Air switch failure for concrete plant that has property on ours which was on a pole that was on mine property. Power was turned off to the plant and the fire department put the fire out. NO team members were near the pole when it caught on fire.
Employee was standing on a 8 inch channel and lost footing and reached up and grabbed what they thought was the structure but was the screen perch they were getting ready to weld and it fell and hit their leg and smashed it between the perch and a support beam.
As part of a blast crew IP was stepping over a stemming pile and twisted their lower back.
Removing sheave from ISC motor-no space to fully lay motor parallel to the ground. The motor/frame were blocked from motion but sheave was hard to remove. EE was using impact wrench on sheave when the tapered bushing let loose & the wrench, sheave & EE's left hand were pulled approx 16" downward. The wrench & sheave pinched the left pinky finger b/t the sheave & crusher structure.
A jaw die bolt had broken off & was to be removed. The space to access the bolt to push it out is limited. The employee was attempting to pull the broken bolt out with a come-a-long & a welded on lifting lug. The weld broke & the chain & lifting lug were under tension came loose & hit the employee in the hard hat & made a small cut on employee's cheek requiring 4 stitches
Employee failed to maintain control of a Case 440 skid steer while backing down the incline of a clean-up pile, causing the skid steer to tip over. While the employee initially stated EE was fine, several hours later EE complained of a headache and soreness and was taken for medical evaluation.
Employee was cleaning material from the second floor of the screen house. A metal plate approx. 6"x 6" had been pushed back and hidden on a ledge of the chute beside the employee but shifted due to the vibration of the plant. The metal plate fell off the ledge, hitting employee's left forearm and resulting in a laceration with three stitches.
Employee was standing 15 feet away from a crusher counterweight while a hook and crane were being used to manually rotate the counterweight. The hook snapped and a piece of the hook flew approx. 20 feet down and at an outward angle, striking the employee in the left forearm. Result was a laceration requiring 3 stitches.
Taking bolts out of a steel liner when liner came loose and struck left wrist. (surgery on 12/28/2017).
Employee was getting into an excavator and turned the wrong way and felt sharp pain in middle of low back. Treating with chiropractor.
Employee had been grinding and welding and then wiped his face on shirt sleeve, after which he felt something in right eye. Treatment received where piece of steel was removed and two prescriptions given
EE was servicing a loader and standing on the left side of the cab when he fell off loader and hit the ground.
The head box chute on the C-1 conveyor plugged up and EE was using a pry bar to dislodge the plug through a small door on the side of the chute. The material broke loose and fell through the chute all at once. The material hit the end of the bar that was sticking through the hole & forced it down forcing EE's end up & pinching his left pinky finger against a beam.
Injury was sliding a 6 ft long rail into a chute when his hands slipped off the rail and his right ring finger was pinched between rail and chute. was wearing leather gloves.
WHILE EE WAS WALKING FROM SHOP AREA TO MIDWAY BUILDING HE STEPPED ON A LARGE STONE CAUSING HIM TO TWIST FOOT. HE DIDN'T REPORT ANYTHING WRONG & TOOK 2 PERSONAL VACATION DAYS W/O TELLING ANYONE WHAT HAD HAPPENED. 3 DAYS AFTER THE INCIDENT HE WENT TO THE HOSPITAL TO FIND THAT THE FOOT WAS BROKEN. ON MON. (5 DAYS LATER) HE CAME TO THE PLANT TO TELL US WHAT HAD HAPPENED. NO WITNESSES.
ELEVATOR COUNTER WEIGHT FELL ON INDIVIDUALS RIGHT THIGH & LEFT SHIN WHILE HE WAS VACING MATERIAL OUT. THERE WERE NO BROKEN BONES - JUST SKIN ABRASIONS.
WORKING AT THE DUST COLLECTOR SCREW DVIER ON A 10' STEP LADDER. TWISTED & FELL ON RIGHT SIDE. INJURING LEG, ANKLE & FOOT.
CHANGING SLIDE ON OHSO CHUTE AT DRYER PLANT, HE TWISTED HIS LOWER BACK.
THERE WAS NO INJURY. MSHA CONTACTED AT 7:30 AM AND INSPECTOR PERFORMED AN INVESTIGATION ALONG WITH STATE INSPECTOR. THE CO. CORPORATE ENGINEER WAS ALSO INVOLVED IN THE INVESTIGATION. THE #2 CO AL FAN EXPLODED THROWING DEBRIS 15 FT. FROM THE HOUSING. FAN ITSELF REMAINED INTACT BUT THE HOUSING WAS DAMAGED BEYOND REPAIR. FULL INVESTIGATION IS TAKING PLACE & FULL REPORT WILL GO TO MSHA
