Employee was operating a D9 dozer pushing off the overburden dump. When backing up, the dozer ran over a large rock which caused the back of the dozer to rise and then fall over the large rock.

Employee was welding grizzly rack for structural repairs and received welding flash to right eye.

Worker was using a belt polisher for approximately 1 hour on 9/10/20. Worker told a co-worker that worker's back was hurting after task was complete and continued working for the shift. On 9/11/20 worker was sitting in the service truck for about 10 minutes when right leg turned numb and stated worker could not walk.

Employee using grinder broke thumb. Should have used vice to hold metal prior to grinding.

Employee was trying to dislodge a rock from the primary crusher with a 30lb jack hammer when a piece of rock struck EE in left wrist and arm knocking EE backwards.

EE noticed winch cable had slack and was hanging down the side of dock. EE grabbed a pole to try to grab cable and move it to proper side of gavel. The winch cable was activated causing the cable to jump the gavel striking EE on the outside of left knee.

Stationary jaw die bolts were loose cut out old bolts with Air Arc wasn't wearing proper PPE and the slag rolled down arm into glove opening causing burn to right wrist.

Employee removing bolts from belly plate of a wheel loader and the torque multiplier on the wrench slipped making employee loose balance employee felt a pull in the groin area.

Removing bed liner plate from haul truck with cutting torch. As he was cutting the plate with the torch, the plate flexed and hit his welding helmet and knocked him back onto the ground.

Operating pug mill when foreign body entered eye.

While using grease gun on maintenance truck, the hose slipped and handle hit top of left hand.

Employee was repairing wedag crusher. Right hand was inside wedag attempting to turn rotor when hand was caught between rotor and wall of crusher.

EE RAN OFF RAOD WITH EXCAVATOR.

EMPLOYEE WAS CUTTING A PIECE OF METAL AT THE SCRAP METAL PILE. WHEN THE METAL WAS CUT, EMPLOYEE STEPPED BACKWARDS TO GET OUT OF THE WAY OF THE METAL AND FELL BACKWARDS ONTO HIS BACK.

TRYING TO REMOVE BOLT OFF 988-B LOADER. BOLT CAME OUT AND STRUCK EE . WE WERE NOT NOTIFIED UNTIL 6-1-98. WHEN EE WENT TO DR. THEY PUT HIM IN HOSPITAL FOR HIGH BLOOD PRESSURE. THEY DIDN'T MENTI ON SHOULDER INJURY.

HE WAS HELPING IN THE SETTING UP OF NEW PLANT, WATCHING EE, WHO WAS USING THE CRANE, TO LACE A PIECE OF H BEAM STEEL INTO PLACE, TH ESTEEL SLIPPED AND CAUGHT EE ON THE LEFT WRIST.

HE WAS GRINDING A PIECE OF METAL ON A STATIONARYGRINDER WHEN THE GRINDER GRABBED THE METAL PULLING IT AND HIS LEFT THUMB INTO THE GRINDING WHEEL.

EE WAS OPERATING THE DOZER PUSHING DIRT BACKED OVER BIG ROCK AND BACK WAS JARRED.

EMPL WAS CUTTING 1/2" THICK PIECE OF METAL WITH TORCH METAL WAS ON SAW HORSES WHEN CUT WAS FINISHED ON ONE SIDE THIS DROPPED TO GROUND REMAINDER WAS UNBALANCED & FELL ON RIGHT FOOT.

HE WAS HELPING WELD METAL ON THE SCREEN HOUSE, WHEN A PIECE OF METAL SLAG FLEW IN HIS RIGHT EYE AND SOME RUST GOT IN HIS LEFT EYE.

CRANE OPERATOR 4AS USING HEADACHE BALL TO BREAK BOULDERS FLYROCK STRUCK LEFT LENS OF SAFETY GLASSES,BROKE LENS OUT OF GLASSES FRAME AND LENS AND ROCK STRUCK LEFT EYE OF OPERAOR SEVERED BLOOD V EIN OF OPERATOR2S LEFT EYE.

RUNNING AIR TRA7K PULLING STEEL OUT OF HOLE USING LEFT FOOT T O HOLD GATES BACK STEEL SLIP CAUGHT LEFT FOOT BETWEEN GATES & HAMMER

RUNNING AIR TRA7K CHANGING STEEL TRYING TO REMOVE COUPLING AND STRIKING BAR, PIECE OF STEEL CHIPPED OFF STUCK IN LEFT LEG.

EMPLOYEE WAS CUTTING LEG OFF A FEEDER WHICH WAS LYING ON GROUND WHEN THE LEG FELL OFF IT TWISTED THE REMINDER OF FEEDER WHICH CAUGHT EMPLOYEE FOOT UNDERNEATH AND MASHED HIS FOOT.

DRILL MOST WAS BEING RAISED,STEM CAME LOOSE AND STRUCK EMPLOYEE ON BACK.IT WAS TREATED AS POSSIBLE CRACKED VERTEBRAE.

USING A CUTTING TORCH & CHIPPING HAMMER, EMPLOYEE WAS USING SAFETY GLASSES BUT A PIECE OF SLAG GOT IN EMPLOYEES EYE
