Employee was climbing off of the rock truck when employee lost footing and fell approximately 4ft. The employee tried to catch themself but landed on left shoulder and left hand.

EE was walking and using the hand rail on walkway down to #9 belt Tunnel when EE's left foot slipped forward and EE's left leg extended outwards. EE caught self before EE fell but said EE heard a popping noise around left knee. This was not a lost time until EE went to doctor on 5/16.

Employee was putting cab corners on a truck. EE was running a hand grinder when a very small piece of metal bounced off the bed and under safety glasses into right eye. EE came to work 8/4 but was sent to doctor that day. Came back 8/5 no restrictions. EE is trained to wear a face shield but didn't.

Employee was cutting welds off of the shaker screen shaft housing cover so it could be removed and replaced. The angle grinder employee was operating was caught on a bind and caused it to kick back into employee's chin under the face shield. This caused a laceration on employee's chin.

While removing the front pad housing steel from the double boom face drill the housing steel jumped up and struck the employee on the left eyebrow.

The employee slipped and fell on the ladder while climbing into the water truck.

While changing out a drill steel on the roof bolter the employee got his hand caught in between the front drill support, which is a split bushing type that opens during the installation process and closes after steel installation.

The employee was cutting a gasket off of a steel plate with a razor knife when the knife slipped and cut the thumb on the left hand. The injury required five stitches to repair the thumb.

Not wearing any gloves using a knife to cut nylon wire tie from a piece of wire.

Unplanned detonation of explosives on the face #27 room, left lower corner of bottom hole, struck by drill steel used in the Atlas Copco Boomer Face Drill which hit a pervious misfired hole. No injuries occurred from the incident.

While working /cleaning, stepped back on a pallet and twisted foot, hyper extended the leg and damaged tendons on right foot

While setting an engine back into the scaling truck the engine shifted and caught the mechanic's left index finger resulting in an amputation right at the base of the finger nail.

A roof fall occurred in heading #25 between 25-2 & 25-6 blocks. The fall was approximately 450' long & spanned the width of the 60' heading. The area had been previously supported by 14', grade 75 mechanically anchored roof bolts installed on a 5 x 5 pattern.

While climbing a conveyor ladder the employee's foot slipped off and jerked his back when he caught himself with his arms.

The employee was prying on a rock with the hi-lift when the bucket slipped off the rock jarring his back.

While cleaning the sample screen the employee struck his finger with a rubber mallet.

A Chicago "A" coupling came lose from the compressor and hit the employee on the chin, causing a laceration requiring stitches.

The employee injured his back while helping to unload explosive stack and putting them in the powder magazines.

THE EMPLOYEE HIT HIS LEFT PINKY FINGER WITH A 2 POUND SLEDGE HAMMER WHILE DRIVING A PIN OUT OF A CHAIN DRIVEN CONVEYOR.

While climbing up shot pile the employee felt his foot hurt and give out. The diagnosis was planter fasia.

EE WAS MOVING A BOX OF RED D-GEL EXPLOSIVES AND LOST BALANCE, RESULT WAS A MUSCLE STRAIN OF THE LOWER BACK

EE INJURED HIS THUMB WHEN HE GOT IT CAUGHT BETWEEN THE MAN LIFT BASKET OF A LIFT TRUCK AND A PIECE OF DRILL STEEL USED TO SERVE AS A BRACKET FOR A 2300 VOLT DROP.

WHILE FUELING THE 988 CAT HI-LIFT THE EE MISSED THE LAST STEP OF THE PROVIDED LADDER AND FELL APPROXIMATELY 3 FEET TO THE GROUND.

WHILE STRAIGHTENING A BRAKE SHOE ON THE 662 GRADALL THE EE WAS STRUCK IN THE FACE WITH A HAMMER BY ANOTHER EE.

EE STEPPED DOWN OFF OF THE BACK OF THE POWDER TRUCK AND TWISTED HIS RIGHT ANKLE.

EMPLOYEE WAS CLIMBING DOWN OFF OF BC2 BELT AND HIT HIS LEFT SHIN BONE ON THE BELT SCRAPER SHAFT.

WHILE REMOVING HYDRAULIC FITTING ON THE FACE DRILL THE EMPLOYEE TURNED TO PICK UP A WRENCH & FELT PAIN IN HIS BACK

EMPLOYEE WAS WORKING ON A CONVEYER BELT CHUTE AND TRIPPED ON A BRAKE WHILE CLIMBING OUT AND FELL APPROX 3 FT ON HIS ELBOW.

THE HI-LIFT OPERATOR WAS TRAMMING UP #13 HEADING WHEN A FALL OF ROOF OCCURRED AND LANDED ON THE MACHINE.

WHILE MAKING TO REGULAR PRE-SHIFT INSPECITON THE MINE SUPERINDENDANT DISCOVERED THE ROOF FALL IN THE MINE'S SECONDARY ESCAPEWAY.

WHILE BENDING CONDUIT, THE EMPLOYEE THINKS THAT HE PULLED A MUSCLE IN HIS ARM AND CHEST. (EMPLOYEE NEVER MISSED WORK UNTIL 8/11/00)
