Employee was using a jigsaw to cut siding on #11 conveyor in area. While cutting Employee was taken to first aid where the eye was flushed again. Employee was then taken to local eye care center where debris was removed and antibiotic eye drops prescribed. Employee was wearing required PPE at the time of incident.

While operating bulldozer employee noticed pain on the left side of their neck. The stiffness progressed overnight.

Employee was using cutting torch to cut a hole in a piece of steel plate when some slag blew back striking side shield and running into left eye.

Working with float gang, employee was holding a spud on the head side of the bolt while attempting to replace a 45 degree fitting at #2 lift on the short north tailings line. Employee's finger tip was caught when the spud turned causing an avulsion to left middle finger.

Employee operates a large dragline that utilizes a joystick for control and began noticing pain and numbness in his hand. Referred to a doctor 10/7/14 diagnosis is rupture tendon left hand.

Employee was adjusting the air flow on the green float bank B-Cell when the middle finger on his right hand got caught between a moving paddle wheel and the guide frame for the tee handle used to adjust the air.

3 employees setting up an ACI to burn pile, needed help from excavator to extend the last 3'of burner pipe. Excavator operator while clearing path to ACI grabbed a small stack of limb debris in his way and pushed it into the pile. 4" by 6' long limb sprung out of pile striking employee on the ground 25' away. Breaking forearm (Ulna) and c1 vertebra.

Sandblast hose came on for a second, grit impacted face, caused abrasion and impacted left eye.

Mashed little finger between impact wrench and steel plate on clean out tee.

Employee was placing body bolts with a port a power when the shim plate used to distribute the load pressure fell on his hand causing a fracture to his ring finger.

Employee had been using an impact wrench removing bolts. While picking up bolts after finishing the job he experienced discomfort to his left wrist and forearm. Employee's discomfort remained unresolved and was then referred to an orthopedic institute on Monday, June 24 for evaluation and was placed on modified work with no use of left wrist.

Employee was struck by a torch causing a 2nd degree burn to his left wrist.

Heavy equipment operator was driving excavator to top of "muck" pile (muddy organic material remaining after deforestation of a swamp). The excavator's tracks unearthed a 15' oak log approximately 10" in diameter causing it to enter the cab of the machine through the lower front windshield. The operator received a fractured left ankle.

Employee was attempting to start a 4" pump using a pull rope, the battery was dead. When he pulled the rope it recoiled & the handle snatched out of his hand. He was treated w/ice & non-prescription pain meds at the clinic, returned to work. He worked a normal schedule until he was seen by co dr on 10/1 & referred for an x-ray, which revealed a fracture R ring finger.

The injured employee was in the process of cleaning clays & debris from the dragline bucket with a pneumatic chipping gun. He was working at approx. chest height & was applying pressure to the chipping gun using a twisting motion of his upper body with his arms extended. As he applied pressure and twisted his upper body, he felt a pain in his right lower back area.

The backhoe operator was pulling power cables with a backhoe. The employee was standing between junction box and backhoe. The cable became tight and hit him.

Employee was removing conveyor belt rollers on the prestripping system conveyor with the assistance of a backhoe. Employee placed his hand on the roller support bracket to remove it from the handler peg once the weight of the roller was supported. He signalled the backhoe operator to lift the roller. As the roller was lifted, the hanger bracket rotated and pinched his thumb.

Employee was removing steel grating in the Flotation Bldg. He rigged the hoist hook to the connecting rod of the grating and bumped the hoist up. Not knowing that the grating was not free, he bumped the hoist again, causing the connecting rod to break. The hoist hook released from the grating and struck him in the left hand, causing a cut and a hairline fracture.

Employee was applying light oil to the dollie rollers on #5 dragline. He was using an oil can with an approx. 18" spout. He was standing on the shoe and leaning over to reach the rollers while the dragline was swinging. He then fell between the dragline shoe and tub, fracturing his pelvis.

Employee was drilling holes in a metal substation housing, some overhead. He was wearing safety glasses. At 4:30 PM he felt irritation in his left eye & he washed his eye with water and went home. The next morning he reported the incident to his foreman & was seen by the company doctor. He was referred to an Optometrist and a small piece of metal was removed from his eye.

EE was removing a cross member under a pickup truck that was on a lift approx. 6.5' off the floor. The cross member has 4 bolts on frame and 2 in middle. EE removed the fourth frame bolt, the cross member fell to the floor, the center bolts were broken. The cross member struck EE on back of right leg as he jumped out of the way.

Employee & co-worker were welding flanges on pipes. Employee notice eye irritation after appr. 10 hrs. Irritation got progressively worse until he returned to work on 1/27 and received treatment from the company nurse. Prescription medication was administered.

Employee was removing the bottom bolts in 20" matrix fitting (all other bolts had been removed). A 980 loader was holding the pipe. Employee was using impact wrench w/his R hand, L hand was supporting imp. wrench. As he removed a bottom bolt, it sheared, & another bolt pulled through the bolt hole. Pipe fell on employee's hand pinning his L thumb between the impact wrench & pipe.

Grinding bolt off of a flange. The grinder snagged and it was torqued out of his hand. Grinder fell and cut his side.

Assisting in changing out a section a tailing piping. He was using a 1" impact wrench to remove bolts from the flange. The impact wrench slipped off of the bolt head. This caused the employee to lose his balance and he used his left hand to catch himself. This resulted in a laceration of his left palm.

Welding on 2570 boom, noticed pain in right elbow.

Employee was welding and lifted his shield up. Hot slag "popped" and went over safety glasses and into left eye.

Employee reported to his Superintendent that he had struck the side of his right hand against the window frame of the dozer as he was turning in his seat. He did not seek immediate medical attention. He was later evaluated by the Company Nurse and further by the Company physician, who ordered an X-ray which revealed a fracture at the base of the right fifth metacarpal.

EMPLOYEE USED DOZER TO PUSH A 60' LENGTH OF RAILROAD RAIL OUT OF THE WORK AREA. HE POSITIONED THE RAIL PERPENDICULAR TO THE DOZER BLADE. THE RAIL STOPPED MOVING FORWARD AND THE DOZER CONTINUED TO TRACK. THE RAIL BOWED AND SPRANG UP OVER THE DOZER BLADE AND CAME DOWN THROUGH THE LEFT SIDE CAB DOOR GLASS AND ONTO OPERATOR'S LEFT KNEE.

THE METAL WAS CUT THROUGH WITH NOTHING HOLDING IT FROM THE EMPLOYEE. THIS PUT HIM IN THE LINE OF FIRE FOR THE FALLING METAL.

EE USED DOZER TO PUSH 60' LENGTH OF RAILROAD RAIL OUT OF THE WORK AREA. HE POSITIONED THE RAIL PERPENDICULAR TO THE DOZER BLADE. RAIL STOPPED MOVING FORWARD & DOZER CONTINUED TO TRACK. RAIL BOWED & SPRUNG UP OVER DOZER BLADE & CAME DOWN THROUGH LEFT SIDE OF CAB DOOR GLASS ONTO OPERATORS LEFT KNEE.

Employees were welding a broken rack gear tooth inside the tub of #2 dragline. The injured employee was assisting with fire watch duties. While performing the fire watch duties, he received flashes to both eyes from the mig welding and air arc gouging.

EMPLOYEE WAS HOLDING AN OBJECT WHILE THE WELDER WAS TACKING. EMPLOYEE HAD CUTTING SHIELD ON. HE WOKE UP AT 3:50 AM ON 12/16 COMPLAINING OF RED, TEARING EYES. HE FELT LIKE HE HAD ROCKS IN BOTH EYES.

DRILLING OUT A FITTING THAT WAS IN A DRILL PRESS VISE. DRILL BIT GRABBED, SPINNING VISE AROUND STRIKING EMPLOYEE'S RIGHT THUMB.

PUTTING IMPELLOR ON PUMP. EE WAS REPLACING SLEVE AND FINGERS GOT CAUGHT ON BACK KICK BETWEEN BELTS AND SHEAVE.

EE WAS WELDING BRACKETS. AFTER WELDING A BRACKET, EE LIFTED HIS WELDING HOOD. DICAL DUST ON HOD FELL DOWN BEHIND HIS SAFETY GLASSES INTO EYE.

EMPLOYEE WAS PUTTING PLUGS IN PIPE. APPROX 10-15 FEET AWAY OTHER EMPLOYEES BEGAN WELDING.

WENT INTO A TANK TO GET A GLOVE. WHILE IN THE TANK AN ELECTRICIAN STARTED BLADE (AGITATOR) TO CHECK ROTATION. STRUCK EE ON LEFT KNEE, SHOULDER, BACK ANKLES AND RIGHT UPPER LEG.

PULLING UP IMPACT ROLLER WITH A BOOM TRUCK. WENT TO PUT KEEPER IN PLACE. OPERATOR OF BOOM TRUCK LEFT ROLLER DOWN TOO SOON AND CAUGHT FINGER UNDER KEEPER. MASHED INDEX FINGER ON LEFT HAND.

EMPLOYEE HAD STEPPED UP ON THE PUMP PEDASTAL TO UNHOOK THE CHAIN THAT WAS USED TO SET A NEW MOTOR IN PLACE. HE WAS HOLDING ON WITH HIS LEFT HANDAND USED HIS RIGHT HAND TO UNHOOK THE CHAIN AND PULL TOWARDS HIM. WHEN THE CHAIN HOOK PASSED OVER THE CHERRY PICKER BLOCK HOOK, IT FLIPPED TOWARDS THE EMPLOYEE HITTING AND CHIPPING HIS LEFT FRONT TOOTH.

WORKING IN BACKHOE AT PIT CAR SUMP WHEN WATER FROM PIT CAR CANNON HIT WINDSHIELD, SHATTERING GLASS. GLASS FLEW IN FACE.
