Employee reported that EE put a split set bolt through wire mesh and lifted it over head and put the end of the split set into a drilled hole and as EE was attempting to maneuver the jackleg to drive the bolt, EE slipped and let go of the bolt and it came down and the bolt struck EE in upper lip causing a laceration.

After installing a 3" victaulic valve assembly below the pump on an 8" waterline employee turned the valve on & the valve assembly began to spin, the valve handle caught the glove of the employee's right hand twisting their hand & arm around the valve assembly & pipe causing injuries to right hand & arm. Seen by the site medic and referred to ER, Restrictions were issued.

Two employees were hanging 80' of 2" HDP pipe in the Main behind the bolter. The two employees secured the pipe and blew the pipe out prior to hanging it. They locked out the compressor then hung the pipe. They unlocked the compressor, one employee entered the cab of the bolter, the other charged the pipe. Material from the pipe struck the bolter window and employee.

Employee was rehab bolting with a jackleg and while attempting to drive a 5-foot split set in the upper hanging wall rib the jackleg pushed back pinching arm between the footwall rib and the jackleg feed control handle resulting in a fracture to the right ulna.

Employee was cutting broken angle iron framework on the trash screen and the cutting tool slipped off of the angle iron and contacted left forearm resulting in a laceration.

Employee was pulling out of a drill hole with the jackleg when the leg kicked out causing the jackleg to drop un-expectantly pinching the employees left little finger between the machine and a feed frame of a diamond drill. Employee was seen by the medic and referred to the ER, no sutures were required and x-rays were negative, but restrictions were issued.

Employee was performing overhead welding when a piece of slag fell inside of the employee's boot resulting in two burns to the employee's foot, one on top of the foot and the other on the inside of ankle.

On 3/30/16, employee reported that on 3/24/16 was injured when lost control of the jackleg while bolting, and while getting out of the way something contacted left shin, causing a small abrasion to shin. On days off, pain and swelling persisted. Employee was seen at the ER, referred to a specialist, a small piece of metal was removed, restrictions issued.

Employee was mucking on a blasted round when his loader bucket made contact with the toe of the foot wall rib causing the loader to articulate towards the hanging wall rib. The employee was jolted back in his seat and the back of his hard hat contacted the hanging wall, resulting in a neck strain. X-rays were negative, but restrictions were issued.

Employee stated that the drill steel was hung up. He adjusted the leg on the machine & the steel freed up - his finger was caught between the machine & rib, resulting in a laceration to his right index finger. Sutures were required. Full release issued. No Days Lost.

Employee stated that he slipped while pulling jackleg out of hole. He did not fall, but strained his lower back. Went to medic on site, then to ER. X-rays were negative, but restrictions were issued.

Employee was in the bolting process when the steel hung up. He put 4' steel in chuck to break free. Employee's left hand contacted the screen when the steel broke loose causing a puncture wound to left hand. Went to medic, then to ER. No sutures required, but restrictions issued.

Employee received lacerations to his left ear (external) after catching the edge of the screen while operating a mucker. Sutures were required. Full Release issued. No Days Lost.

While mucking, employee leaned forward & contacted his forehead on the mucker frame, causing a laceration which required sutures. No Days Lost - Full Work Release Issued.

Employee was bolting & received a small abrasion/cut to his right wrist which became infected. 1st day missed was 9/3/2014

Employee was tramming a 2 yd mucker, when he ran over a small rock & jarred his back. Restrictions were issued.

Employee was prepping a slusher bucket to be lowered down in the stope. He swung the left slusher bucket arm out & reached in to remove the pin, when his finger tip (#3 finger) on left hand got pinched.

While bolting, employee's #2 finger-left hand was caught between the jackleg drill & hanging wall causing a laceration which required 2 sutures. Full Release issued. No Days Missed.

Employee was driving a bolt & the driver shanked causing the drill to jolt forward, pulling employee & he was struck in the face/safety glasses by derbis which caused his glasses to impact his face, causing a contusion. Doctor issued restrictions.

On 7/6/2013, employee was operating a jackleg drill & experienced some discomfort/soreness in the rib area. He reported this on 7/8/2013. Doctor issued restrictions in which we are able to work with - light duty.

Employee was using an air moiler(jackhammer)-with the whipcheck in place when the fitting rotated & contacted the employee's fingers. X-rays were negative however, employee was given restrictions.

On 6/12/2013, employee reported that he strained his back while bolting on night shift on 5/27/2013. This was not reported until 6/12/2013. We took him to doctor, restrictions were issued.

Employee's left foot got caught between hose bulkhead & traveling centralizer when it retracted. x-rays revealed fracture to foot.

While drilling with jackleg drill, employee was retracting drill steel & jackleg fell over causing the "leg" on the drill to strike him in the groin area.

Employee slipped while operating jackleg causing himself & drill to fall down. Drill landed on his right knee. x-rays were negative. Doctor issued restrictions.

A co-worker was using an air chisel working on the cutter head of the TBM, when a flying object struck the injured employee in the arm pit (right side) causing a puncture. Injured employee went to medic on site, then down to ER room.

Employee was operating 4 yd mucker when his foot slipped off the D-clutch while going around a corner on ramp striking the rib. The employee's head hit the inside of cab, causing a laceration on forehead (4 sutures required) and some neck discomfort. Doctor issued restrictions.

While bolting, employee felt a "pop" in his lower back, causing pain & discomfort. He went to medic on site. We took to doctor and restrictions were issued.

Miner was walking past a 2 yard mucker when the mucker articulated and the bumper struck the miner's lower leg resulting in fracture.

Employee was mucking in a cabbed mucker when he hooked a boulder with the bucket & it slipped off the boulder causing him to hit his head on the inside of the cab, Resulting in neck pain. Went to medic on site. We took to ER. X-rays were negaitve.Doctor issued restrictions. He missed 2 days and received a Full Release back to work.

While bolting the right rib, the steel slipped out of the wire hitting his hand that was on the machine. His hand was hit by the drill steel. Went to medic on site. We took him to ER for x-rays, which resulted in fracture to index finger on left hand. Restrictions were issued.

Employee's left hand got pinched between the support frame & drill head on diamond drill machine. This resulted in a fracture to his hand.

Employee was drilling through screen, when he released the leg on the machine & on it's way down, a small rock(1" x 3/4")bounced off the muffler on the machine & hit him in upper lip, and broke part of his front tooth off. On 7/9/12, he went to dentist & tooth was extracted, with implant procedure under way. No days missed.

Employee was bolting w/jackleg, when a rock fell & pushed jackleg back, pinching finger between bolt & drill handle, resulted laceration. went to medic. Worked his next shift then went on days off. later, finger started to look like it was infected, went to ER. finger was cleaned out, x-ray showed a bone chip. Restrictions were issued, we can not accommodate. This is a LT injury

Employee shanked a steel while bolting with a jackleg & noticed some pain & discomfort in right shoulder. At this time, it was a Report Only. As time went on, the shoulder discomfort did not go away. He went to doctor & surgery was preformed on March 19, 2013. His first shift missed was on March 20, 2013.

while employee was bolting, he went to put his 6' steel in & he had his right hand in a pinch point between machine & leg, which resulted in laceration to his right hand # 3 finger. sutures were required. Full release was issued. no days lost

Employee was bolting and driving a split set when the machine got away from him and it took him to the ground, landing on Right elbow. Went to medic on site for exam. All was good. This happened on his Friday, then went on days off. On 2/1/12, his elbow still hurt & went to Doctor. Restrictions were issued and we could accommodate them. He is currently working light duty

Employee reported on-going back pain. While bolting on 12/28/11-night shift, he again experienced pain. He was evaluated & referred for diagnostic testing. His release to return to work was withheld pending these results. An MRI was performed on 1/5/12 w/physican follow up on 1/6/12.At this time, he was diagnosed with 2 herniated disks & was taken off work

Employee was drilling, when a rock struck the jackleg drill causing the drill to move & come back striking employee in the hip, it bumped him back and drill hit him in R. ankle. went to medics, and we took employee to ER room. x-rays resulted in R. ankle fracture.

Employee was installing a split set rock bolt in rib, he went to adjust the bolt driver while jackleg was running & it caught L.#2 finger between jackleg chuck & bolt driver. Sutures were required. Full release was issued. No days were missed

Employee was drilling a slab round, when a small sharp rock slid off the foot wall, and struck him in the right wrist causing a laceration which required sutures. He was issued a full release, and returned back to work on the same shift.

Employee was operating an open cab road grader on a ramp, employee hit his head on a pipe, causing some soreness in his lower back. He went to medics, and was transported down for further evaluation. Doctor issued a few days of rest.

Employee was operating a mucker, mucking out the last scoop when a pvc loading pole broke off in the bucket, and a piece of the broken pvc came back & struck him in the face, causing a laceration, which required sutures. He was issued a full release and returned to work on his next scheduled shift.

Employee was operating a mucker, when hit his head on the inside of cab, causing a laceration, which required 2 sutures. he was issued a full release & returned to work on the same shift.

Employee was turning a 4 yard mucker around at a muckbay, when he over steered in the turn, running into the rib with the bucket, he was leaning forward when the bucket hit the rib & he struck his mouth on the door. This resulted in a laceration, which required 1 suture. Full release was issued & he reported for his next shift.

Hoisthouse overhead door was closing & employee walked right under the door as it was closing, striking him in the hardhat.He reported to the medics.We took him in for x-rays,which were negative.He was issued a full release. The next morning 3/2/11(a scheduled shift for him)he felt he needed to see a chiroprator.The chiroprator issued restrictions.

Employee was tramming a road grader, when he struck a header with the top of his hard hat. He drove to the 38 level and then went to medic on site. He had headache & was sore. He was taken to doctor, and was given some days off.

Employee was bolting. He repositioned the jackleg to finish driving the split set when he felt his back "pop". Pain was in the lower back.

Employee was mucking, while backing out of the transfer, a crushed split set bolt came thru a small drain hole in floor of mucker, striking her in the right leg causing a laceration, which required sutures. Full release was issued.

Employee was bolting, and was struck in face with tool, chipping tooth.... on 9/28/2010 he got his tooth fixed.
