A roof fall was discoverd at #13xc, #8 entry in the south E panel. No equipement was trapped, ventilation was not affected, the passage of men or material was not affected. The fall was 17ft wide, 17ft long and 7ft high. The intersection was bolted wirh 5ft fully grouted bolts

A roof fall was discovered in the #3 main 2500' outby the section in a parallel entry. Fall location is #3 main #5 xc #10 entry. No impairment or damage to persons, equipment or systems.
Cutting ties off plastic lid of 5 gal. bucket of plaster with utility knife and cut hand in thumb area of left hand.
Injured man was roof bolting on a twin boom bolter when a piece of rock fell between the ATRS and the canopy striking him on the back of his left lower leg and foot.

A roof fall measuring 17'x30'x7' high was discovered during a rock dust surveyor being conducted during an EOI inspection in the south E panel in the intake air course, #8 entry. #10 xc. The area was bolted w/5' fully grouted bolts. The passage of men and supplies, ventilation or escapeways were not involved.
Injury walked into protruding stub end of cable bolt hanging down below roof line and hit nose.
Injured was preparing to rock dust an area with a co-worker and was holding a bag of rock dust and co-worker was slashing bags open with utility knife and cut injured person on right hand.
Injured was walking on section and a protruding cable bolt end caught him on the right ear causing laceration needing stitches.
Miner had loaded shuttle car and miner operator was on knees at end of miner boom along rib line. When shuttle car was pulling out, the s.c. operator swerved s.c. to right and rear of s.c. pulled toward miner operator, catching his feet with its bumper/frame, injuring his left foot.
Roof bolting with Fletcher RR2 twin head bolter, he had 4' resin bolt in hole and it was partially bent. He was pushing on bottom end of roof bolt with his body weight trying to straighten bolt when bolt twisted in hole causing him to fall into the bolt hurting his shoulder area.
When a 15' cut from #4 to #3 entry holed out, the roof for 2 rows of bolts in #4 fell out, along with the unbolted cut. The approachs were cribbed off; at 4:50 pm the fall extended overriding the cribs, falling through the intersection of #4 entry, landing in the head of the cut. The fall area was bolted with 8' fully grouted bolts & 12' cable bolts.
Was preparing to bolt on twin boom Fletcher truss bolter and maintenance had been preformed on hydraulic system and an oil slick was left in the work area. He had hold of drill steel looking up at roof & slipped in oily area and right hand hit levers and clinched steel clamps around steel catching left hand.
Injured was making a splice on a trailing cable and bent over to pick up roll of tape and twisted lower back.
During preshift examination of the North section faces (MMU #003) a fall was discovered in the face cross-cut #1 entry - 2 entry xc, through the intersection of #2 entry, through xc 2 to 3 stopping at the #3 intersection. The laminated shale fell to the sandstone. The area was bolted w/ 5 ft fully grouted resin bolts. The fall stopped at the cable bolts.
Injured was jacking red devil tugits when he slipped causing him to twist left leg injuring knee.
On the north section on unbolted cut in the face of #4 entry, fall at the end of afternoon shift. At 1:30 a.m. a triangle shaped piece of roof at the row of bolts fell out. A total of 6 bolts were dislodged. This happened on the idle shift.
Injured was using blade knife cutting off shuttle car cable to make splice when knife slipped cutting left index finger-had 5 ctitches.
Was splicing trailing cable and cut right index finger with blade knife.
Checking battery charges in surface shop, he tripped over charger cable and fell onto the floor and SCSR on belt.
Miner operator had A & B run out of face of #4 entry on south section. The clean up was in progress. Some rock had fallen out of last 10' of cut & was being loaded out. While cleaning up right side of cut the operator stumbled over a piece of the rock falling on miner remote and hurting ribs/hip/shoulder.
Person operating continuous miner on south section, #7 entry taking B run. Operator was setting 6 rows of bolts outby last row. Rock fell from between rib and 1st row of bolts and hit operator in shoulder, wrist and leg.
The fall at #7 entry, #23 xc on the south "B" submain, continued on riding out the cribs set between the #6 & #7 entry. The fall went through the crosscut from #7 entry to #6 entry and continued to within 15 feet of the #5 entry. The fall was 18' wide, 70' long, 10' high. The area was bolted with 5' fully grouted resin bolts. No one was injured. No equipment was damaged or trapped.
The intersection of #7 entry, #23 xc on the south "B" sub-main fell in. The fall was 18' wide, 13' long, 10' high. The area was bolted w/5' fully grouted resin bolts. No injuries; no equipment damaged or trapped.
Was hooking broken down 2 man mantrip to 5 man rubber tired mantrip with chain to pull to another area. The 5 man mantrip drifted back & pinned left leg between mantrips.
Was on kees in muddy area, repairing trailing cable when finished, was in process of standing up when right leg slid in mud in outward direction to the side and heard knee pop.
Person was doing labor work in outside supply yard and suddenly got chest pain-shortness of breath, weak on feet.
During the preshift for the first shift back after an idle weekend, the fall in south "B" #9 entry #20 xcut intersection was discovered. This is an intake entry (not the escapeway) 3-way intersection, bolted with 5' fully grouted roof bolts. Fall was just above anchorage and confined to the intersection 18x13x5 feet.
Injured was tramming Fletcher bolter out of working place. He had his rt hand on top of canopy. Machine raised and caught his rt hand between canopy and mine roof.
EE was backing miner out of xcut 6-5 and had head down watching miner cable when a piece of rock fell from between bolts and hit him in back of head. He was wearing hard hat.
Injured was putting in rib bolt on mains section #6 entry on left rib when rock fell from between rib & 1st row of bolts and hit him on head. He was wearing hard hat.
Was taking hex nuts off south section scoop panel board. Was pulling in up motion on extra tight hex nut when socket slipped off nut causing injured to pull back muscles.
Was putting tension on cable bolts and tension unit had metal burr on it and caught rt index finger. Made 1" laceration.
Was walking thru intersection and caught left ear on cable bolt stub hanging down approx 5" from roof & cut ear.
Injured was roof bolting when rock fell from roof hitting him in left elbow and forearm (bruise).
The mountain #11 outy @ 20xc @ plus 15+69 on the South "C" section fall. The fell was above and 6 to 8 high, 50 ft long, it connected to a area that was left unbolted and cribbed before the area was abandonded. The area was bolted w/5th fully grouted rebar 3/4".
Injured was bending a resin roof bolt to put in roof and pulled muscle in lower back.
The intersection of 13xc, #10 entry, plus 12+13 on the #3 main fell. The area was bolted with 6' fully grouted resin bolts and 12' cable bolts. The top was composed of black shale with some quartz. The area in the Pittsburg seam had been mined out. No men were injured. No equipment was trapped. The fall will not be cleaned up.
Was on belt moving crew using come-a-long to pull belt and tail piece. Got hand & fingers caught between chain hoist & tail piece.
Was bending a roof bolt that was part way in hole, he slipped on muddy wet pavement & pulled groin muscle.
Injured was picking up heavy object from floor level in warehouse and heard right knee pop.
A roof fall was observed during the preshift exam in the crosscut between #9 & #10 entry @ 10+33. The area was a sump that was cut after mining. The intersection on both ends was cable bolted w/passive cable bolts & cribbed. The fall was approx 70'l x 12'h x 17'w.
Was breaking metal band on bundle of roof bolts. When band broke open he was cut on right ring finger.
Injured person was pulling shuttle car cable to a new location on section when injured stomach muscle's. Diagnosed as hernia. This person worked light duty until 6/12/07-day of surgery.
A roof fall was discovered while pre-shifting the mine after a washed idle. The interesection in #5 entry, #13 x c spad plus 12+13) in the #3 main on section MMU #001 fell above primary anchorage. Roof was bolted w/6 for fully govt bolts. supplement with 12 ft cable bolts. water was running out of the rest at the loader full dimension are 18x18x12 for high.
At 9+43, #10 entry, #3 mains, approximately 1/2 thru intersection and 1/2 thru #10 to #11 entry. The roof fell above anchorage. Fall dimension 25' long, 17' wide, 10' high. Roof was composed of laminated shale. Area was bolted with 6' resin bolts. Fall stopped at cable bolts (12').

Was putting a bucket of mortar over a stopping wall. Being submitted due to Part 50 Audit.
