While bolting low ribs with a jackleg, the leg kicked out causing the machine to fall. Employee picked up the machine and in the process of repositioning it his left index finger was pinched by the chuck tender. This caused a cut to the left index finger, that required 6 stitches to repair, and employee was given a full medical release back to work.

Employee was drilling an electrical box on the Maclean bolter when the drill penetrated through the box cutting the back of his hand.
Drilling using a jackleg when the steel broke. The machine shot forward causing his hand to strike the broken steel resulting in a fracture to the tip of the left little finger and a laceration needing nine sutures to the left little finger.
EE was using a pressure washer when a surge of pressure caused a loss of control of the wand resulting in the tip going over and cutting through his boot.
The employee was wearing safety glasses but did not wear a face shield while grinding. The face shield was available and not used so the employee was reprimanded with a first offence documented verbal warning. The employee had to have metal dust removed from his left eye. There was no impairment but a prescription was given to prevent infection.
Employee got his finger caught in a pinch point on a Mclean bolter and caused a laceration that needed sutures, also a small fracture to his right index finger tip was found by the xray. Employee recevied a full release to work.
While pushing a dewy-dag resin bolt up in the bolthole, the bolt slipped off of the bolt diver. The bolt fell back out of the hole striking the jackleg operator's helper in the mouth, breaking off one of his front teeth, and splitting his lip. He was taken to the dentist for an evaluation. No dental work was done at this time, but a crown will be needed in the future.
Operating 6 yd. mucker, going down an underground ramp and mucker ran over a rock. This caused the mucker to bounce up throwing employee foreward striking his right lip on the steering lever, causing a laceration that required 3 sutures to close.
Employee's were installing resin bolts with a jackleg, and a bolt came out of the driver and hit the employee in the mouth. This incident chipped a tooth and required sutures in the lip for one of the employee's . Making this accident reportable.
Employee was installing 5' resin bolts in the rib of the stope access, using a jackleg and 4' driver. As he was spinning and inserting the bolt, the driver popped off of the bolt causing the jackleg to fully extend and fall in his direction. Employee started to back up out of the way but the leg prat of the jackleg came down across his right leg breaking his ankle.
Employee was using a drill when it bound up and twisted in his right hand.
While ee was drilling for rockbolts on a manbasket, his steel got stuck at the face where he was drilling. EE stuck a 2' steel in the machine and inside the man-basket against the crossbar. Steel which ee was holding with left hand went forward and pinched his left pinky against jackleg macine causing laceration and fracture.
Operator was compacting bench and backed off slope. Tried to correct by articulating and backing out of position but equipment rolled once and landed upright. Individual was belted in and ROPS system provided protection. Hosiptal treated minor scrape and bruise. Individual was released same day. Equipment damage not yet assessed.
EMPLOYEE WAS ATTEMPTING TO LOOSEN DRILL RODS AS DRILL WAS IN ROTATION. A 36" PIPE WRENCH LOST HOLD AND ROTATED, STRIKING HIM IN THE SHOULDER.
