EE was signaling pump truck operator during concrete pour and lost hold of the hose. As the EE tried to regain control of the hose it whipped and hit the EE's left hand. EE received basic first aid for injury and refused further medical treatment. EE requested medical evaluation due to continued pain on 8/13 and was diagnosed with a distal radial fracture.

Operator was running a dozer on a side hill for a long period of time causing a pain in EE's lower back
Working on CV 17. When entering the HSI crusher EE slipped and fell, EE tried to catch self and hit EE's shoulder. Then was help by EE's coworkers by making sure EE was ok and called mine supervisor before transporting EE to main office.
A Process operator was using a 30lb digging bar to clear lodged material form the jaw crusher. As the operator was working to clear the lodged material, EE felt a pinch in the lower back.

Loader operator stepped off the lower step of the loader onto a hose causing an ankle sprain. Was seen by the physician 10/8/2014
Mechanic was using a 1/2 inch drill without securing it from movement causing his wrist to twist.

Miner was surveying the pit when he stepped on a rock spraining his ankle, Miner was initially diagnosed with an ankle sprain but returned to the doctor two weeks later and was re diagnosed with a foot fracture.
Employee was removing a digging bar from the travel way to secure a safe work platform when the bar slipped from his hands through the grating. When the employee reached for the bar to stop it from falling his right ring finger got caught between the railing and bar causing a laceration and requiring one stitch.
Employee was using a utility knife to cut drip line on the leach pad. Employee was cutting towards his body and not away from his body. As the employee was cutting the drip line the utility knife slipped and lacerated his left forearm requiring eight stitches.
Employee was cleaning screen deck, pulling plate when he felt a pop in his arm.

An employee was removing cutting edges from the bucket of a IT-28 when one of the plow bolts began to spin cutting through his glove and lacerating his hand. The employee was taken to a medical facility where he received medical treatment for the laceration and was released to full duty status.
While using a razor knife to cut a section of conveyor belting, EE cut his left thumb. The cut required 5 stitches.
Employee was standing outside of the lab building on a windy evening, directing a forklift when she felt something enter her eye. Employee was wearing safety glasses at the time. Employee returned to work after hospital flushed out her eye and prescribed antibiotics.
Miner was tightening bolts on a screen shaker with a wrench when the tool slipped off of a bolt causing his hand to strike the side of the shaker. Miner was taken to hospital on 5-11-12 but no fracture was found. He complained that his hand was still sore. Took him back to hospital on 6-12-12 and fracture was found.
Mechanic was grinding on a discharge belt when a small piece of metal embedded itself in the miners right eye. Witness said the miner was wearing dual eye protection.
The miner & drill operator were changing the striker bar on a top hammer drill. After replacing the striker bar, they stepped away from the assembly at which time the drill operator lowered the drifter onto the head & exited the cab. The miner approached the assembly & proceeded to align the bolts. The assembly shifted causing his hands to slip up & pinch his left thumb.

While moving a heavy VSI crusher anvil onto a pallet, the employee got his hand between the old anvil and the replacement anvil, causing a laceration on his right middle finger that required stitches.
While lifting a tail pulley in place on a conveyor the employee suffered a strained shoulder when the tail pulley slipped and he tried to take the weight of the tail pulley.
Miner reported that he slipped on plastic liner, but that he was OK. Miner AWOL for 3 days before calling in to report that he was seeing a doctor on his own for a strained back. Safety Dept notified 12/09/2011.
Miner was scaling frozen mud from agglomerator when he was struck by a chunk falling from the top, glancing off of his hard hat and hitting his shoulder.
On 10/26/11 a Lab Tech was found in the lab storage room crying. He said he had chest pain & shortness of breath. Emt's on scene at 2:05 pm. ee is overweight, has high blood pressure and diabetes, heart attack was suspected. At 3:15 ee arrived at the hospital and was stable and responsive. On 10/27/11 ee called and said he was OK.
Employee was cutting plastic tubing with a razor knife and was cutting towards himself when the blade contacted his left forearm leaving a 1 1/2" cut.
Employee got out of his pick-up truck, turned in the open doorway to retrieve his respirator and while doing so the wind blew the door shut, forcing the employee's left knee to impact the truck body.
Driller was raising mast and struck an overhead power line. Driller Assistant was holding wireline at the time and received electrical shock.

Employee was attempting to enter a closed door to the maintenance shop with his hands full of parts. As he was opening the door he placed his foot on the edge of the cement pad in front of the door, lost his balance and strained his back.

EMPLOYEE WORKS IN THE PROCESS PLANT AND HAS DEVELOPED A RASH ON HIS FINGER.
EMPLOYEE WAS LEARNING TO DRIVE A 150 TON HAULAGE TRUCK. HE SPENT TWO HOURS ON THE "BUDDY SEAT", THE REMAINING 10 HOURS WERE SPENT BEHIND THE WHEEL IN THE CATERPILLAR DRIVER'S SEAT.
EE WAS PULLING ON A METAL SKIRTING BRACKET ATTEMPTING TO REMOVE IT. HIS HAND SLIPPED AND STRUCK THE METAL BELT GUARD CAUSING A LACERATION.
EE WAS CHANGING A 50 LB. DRILL BIT. THE DRILL BIT FELL UNEXPECTEDLY CATCHING THE EE'S FINGERS BETWEEN THE BIT AND A PIECE OF METAL ON THE DRILL DECK.
HE WAS TAKING OFF TRANNY FILERS ON A DOZER.HE WAS USING A RATCHET WITH A CHEATER BAR HOOK TO IT. CHEATER BAR SLIPPED AND CAUGHT HIS HAND BETWEEN THE CHEATER BAR AND THE DOZER.

HE WENT TO TIGHTEN UP A CAP THAT WAS LEAKING ON THE HYDRAULIC TANK OF A DOZER. THE DOZER WAS RUNNING AND THE TANK WAS PRESSURIZED. THE CAP BLEW OFF AND KNOCKED HIS SAFETY GLASSES OFF, AND BL EW HOT OIL IN HIS RIGHT EYE.
LAB EE'S WERE MOVING EQUIPMENT FROM A CONEX TO THE LAB. EE WENT TO ROLL OUT A CRUCIBLE SHAKER. THE LID HAD BEEN WEDGE OPEN BY A SHELF IN TRANSPORT. SHE PUT HER LEFT HAND ON THE LID, SO SHE COU LD LOWER IT WHEN SHE MOVED IT. SHE HAD PUT HER RIGHT HAND IN THE HINGE AREA & WHEN THE SHAKER MOVED, THE LID WAS TOO HEAVY FOR HER TO HOLD UP AND LID FELL ON HER RIGHT THUMB
EE WAS SURVEYING A BOULDER PILE. AS HE WAS WALKING THROUGH THE PILE, HE STEPPED ON A BOULDER AND A PIECE BROKE OFF, CAUSING HIM TO LOSE HIS FOOTING AND FALL. AS HE FELL, HE HIT HIS RT WRIST ON A JAGGED ROCK CAUSING A LACERATION REQUIRING STITCHES.
AT THE TIME OF THE ACCIDENT IT WAS SNOWING. AS THE EE EXITED THE MINE FORKLIFT, HE SLIPPED ON SNOW COVERED ICE AS HE STEPPED TO THE GROUND. HE FELL ON HIS RIGHT ARM & SIDE. HE WAS EXAMINED BY MINE EMT & CONT WORKING. DUE TO SORENESS HE WENT TO A DR. THE NEXT DAY & WAS DIAGNOSED AS HAVING A PARTIAL ROTATOR CUFF TEAR.

EE WAS WALKING DOWNHILL WHEN HE SLIPPED ON A PATCH OF ICE/SNOW, LOST HIS BALANCE & FELL. WHEN HE TRIED TO BREAK HIS FALL, HE HAD A STRANGE FEELING ON HIS RIGHT SIDE. HE REFUSED TO SEE A DOCTOR AT THE TIME OF THE ACCIDENT. THE NEXT DAY HE HAD A LUMP ON HIS SIDE & WENT TO A DOCTOR ON12/12/97, HIS DAY OFF. DIAGNOSED AS A POSSIBLE HERNIA & REFERRED TO ANOTHER DOCTOR.
EE WAS SHVELING WET CLAY ON THE HEAP LEACH PAD. ON 10/29/97 EE INFORMED HIS SUPERVISOR THAT HIS LEFT ARM/ELBOW WAS SORE FROM SHOVELING. HE SUBSEQUENTLY WENT TO A DR ON 11/3/97 WHO DIAGNOSED A STRAIN. MINERAL RIDGE WAS NOT INFORMED OF HIS VIIST TO A DR UNTIL 11/18/97
EMPLOYEE BENT DOWN TO PICKUPA CARBON TRANSFER HOSE. AS HE STOOD UO HE HIT HIS HEAD ON A CARBON COLUMN. HE WAS WEARING A HARD HAT. EMPLOYEE DID NOT REPORT THE INJURY FOR TWO DAYS. HE ALSO WAS N OT INTERVIEWED UNTIL 10/17/97 WHEN HIS NEXT REGULAR SHIFT STARTED.
EE INSTALLING EMITTER LINES ONTO HEADERS. EMITTERS ARE PUSHED ONTO HEADER AND SECURED WITH A SMALL PLATIC NUT. AFTER INSTALLING A LARGE QUANTITY OF THESE, THE EE SAID HIS WRIST HURT. THE EE WO RKED 5 MORE DAYS BEFORE GOING TO A DR. THE DR SAID HE HAD A LEFT FOREARM STRAIN.
WAS REMOVING CATHODE ADN ANODE'S FROM ELECTROWINNING CELLS, WHILE PULLING UP ON THE CATHODE AND ANODES, THERE IS NOT VERY MUCH ROOM BETWEEN THE CATHODE AND ANODES AND JJUST ABOVE THE MIDDLE KN UCKLE OF THE RIGHT HAND BRUSHED AGAINST THE CATHODE WHILE PULLING THE ANODE.
