There was a frictional ignition on the 6-mains section. Miner operator loaded 1st S/C and made the connection from 5 entry to 5 to 6 x-cut at block 15. The operator was loading the 2nd S/C and was in the process of trimming the bottom when a frictional ignition occurred. The flash was orange/yellow in color and lasted a few seconds. Two employees were injured.

The S/C operator was asisting the miner operator with his cable and curtain. The first S/C was loaded and the miner operator made the connection from the 5 entry to the 5 to 6 x-cut. The operator was loading the 2nd S/C and was in the process of trimming the bottom when a frictional ignition occurred. The flash was orange and yellow in color and lasted a few seconds.

The miner operator was operating the remote control miner on the 6-Mains section. The first S/C was loaded & the miner operator made the connection from the 5 entry to the 5 to 6 X-cut at 15 block. The operator was loading the 2nd S/C & was in the process of trimming the bottom when a frictional ignition occurred. The flash was orange/yellow in color & lasted a few seconds.

There was a frictional ignition on the 6-mains section. The remote control miner ee started his machine and his bits were not penetrating the coal. His right side bits contacted the right rib that had a sandstone streak. The operator stated that he saw a yellow flame above the right drum that lasted a few seconds.

The foreman was installing rib bolts with a hand butterfly drill and while drilling, the bit caught something and kicked back. The drill handle struck the foreman on the right side of his head. 10 sutures wer required above his right eye.

The mechanic was changing a scoop tire and in the process the tire fell on his mid section. The EE completed his shift and continued to work, on 9/10/08 the EE decided to see a DR. THe DR diagnosed a umbilical incarcerated hernia.

The foreman was standing under a newly constructed supported arch. He was using a rod that he pushed between the arch boards to slide a styrofoam pad on the top for additional cushion. The foreman was looking up and tripped, fell backward and struck the back of his head on a four foot piece of track rail. The CAT scan was negative.

On 8/21/2008 this foreman was loading posts in a scoop bucket and he felt a pain in his lower back. The foreman continued to work and on 9/10/08 decided to see his physician. Therapy and rest was prescribed.

The employee was driving a wedge with a sledge hammer and struck his left index finger with the hammer. Ex-ray showed a hairline fracture to his left index finger. Splint was applied and he returned to operating his shuttle car his next scheduled shift.

The employee was using a sledge hammer to knock a pin back in the belt feeder and a piece of metal flew fromthe hammer head and lodged in his right index finger. The piece of metal was removed and 7 sutures were required.

The employee was working at W 1 tailpiece. While shoveling, a piece of rock came back on bottom belt striking employee's right hand resulting in a laceration to underside of last digit on his middle finger. After being treated and returning to work, the finger became infected subsequently requiring the employee to begin to miss work.

The employee was shoveling belt and felt a pain in his lower back. X-ray was negative and the employee returned to work. A ignition occurred on the 6-mains section, approx. 65 feet in 24 block, 1 entry. Shuttle car was loaded from this cut and ee was trimming the top before the second shuttle car arrived. EE seen an orange ball that started on the curtain side of the miner head. He put his head down and when he looked back up, it was gone. Bits striking sandstone appeared to be the was cause

Employee was hanging miner cable and stepped in a hole, twisting his right knee. The employee completed his shift, but swelling developed. The X-ray showed a crack to the small bone in the back portion of his right knee. A brace was applied

The ee was jacking a bus back on track. The bus was raised with a lifting jack & the ee & co-worker were using a post as a pry bar to push the bus toward the rail. The jack kicked out and the post swung down and sideways, striking his right upper leg. This caused his knee to bend inward. The ee completed his shift, but swelling developed. Cat-scan revealed a torn meniscus.

Employee was riding in an encolsed manbus and the bus came to a sudden stop. No injury was reported to the supervisor, but the following morning the employee complained about numbness in his right side. Management decided to take the employee for an examination and was negative, slight whiplash was determined.

The ee was bending over to tie a pull rope on the miner cable and felt a pain in his lower back. Examination negative, strain (2 days restricted duty).

EE was watching slack in cable while feeder was being moved. The cable caught on a belt stand & the EE grabbed the belt stand, not knowing that the stand caused a small cut in the cable. A slight shock from grabbing the stand frightened EE & he fell against the rib. The EE sustained a dislocated right shoulder. Shoulder was re-aligned & EE returned to work the next shift.

The Connellsville, 66"W x 84" Dia., 1 1/2" rope, 300 HP slope hoist was down from 9:20 am until 10:30 am. All persons were removed from the mine after 30 minute time limit.

The foreman was working on the truss bolter on the operator's side and was resting his left hand on the canopy. The bolt machine raised suddenly, pinching his left hand between the canopy and top. Fracture to left index and middle finger.

The employee was cleaning the hi-lift with a water hose. He was standing on the bucket and when he climbed down he slipped and struck his left hand on the bucket. X-Ray showed a simple fracture to the bone below his little finger.

Mechanic was changing water sprays on the scrubber motor and a rib (12x4x8) fell suddenly, struck & pushed EE against Miner. The rib fell in a way that the Cont. Miner took the blunt of the weight & force. The EE was not pinned & was able to walk back to the back of miner. The EE was conscious & talking, but pain in shoulder & chest.

The EE was installing truss bolts. After installing the outside rib bolt, his next task was to bend the bolt against the top. He used the head of the bolter to bend the bolt and the bolt caught behind a roof bolt plate. The EE tried to push the bolt free with his right hand. When the bolt came loose, it sprung down and jammed his hand and wrist. X-ray showed hairline fracture.

Ee was preparing to install a roof bolt in an out-by area of the 6 mains section. When the ee raised the canopy, a rock (3 ft x 1 ft x 3 inches thick) fell from the top & struck the edge of the canopy. The rock rolled around the edge of the canopy & struck the ee's lower back. The x-ray showed hair line fractures to 4 of the lumbar vertebraes. Dr stated that surgery isn't needed.

Employee was having chest pains. Ambulance was called.

The employee was assisting changing a shuttle car tire. The lug bolts were removed and the tire suddenly came off and rolled, striking the employee's right foot and lower leg. X-ray was negative, bruise.

Injured was building stopping and cement block fell and struck right index and middle finger. Smashed fingers.

The employee was riding in a battery manbus and was sitting in the jump seat located by the operator. An undetected rock that fell from the top and was on the off side of the track caused the manbus to stop suddenly. The sudden stop caused the employee to fall forward. His right foot slid under the operator's seat and twisted. The x-ray was positive: fracture to right ankle.

The employee was lifting a cable reel on a roof bolt machine and strained his left wrist. The employee continued to work with some pain. Pain continued an an MRI was scheduled and the results showed a tear in the left wrist cartilage and tendon.

An alledged frictional ignition occurred on the 7 Mains section, approx. 100 feet int he #5 entry. The operator was trimming his top in the b-cut and noticed an orange flash that lasted less than one second.

At approximately 12:55 on the North Left section, MMU 003-0, 18 block, x-cut #3 to #2 there was an alledged flash generated from cutting sandstone. The intial investigation by MSHA and the state determined negative findings.

Injured did not feel any pain until the next morning in his lower back. He went to the doctor and had xrays and it showed he had inflammation in his lower back. He also went to a chiropractor who said he had 2 bones "out of wack".

The foreman was walking away from the face and a rock approx 16" long, shaped like a wedge) fell from the xcut corner and the sharp edge struck his left leg below his knee. The xray was negative but 8 sutures were needed. The employee returned to work restricted duty his next shift.
