Employee was removing a belt wipe for replacement and was holding an end wrench on a stuck nut while another employee used an electric impactor to the bolt end for easy removal. The injured employee was holding the wrench loosely and the wrench spun with the nut striking the end of right thumb fracturing the most distal phalange.

Lying on back grinding metal from a screen deck and debris entered EE's left eye. Employee was wearing a shield and safety glasses. The condition did not bother EE until a day and a half later. EE never missed work but was restricted for one day. (No operating equipment and out of any dust)

EE was grinding the head off a bolt in a belt splice when the finger dug into the belt and grabbed his right hand making a 1/2" cut into his hand.

Employee was using a cutting torch to cut a piece of steel(on his back beneath the work) and a piece of hot metal entered his ear canal.

Employee was using an angle grinder with a wire brush to remove rust from a steel plate when his glove got caught in the wire wheel. The wheel pulled his left index finger into the wheel by the glove. No skin breaks but slight fracture in middle phalange.

EE was grinding pipe at the wash plant in prep to weld. The grinder got into a bind, jumped out of the pipe striking his left thigh. The injury was seen in the ER & the Dr thought he would leave it open to heal. (non-recordable) Recently the wound has flared up & the Dr wants to remove the object that must be present making the accident reportable now. (Surgery today 8/24/06)

EMPLOYEE WAS GRINDING RUST OFF OF A PIECE OF METAL IN PREPARATION FOR WELDING WHEN A PIECE OF DUST CAME UNDER HIS SAFETY GLASSES AND ENTERED HISEYE.

EE WAS GRINDING A PIECE OF STEEL WHEN A PIECE GOT UNDER HIS FACE SHIELD AND HIS PRESCRIPTION SAFETY GLASSES ( WITH SIDE SHIELDS), AND GOT INTO HIS EYE. FOREIGN OBJECT IN RT EYE.

EE WA PREPARING TO LOOSEN A BOLT ON A CRUSHER WHEN HE DROPPED A SOCKET. WHEN HE BENT OVER TO PU THE SOCKET THE RATCHET ENTERED A SMALL HOLE IN THE MACHINE AND KICKED THE RATCHET OUT HITTING EE WRIST

EMPLOYEE WAS USING A CUTTING TORCH WHEN THE TIP GOT TOO CLOSE TO THE SURFACE CAUSING THE METAL TO POP SENDING HOT METAL INTO HIS EAR AS HE TURNED HIS HEAD AWAY FROM THE POP.

EMPLOYEE WAS GRINDING THE BLADE OF A SCREW DRIVER TO SHARPEN THE END WHEN A PIECE OF THE WHEEL OR THE METAL CAME UP UNDER THE FACE SHIELF INTO EMPLOYEES LEFT EYE [IRIS]. THE OBJECT WAS SO SMAL L THE EMPLOYEE DIDN'T REALY HAVE ANY DISCOMFORT UNTIL LATER IN THE DAY.

EMPLOYEE WAS HAMMERING ON A SHAFT AND BLOWING DUST OFF PARTS WITH COMPRESSED AIR WHEN HE APPARENTLY GOT A PIECE OF METAL IN HIS EYE. HE WAS NOT AWARE OF ANYTHING EVER GETTING IN HIS EYE AS HE CAME TO WORK THE NEXT MORNING THINKING HIS EYES WERE SIMPLY TIRED. AS THE DAY CONTINUED HIS EYE GRADUALLY BECAME SORE ENOUGH THAT LATE THAT EVENING HE WENT TO THE EMERGENCY ROOM TO HAVE IT EXA
