Mostrando los 31Miner was accessing leach pad cut in a 631 G Scraper from the Haul Road. When the miner made the transition into the leach pad they struck a rough patch which caused the scraper cab to move up and down in a rapid motion. This caused the hairline fracture to thorax vertebrae T7 & T8. Seat Belts were worn, PPE was used.
The employee was cutting sheets of plywood and debris had gotten into the employees eyes. The employee received medical attention for corneas abrasion to the eyes.
In response to Part 50 Audit 2-5-14. Driller left foot was caught between the deck wrench and the stopper when the steel was spun to the bit.
While assisting with cleaning of filter presses, the EE's arm was contacted by the discharge of a steam cleaner wand. Another EE was changing position and allowed the discharge end of the high pressure wand to pass in close proximity of the second EE's arm causing a pressure contact wound.
In response to Part 50 audit 2-5-14. Getting loading shovel, rock dropped in bucket causing pain to neck.
In response to Part 50 Audit 2-5-2014. Process operator drilling a hole received metallic foreign material with rust in left eye.
EE was tripping out drill rods, on one pull he failed to properly tension the rods with the mainline. This caused the weight of the rods to pull the slack out of the mainline causing the line to break and the line weight ball to fall striking the driller in the right thumb.
In the process of changing the drill bit on a mobile bast drill, the drill steel kicked off to one side while the EE was holding the bit. The right pinkey finger of the EE was mashed between the bit and the steel frame work.
Trying to get heater working employee tried to remove a fuel hose to see if it was clear. Hose came free and the employees thumb went into fan blades on the heater cutting the top of the thumb which required stictches to fix.
WHILE USING A HAUL GRINDER, A SMALL SPECK OF MATERIAL GOT INTO HIS RIGHT EYE. HE SAID HE DID NOT NOTICE IT AT THE TIME. HE RETURNED TO WORK THE NEXT DAY SAYING THAT THE SPECK WAS IRRITATING HI S EYE. HE WAS SENT TO THE EYE DOCTOR. HE WAS PREVIOUSLY SCHEDULED FOR VACATION & WILL NOT RETURN TO WORK UNTIL MAY 6, 2002.
HE WENT TO TIGHTEN UP A CAP THAT WAS LEAKING ON THE HYDRAULIC TANK OF A DOZER. THE DOZER WAS RUNNING AND THE TANK WAS PRESSURIZED. THE CAP BLEW OFF AND KNOCKED HIS SAFETY GLASSES OFF, AND BL EW HOT OIL IN HIS RIGHT EYE.
AFTER USING A HAND HELD "DIE GRINDER" WITH A "FLAP WHEEL ATTACHEMENT TO CLEAN A SPINDLE, THE EM PLOYE NOTICED SOMETHING IN HIS EYE.
WHILE CUTTING WEAR PLATE AND STANDING ON LADDER. PLATE CAME LOOSE, KNOCKED LADDER OUT FROM UNDER EMPLOYEE AND HE JUMPED LANDING ON ANKLE.
EE WAS GUIDING HAMMER BARREL, WHEN HIS HANDS SLIPPED. HE WENT FOR ANOTHER GRIP & GRABBED BETWEENTHE HAMMER BIT & THE DRIVER SUB. THE PINCHING CAUSED A LACERATION TO THE LEFT & RIGHT HANDS. THE RE IS A BURST TYPE LACERATION TO THE LITTLE FINGER ON THE LEFT HAND.
WHILE USING A CHIPPING HAMMER, A PIECE OF METAL CAME OFF & IMBEDDED IN THE BACK OF THE RIGHT HAND.
EE WAS REMOVING A RADIATOR FROM THE TRUCK. THE RADIATOR WAS APPROX 4 FT OFF THE GROUND WHEN THE SLINGING BROKE. IT DROPPED STRAIGHT DOWN AND FELL TOWARDS EE. IT PUSHED HIM ALMOST OUT OF THE WA Y, COMING TO REST ON HIS FOOT.
EMPLOYEE WAS REMOVING AN AIR HOSE THAT WAS WRAPPED AROUND A MIRROR BRACKET. AT THE SAME TIME THE DRILLER WAS LOWERING THE STABILIZER JACK DOWN PINNING EMPLOYEE'S LEFT FOOT UNDER THE JACK.
EMPLOYEE HAD BEEN DOING A LOT OF WELDING AND GRINDING ON A MAJOR REPAIR PROJECT.
WHILE USING A PNEUMATIC SLAG CHIPPER THE THREADED QUICK CONNECTOR VIBRATED LOOSE. WHEN TH EHOSE CAME UNDONE IT FLIPPED UP AND HIT EMPLOYEE IN THE MOUTH CHIPPING HIS TOOTH.
EMPLOYEE WAS HELPING THE DRILLER CHANG EBITS. THE BIT WAS NOT COMING OFF EASILY. THE EMPLOYEE REACHED INTO READJUST THE BIT TOOL THE SAME TIME THE DRILLER REVERSED THE DRILL THE EMPLOYEES FI NGER WAS CAUGHT BETWEEN THE BIT TOOL AND A MACHINER GUARD.
EE WAS GRINDING RUST METAL IN THE TAIL CHUTE OF 7 CONVEYOR.
WHILE GRINDING A NUT OFF OF A METAL GUARD THE GRINDER CAUGHT KICKING BACK & HIT I NAME FINGER CUTTING ABOUT 3/4IN ON THIRD KNUCKLE LEFT HAND
EMPL WAS OPERATING DRILLTECH 040K DRILL WHEN IT APPARENTLY LIFTED OFF LEVELING JACKS & TIPPED OVER. WHEN DRILL STEM WAS STARTED BACK DOWN HOLE. EMPLS FOOT WA CAUGHT BETWEEN OPERATORS CAB & GRO UND.
INJURED WAS ADJUSTING THE GOVENER ON THE WELDER IN THE BACK OF P42.THE ADJUSTMENT SCREW IS NEAR THE FAN .WHILE ADJUSTING THE GOVENER THE NUT DRIVER SLIPPED ANDINJUREDS HAND WAS CAUGHT IN THE F AN.
EMPL CLIMBED DOWN INTO #15 CHUTE TO CLEAN IT. APPARENTLY HE DID TO SHUT OFF AIR TO SLIDING GATE.SWITCH WAS IN B POSITION & THAT WOULD MAKE GATE CLOSE TO RUN ORE TO #1 7. EMPL WAS APPARENTLY CLEANING OUT ORE BUILD UPUNDER GATE. WHEN GATE TO SWITCH ALSO I'M NOT SURE WHY SWITCH IN SECONDARY CONTROL ROOM WAS IN B POSITION.
THE BED OF A 100 TON DUMP TRUCK WAS BEING MANUALLY RAISED (AFTER BYPASSING THE HYDRAULICS) BY USING AN OVERHEAD CRANE. THE WIRE ROPES SLINGS FAILED CAUSING THE BED TO DROP. WHEN THE BED BOTTOM ED OUT THE EMPLOYEE WAS SEVERELY ABOUT.
SUPERVISOR WAS SHOWING HIS EE BOW TO USE JACKHAMMER ON A DIFFICULT PIPE OF CONCRETE WHEN HE STARTED THE HAMMER UP THE BIT JUMPED OUT OF THE HOLE AND SLID DOWN THE SIDE OF THE CONCRETE PULLING THE HAMMER DOWN ALSO HIS HAND BECAME CAUGHT BETWEEN THE HAMMER HANDLE AND THE STEEL WEIR BOX.
WHILE HANGING HEAVY MAGNET WITH GROVE CRANE ONE OF THE LIFTING EYES SLIPPED STRIKING EMPL FINGER WHICH WAS BETWEEN CLEVIS & LIFTING RING.
EMPLOYEE WAS GUIDING THE HYDROSET BACK INTO THE CRUSHER HOUSING THE CRANE LIFTING THE PIECE JERKED STRIKING THE EMPLOYEES HAND BETWEEN TWO FLANGES.
MOTOR GRADER WAS JUMP STARTED WITHA SCREWDRIVER STARTED MOVING AND EMPLOYEE JUMPED UP ON THE GRADER STRIKING HIS KNEE TRYING TO STOP IT.
