EE was scaling the right rib with a CAT Excavator in 42 Face when a rock from the face fell and hit the FOPS of the machine and broke through the left rear glass and ended up on the seat behind EE. EE claims that EE does not believe the rock hit EE. However EE did have lower back pain before exiting the machine.

Mostrando los 22The worker was using an air-powered chipping hammer to take out mixing plates at the asphalt plant. While the worker was performing EE's task a piece of debris flew into EE's right nostril and made EE's nosebleed. The worker was taken to Waterfront Urgent Care to get EE's nose cauterized to stop the bleeding.
Employee was putting cab corners on a truck. EE was running a hand grinder when a very small piece of metal bounced off the bed and under safety glasses into right eye. EE came to work 8/4 but was sent to doctor that day. Came back 8/5 no restrictions. EE is trained to wear a face shield but didn't.
Employee was cutting welds off of the shaker screen shaft housing cover so it could be removed and replaced. The angle grinder employee was operating was caught on a bind and caused it to kick back into employee's chin under the face shield. This caused a laceration on employee's chin.
While cutting a Gradall shank with an oxygen acetylene torch, a piece of molten metal flew into the employees ear.
Drilling holes in ceiling by hand causing injury to right arm and shoulder.
Employee was installing conduit and brackets for conduit onto a block wall. He was using a hammer drill to drill pilot holes for the conduit brackets. Lime dust on the ceiling fell on top of him. Some of the dust entered his right eye causing a burn to his cornea and conjunctival sac
While changing out a drill steel on the roof bolter the employee got his hand caught in between the front drill support, which is a split bushing type that opens during the installation process and closes after steel installation.
While using a cutting torch to remove a bearing the employee was burned by the torch tip after another employee tripped over the torch hose causing the torch flame to contact his left hand.
EE WAS GRINDING A SCALING BAR DOWN WHEN A PEICE OF METAL FLEW INTO HIS EYE. THE EE WAS WEARING EYE PROTECTION, BUT THE METAL FLEW IN THE SIDE OF THE GOGGLES.
WHILE WELDING ON A BELT LINE, A SPARK FELL INTO THE TONGUE OF THE EMPLOYEE'S SHOE AND CAUSED A BURN.
THE COAL DIVERTER GATE WAS STUCK AT TAHE KILN. THE EE PINCHED HIS FINGER IN ATHE ARM MECHANISM OF THE CYLINDER & DIVERTER GATE. THIS OCCURRED WHEN THE EE GRABBED THE GATE ARM WHILE AIR WAS STI LL APPLIED TO THE CYLINDER.
EMPLOYEE WAS CUTTING PLATING FOR A CHUTE WHEN HE SLIPPED DOWN FROM THE CHUTE TRANSFER HE BURNED HIS WRIST WITH THE TORCH
DRILLING SHOT HOLES AT GUARRY WHEN THE HAMMER FELL OFF THE ROTARY DRILL AND FELL ON EMPLOYEES HAND.

BURNING ON A STONE SEPARATOR WHEN IT UPSET & HIT THE EMPLOYEE
THE VICTIM WAS 6AGGING HYDRATE LIME & THE BAG BRUSTED. THE LIME THEN SPRAYED INTO VICTIM'S EYES. THE VICTIM HAD A SHIELD ON BUT IT STILL WENT UP UNDERNEATH THE SHIELD.
EMPL JUMPED UP 0ROM SEAT TO PUSH STOP BUTTON FOR BELT AND PULLED MUSCLE IN RIB CAGE.
MINER GUARD HYD9AULIC HOSE ON DRILL & GOT STUCK WITH A PIECE OF WIRE ON HOSE CAUSING OIL TO GET INTO HAND. THE FINGER FOT INFECTED CAUSING THE F INGER TO SWELL. THE MINER WAS HOSPITALIZED BE CAUSE OF INFECT3ON IN T HE FINGER.
WAS CUTTING WITH A TORCH AND A PIECE OF SLAG SLIPPED IN ALONG SIDE OF TONGUE AND BURNT FOOT.
STRATA OPERATOR RAISED HIMSELF ACCIDENTLY INTO THE ROOF. RECEIVED FACIAL FRACTURES.
THE EE GOT DUST IN HIS EYE WHILE BAGGING HYDRATE LIME.
