I.E. put a bolt on the boom of bolter after drilling hole. When ee moved boom the bolt hung on the frame of the bolter & popped up, hitting ee in the mouth. It moved one of ee front teeth forward causing ee to get dental work. The I.E. has not missed any time due to this incident.

Mostrando los 28Employee was lowering boom and getting drill steel out of drill pot. EE stepped forward still having joy stick in hand when EE lost footing causing EE to hit rotation, and drill steel struck left hand/wrist.
Employee was loading bolt supplies onto bolter from scoop. The top fell out in front of pinner and 'operator side' bolt person went to inch tram controls to back pinner outby. The scoop was parked 3' (estimate) from bolter with bucket in elevated position. The injured employee was caught between scoop bucket & pinner causing a roofbolt to puncture leg just below right knee.
Drill steel was stuck in drill pot so employee put steel against top trying to dislodge, and doing so bent steel. Employee then tried putting steel into hole that was drilled and was guiding steel into hole with right hand,Employee was using up-pressure only and during the process moved joystick causing it to rotate striking right hand.
Injured was attempting to support a pot with a roof bolter while in the bolting cycle. The hole was being drilled next to the pot when the pot fell out, struck the bolter, then struck the injured in the left elbow, resulting in a laceration and bruising.
Injured was guiding a rack bar down the longwall face when the winch being used un-spooled, causing the rack bar to land on the left foot, resulting in fractures to 2 metatarsals.
Injured was dragging rock dust hose through a stopping when the injured kicked a rock in the walkway buried in rock dust, causing pain in the right knee.
Injured was miner helping. A rock became stuck in the conveyor chain. When the chain started back, the rock was ejected, striking the injured in the face requiring stitches.
Injured was monitoring the thermal dryer when a hot spot developed. The injured called to the control room to turn the fan down. Injured thought the fan had turned off and opened the access door. When the door was opened, the injured suffered burns to the face, neck and wrist as the fan was still running at a lower speed.
Injured was preparing a roof bolter for transport. The canopy was being lowered when the cylinder released and caught EE's left thumb between the cylinder and canopy resulting in a fracture.

Injured was pumping cement mix in water hole. He got legs wet with water and mix, causing burns to right and left legs.
Injured was installing a roof bolt when the roof bolt wrench broke. The wrench struck injured on his upper lip resulting in 4 stitches.
Injured was adding a 4' drill steel in order to complete the drilling of a 8' hole for roof bolts. The bottom section did not align itself properly with the top section. The bottom section when rotated struck injured on his left little finger dislocating it.
A HUNG STAGELOADER CHAIN FLIGHT WAS BEING REMOVED BY CUTTING INTO THE FLIGHT WITH A CUTTING TORCH. WHEN THE FLIGHT WAS CUT INTO A PIECE STRUCK INJURED ON THE LEFT BACK SIDE ON HIS HEAD. injured was advancing a shield manually. injured was sitting on pontoon. when shield was advanced a rock fell from between the shields striking injured employee on his right foot and ankle.
The slack in the longwall belt monorail trolleys was being pulled up. A sheave wheel and wire rope was being used. The injured employee walked along he belt placing himself in the "vee" of the wire rope. The sheave wheel came loose from it's mounting and flew across the belt striking the injured employee in the jaw. This resulted in a broken jaw.
THE INJURED EE WAS CARRYING A HOSE TO #91 SHIELD. THE SHIELDS WERE PULLED UP. A PUSH PRIME WAS PUT IN TO ADVANCE THE PAN LINE OVER. WHEN THE PAN LINE WAS ADVANCED, THE INJURED'S RIGHT FOOT SHI PPED OFF THE PONTOON. #92 SHIELD ADVANCED TO THE FACE ONTO INJURED RIGHT FOOT. THE INJURED PUSHED A STOP BUTTON AND PUSHED THE SHIELD OFF HIS FOOT.
