Injured man was roof bolting on a twin boom bolter when a piece of rock fell between the ATRS and the canopy striking him on the back of his left lower leg and foot.
Was preparing to bolt on twin boom Fletcher truss bolter and maintenance had been preformed on hydraulic system and an oil slick was left in the work area. He had hold of drill steel looking up at roof & slipped in oily area and right hand hit levers and clinched steel clamps around steel catching left hand.
Injured was tramming Fletcher bolter out of working place. He had his rt hand on top of canopy. Machine raised and caught his rt hand between canopy and mine roof.
While maintenance was being performed on equipment with cutting torches and welder. Some hot cutting went down inside top of boot and burned top of left foot.
While lowering drill head with right hand on the hydraulic controls and holding a section of drill steel in left hand, the upper end of the steel came in contact with the side of the drill head and the lower end of the steel contacted the operator's foot. As the head was lowered it pushed the section of steel against his foot.
Miner operator was tramming miner to face of #7 entry, was holding bolter cable up while he was tramming, miner boom was swung toward op side pulling cable and miner slide towards curtain/op side pinning him against rib. remote box was between he and miner boom, knocking power. Co-worker pulled him from between rib and miner boom.
EE WAS BOLTING AND WHILE INSTALLING A 4 FT RESIN BOLT IN ROOF HE FELT SHARP PAIN IN RIGHT SHOULDER.
