The foreman was installing rib bolts with a hand butterfly drill and while drilling, the bit caught something and kicked back. The drill handle struck the foreman on the right side of his head. 10 sutures wer required above his right eye.
The employee stated that while drilling a hole, the drill steel bent and struck his right wrist. The x-ray was negative, bruise.
The employee was installing a roof bolt and the steel was not drilling straight. The steel stuck in the chuck and when it came out, It pinched the ee's left hand between the steel and the chuck. 4- stitches were required and the ee returned to work his next scheduled shift.
The foreman was working on the truss bolter on the operator's side and was resting his left hand on the canopy. The bolt machine raised suddenly, pinching his left hand between the canopy and top. Fracture to left index and middle finger.
The EE was installing truss bolts. After installing the outside rib bolt, his next task was to bend the bolt against the top. He used the head of the bolter to bend the bolt and the bolt caught behind a roof bolt plate. The EE tried to push the bolt free with his right hand. When the bolt came loose, it sprung down and jammed his hand and wrist. X-ray showed hairline fracture.
Injured was bending roof bolt in low top area and felt pain in left shoulder.
The remote miner was in the #3to#2x-cut and the miner head was penetrated in the A-cut. The operator was standing beside the boom while loading S/C a rock 6ftx4ftx6inthick fell from the top and in the miner conveyor. The rock twisted and fell from the boom before entering the S/C. Luckily only the end of the rock struck operators left leg and calf.
INJURED WAS CUTTING A POST WHEN SAW CAME OUT OF POST AND CUT LEFT INDEX FINGER.
INJURED WAS BOLTING AND RIB BROKE LOOSE, HITTING HIS LEFT SHOULDER & SHOVING HIM INTO BOLTER. THIS CAUSED A FRACTURE TO HIS LEFT WRIST.
Scoop operator was loading supplies on the helper's side. When the ee was loading plates, the operator moved the machine and ran over the scoop operator's right foot. Ex-ray was negative, bruise and the ee returned to work light duty his next scheduled shift.
The employee was installing a roof bolt and a small piece of slate rock fell from the top, bounced off the bolter head and struck the employee's upper lip. Small laceration that required 4 stitches. The employee returned to work his next scheduled shift.
the operator was in the back and ee in front attaching the pull-rope operator stopped at curtain, shut machine off, & checked for person on other side.he then turned machine on & raised pan, not knowing the ee was attaching rope to the stop. the ee's little and ring finger was caught between the pan & stop. fracture, laceration to both fingers amputation to tip of little finger.
The employee was drilling a hole and when he raised the drill head, the steel did not go completely in the chuck. The steel bent and sprang back striking his right thumb. The x-ray showed a fracture and a splint was applied. He returned to work the next day restricted duty.
EE WAS INSTALLING A BOLT AND A ROCK FELL FROM THE TOP, HIT THE DRILL HEAD AND THEN STRUCK THE EE RIGHT SHOULDER. X-RAY WAS NEGATIVE AND THE EE RETURNED TO WORK NEXT SCHEDULED SHIFT. ONE DAY RESTRICTED DUTY
THE EMPLOYEE WAS INSTALLING A 12' CABLE BOLT. THE WRENCH WAS IN THE CHUCK BUT NOT QUITE ON THE BOLT HEAD. WHEN HE SPUN THE WRENCH, THE WRENCH WENT SIDEWAYS AND FLEW OFF THE BOLT FROM THE CHUCK, STRIKING THE EMPLOYEE IN THE RIGHT SIDE OF HIS HEAD JUST ABOVE HIS RIGHT EYE. THE LACERATION REQUIRED 6 STITCHES AND HE RETURNED TO LIGHT DUTY HIS NEXT SCHEDULED SHIFT.
NORTH SECTION: THE EMPLOYEE WAS DRILLING A HOLE AND A SMALL SHARP ROCK FELL FROM THE TOP, BOUNCED OFF THE DRILL STEEL AND STRUCK THE EMPLOYEE;S UPPER LIP. THE LACERATION TO HIS UPPER LIP REQUIRED STITCHES AND THE EMPLOYEE RETURNED TO WORK HIS NEXT SCHEDULED SHIFT.
THE EMPLOYEE WALKED AROUND THE ROOF BOLT MACHINE TO HELP HIS CO-WORKER MAKE UP BOLTS. A SLAB OF ROCK FELL FROM THE TOP BETWEEN THE BROW STRAP AND RIB, STRIKING HIS LEFT SHOULDER. THE X-RAY WAS NEGATIVE AND HE RETURNED TO WORK HIS NEXT SHIFT ON LIGHT DUTY.
THE EE WAS BOLTING IN THE X-CUT 5-6 AND THERE WAS A CLAY VEIN PRESENT THAT HAD ALREADY BEEN SCALED. WITHOUT WARNING, A PART OF THE CLAY VEIN ROLLED OUT OF THE RIB, STRIKING THE EE IN THE LEFT CALF. X-RAY NEGATIVE: CONTUSION, BRUISE.
THE EE WAS INSTALLING HIS OUTSIDE BOLT AND THE TOP WAS LOW DUE TO A SANDSTONE ROLL. THE EE SQUATTED DOWN TO MAKE BOLT INSTALLATION EASIER. HE WAS NOT AWARE OF THE PINCH POINT LOCATION OF HIS L EFT FOOT AND WHEN HE SET THE STAB JACK, IT CAUGHT HIS LEFT BIG TOE. FRACTURE TO LEFT BIG TOE.
THE EE WAS INSTALLING A ROOF BOLT TO SUPPORT A BROW. HE STARTED THE HOLE W/A SHORT STARTER STEEL, BUT DID NOT DRILL THE HOLE STRAIGHT. WHEN HE SWITCHED TO HIS REG. STEEL, THE BOTTOM OF THE ST EEL WAS NOT PLACED PROPERLY IN THE CHUCK. THE STEEL WAS ON A BIND & WHEN HE RELEASED PRESSURE, THE DRILL STEEL SPRUNG BACK & STRUCK HIS RT.HAND.
THE EE WAS INSTALLING A ROOF BOLT & RESIN IN A DRILLED HOLE. A SMALL ROCK WITH A SHARP EDGE, APPROXIMATELY 4 IN. X 4 IN. X 2 IN. THICK FELL FROM THE TOP, STRIKING HIS NOSE. THE SHARP EDGE CA USED A LACERATION TO THE TIP OF HIS NOSE AND STITCHES WERE NEEDED. THE EE RETURNED TO WORK HIS NEXT SCHEDULED SHIFT.
THE EE WAS DRILLING IN SANDSTONE AND THE STEEL BENT. WHEN HE TRIED TO BRING THE BENT STEEL DOWN, HE ACCIDENTLY HIT THE ROTATE WITH THE JOY STICK. THE STEEL WHIPPED AND STRUCK HIS HEAD AND LEFT PORTION OF HIS NOSE. THE EX RAY WAS NEGATIVE, BUT STITCHES WERE NEEDED IN HIS NOSE. THE EE DID NOT RETURN TO WORK HIS NEXT SCHEDULED SHIFT.
THE EMPLOYEE WAS PREPARING TO INSTALL THE LAST ROW OF BOLTS IN THE CUT. THE ATR JACKS WERE SET AND THE EMPLOYEE STATED THAT THE TOP WAS CHECKED (VISUAL & SOUND). WHEN HE TURNED AWAY FROM THE F ACE, A ROCK (1 FT X 3 FT X 2 INCHES THICK) FELL WITHOUT WARNING, STRIKING THE BACK OF HIS HARD HAT & RIGHT SHOULDER. EX-RAY NEGATIVE, BRUISE TO SHOULDER & LEG.
THE EMPLOYEE WAS DRILLING A HOLE AND A ROCK APPROXIMATELY 14 IN ROUND & 1 1/2 IN THICK FELL FROM THE TOP AND STRUCK LEFT FOOT.
EMPLOYEE WAS INSTALLING A ROOF BOLT AFTER CONNECTING AN ADDITIONAL STEEL, RAISED THE HEAD TO PUT THE STEEL IN THE CHUCK. THE STEEL CAUGHT THE EDGE OF THE CHUCK, BENT AND SPRUNG BACK. IT STRUCK HIS LEFT HAND. EX-RAY NEGATIVE; NO USE OF LEFT HAND UNTIL RE-EVALUATION.
WEST MAINS SECTION: THE EE PRIED DOWN LOOSE MATERIAL BEFORE STARTING HIS HOLE. HE POSITIONED THE DRILL STEEL AGAINST THE TOP AND WHEN HE TURNED TO PLACE HIS HAND ON THE ROTATED LEVER, A ROCK F ELL AND STRUCK HIS LEFT HAND. HIS LEFT LITTLE FINGER WAS FRACTURE AND HIS LEFT INDEX FINGER WAS LACERATED. METACARPAL GLOVES PREVENTED ADDITIONAL INJURY TO HIS HAND.
