting the glove and EE's hand. The employee received stitches.

While reinstalling a shovel tooth the employee's finger was smashed between the tooth and the adapter. The tip of right ring finger was split open and needed stitches.

Employee, returning from medical treatment without notifying company that EE had been injured on a previous date informed company that EE had a bulged disk in EE's back. The employee stated after leaving work EE slipped. The employee said EE didn't fall down or hurt self. After waking up EE felt pain in one of EE's legs.

A tire technician was removing the inside dual lug nuts of a Komatsu 100 ton truck (T331). While positioning the 1' spline gun the technician twisted wrong causing employee to strain employee's back below the left shoulder blade.

An employee operating a 992 Cat Loader was trying to put a vent cover that had falling off of the AC unit during operations. As EE was trying to snap the cover back into position the cover slipped and the momentum the employee was using caused EE's hand to go inside the vent resulting in EE's hand coming in contact with the fan cutting EE's index and middle fingers.

An experienced miner was blading an area. While working the second pass, EE leaned forward to get a better look at the material coming out from the blade. EE came in contact with a large, unseen, rock that was part of the fill material. In doing so the equipment came to an abrupt stop causing head to come forward and the tooth coming into contact with the steering wheel.

A haul truck was having trouble getting backed into the shovel for loading. To assist the truck into position due to a slight grade and loose material, the shovel operator put the bucket in the bed of the truck for weight/traction and in doing so the bed of the truck raised and then returned to its lowered position causing a jarring effect in the cab.

Employee was looking over the area that was being drilled for blasting. While walking the outside berm the employee slipped and fell onto the berm resulting in a laceration of the right forearm.

Employee was changing the tire of a Caterpillar 777 end dump using a hydraulic operated bead breaker to loosen the inner ring. After the ring was loosened he grabbed the bead breaker to keep it from falling to the ground and inadvertently placed his index right index finger between the slide rails while the hydraulic ram was in motion, resulting in smashing his finger.

Employee states that he injured his back while walking down a slope on the job site.

A mechanic was using a pneumatic cut-off tool to cut slag from a loader bucket, as he started to cut, the new wheel on the cut-off tool broke into 3 pieces. One piece struck him in the side of his left hand, cutting it through his glove. He was taken to the ER and recieved 6 stitches to close the wound.

Mechanic was sliding a corner bit for a D-10 dozer from a pallet onto the back of a service truck. As the corner bit slid off the pallet it rotated slightly and when it slid off the pallet it caught the mechanic's little finger on his left hand against a vise mounted on the rear of the service truck, breaking the finger just below the top knuckle.

Operator stepped down from the last step of the ladder on a CAT 777 haul truck and injured her right hamstring.

Worker pulling material from trailer, climbed over to attach cable to next roll slipped hit knee on floor. No treatment @ time. 1/22/10 MD, diagnosed arthritis, meds given for arthritis no work restrictions. Further treatment given. 3/18/10 MD states treatment due to arthritis & to fall. MSHA contacted w/info, determined fall agrivated pre-existing arthritis making it reportable.

Tireman developed weakness and numbness in his right hand over a period of 25+ years of working. After evaluation by a physian it was determined that he had CTS and that surgery was necessary to correct this condition. On 7/21 it was the physian decided that surgery would be preformed on 7/26, so that is the day that is being used to report as day of accident (illness).

A mechanic had pulled a truck differential from storage using a forklift and a chain. When he reached to remove the chain hook from the differential it rolled slightly, pinching his left ring finger against the floor. This resulted in a cut that required 4 stitches to close.

Two employees were placing a plastic 55 gal. barrel onto a barrel cart/stand. The cart/stand was used to store the barrel on its side for dispensing. The employee on the handles of the cart/stand lost his footing as the barrel was layed onto its side & fell into a sitting position. The barrel slid down the handles & struck him in the left lower leg, fracturing his tibia.

EE WAS OPERATING A HAUL TRUCK WHEN HE STRUCK A BUMP IN THE ROAD JARRING HIM & AGGRAVATING A PREEXISTING BACK PROBLEM. DOCTOR RELEASED HIM TO RETURN TO WORK THE SAME DAY AT LIGHT DUTY.

EE WAS INSTALLING A HINGE ON A SHOVEL BUCKET. WHILE DRIVING PIN INTO PLACE A PIECE BROKE OFF OF SLEDGE HAMMER FACE & STRUCK HIM IN THE MOUTH, CUTTING LOWER LIP & BREAKING OFF ONE TOOTH.

TWO EE INSTALLING FUEL TANK, ACCUMULATED FUEL RESIDUE WAS DEPOSITED ON LOWER COVERALL LEGS. LATER WHILE INSTALLING STINGER COVER, EE WAS STANDING ON COVER HOLDING IT IN PLACE WHEN SPARKS FROM WELDER IGNITED COVERALL LEGS. EE RECEIVED 1 ST. DEGREE BURNS ON HANDS AND LEFT LEG AND SMALL 2ND DEGREE BURN ON RIGHT CALF.

EE HAD BEEN WELDING ALL DAY LONG. THE NEXT MORNING HE FELT THE "SAND IN THE EYES" SYMPTOMS OF WELDERS FLASH.

EE WAS CLEANING A FEED CHUTE WHEN A LARGE CHUNK OF MATERIAL STRUCK HIS RIGHT HAND PUSHING IT AGAINST THE FRAME.

EE WAS PULLIJNG ON WRENCH, IT SLIPPED AND STRUCK HIM IN THE TOOTH.
