EE was removing a starter from an International truck. The bolts were seized up and EE was using a cutting torch to heat the bolts for removal. EE who is nearsighted, slid the prescription safety glasses up to ensure EE was heating the proper bolt when hot grease hit left cheek and splattered into left eye causing a chemical burn.

He fell to the ground. His boot had oil on the bottom and he did not use a 3-point contact. A medical exam revealed that the employee had fractured his radius and wrist on the right arm.
THE EE WAS WORKING INSIDE THE MAINTENANCE SHOP DOING SOME WELDING ON THE WORKTABLE. HE WAS LEANING OVER THE STEEL SLIPS WELDING WHEN SOME OF THE HOT SLAG POPPED UP AND LANDED ON THE BACK OF HE AD. THE SLAG ROLLED DOWN HIS HEAD AND HIT THE INSIDE OF HIS SAFETY GLASSES AND A SLIVER OF SLAG BOUNCED OFF THE SAFETY GLASSES INTO HIS EYE.
EE WAS STANDING A TORQUE CONVERTER ON ITS END WITH A GROVE CRANE AND CHAIN. THE EE WAS HOLDINGON THE CHAIN TO KEEP THE CONVERTER FROM ROLLING OVER. WHEN THE CONVERTER WAS 3 QTR.'S UP IN THE AIR THE CENTER OF GRAVITY SHIFTED CAUSING THE HEAVIER SIDE TO ROLL OVER, JERKING THE LT. CHAINDOWN. THE EE DID NOT WANT TO SEE A DR. ON THE DAY OF THE ACCIDENT. EE SAW A DR. ON 1/08/03.
THE EE WAS HOLDING A STONE CLOSE TO THE SPLITTER BLADE, HE TRIED TO MOVE THE STONE BY USING A LEVER THAT RAISES THE STONE. STONE RAISED UP FASTER THAN HE EXPECTED AND CAUGHT HIS FINGERS BETWEE N THE STONE AND THE TOP BLADE.
