EE was using a winch (model 492) to back off the gravity take up weight when the winch handle slipped out of EE's hand while changing hands. The winch handle then spun around and the handle hit just above EE's right wrist. The handle was free spinning once EE's hand slipped off of it. Right wrist has a fracture of the ulna bone. Injury not in a cast but splint.

Mostrando los 9Removing sheave from ISC motor-no space to fully lay motor parallel to the ground. The motor/frame were blocked from motion but sheave was hard to remove. EE was using impact wrench on sheave when the tapered bushing let loose & the wrench, sheave & EE's left hand were pulled approx 16" downward. The wrench & sheave pinched the left pinky finger b/t the sheave & crusher structure.
Employee was cleaning material from the second floor of the screen house. A metal plate approx. 6"x 6" had been pushed back and hidden on a ledge of the chute beside the employee but shifted due to the vibration of the plant. The metal plate fell off the ledge, hitting employee's left forearm and resulting in a laceration with three stitches.
Employee was changing the cutting edges on a D-6 dozer using an impact wrench. He was pushing the belt down into the cutting edge with his left hand when his right hand bumped the trigger on impact wrench, turning the bolt. The head of the bolt was sharp and it spun cutting through glove and cut L middle finger. Received 7 stitches.
He was operating the clam dredge. He was adjusting a water hose, when bucket came back up and flung a rock at him. The rock struck him in the L upper arm, causing a laceration.
Operating clam shell dredge bucket when rock came out and broke the windshield. Glass and the rock hit his right cheek.
USING A GRINDER IN THE SHOP WHEN HE GOT A FOREIGN BODY IN HIS LEFT EYE. (PRESCRIPTION WAS GIVEN FOR ANTIBIOTIC DROPS.)
GRINDING ON THE HEAD BOX ON FLOATER CONVEYOR #6 WHEN THE GRINDER JUMPED STRIKING HIM IN LOWER LIP.
TWO EMPLOYEE WELDING ON THE INTERIOR OF A CAT 988B LOADER BUCKET. BOTH EMPLOYEES WEARING WELDING HOODS. AT SOME POINT WELDING FLASHES WERE REFLECTING OFF INTERIOR OF BUCKET.
