Employee was bolting 3 left and attempted to install a t-3 strap. He installed one side of the strap and attempted to push up the other end with the drill pot. The strap slipped off of the drill pot and sprung back striking him about 1" above his left eye. The employee received 8 stitches.

Mostrando los 15Employee states that he placed his hand on the conveyor boom and leaned over to get pull rope to hang cable on the hook on the miner boom. As he leaned over his stomach hit the tram lever and this caused the miner to move and pin his hand momentarily between the coal rib and the miner boom.
Employee was struck by drill wrench that was lodged between the drill boom and the tool tray. As employee lowered the drill boom the drill wrench struck employee on his index and middle finger. Employee received a laceration to the index finger and middle finger that required 4 sutures to close. He was treated and released to work.
Employee was attempting at install a roof bolt, roof bolt plate & a roof strap. As he shoved the bolt up in the hole, the bolt bent causing the plate to apply pressure on the roof strap. Employee had his hand guiding the strap into position and the strap pinched the ends of his middle and ring finger.
Employee was attempting to install a roof bolt and was in the process of starting the drill hole. Employee was holding the drill stell and started the rotation while holding the drill steel. Employee's glove caused a laceration to the index and ring finger. Sutures we required on both fingers to close the lacerations. Employee was released to return to work.
Employee was making a cable pull hook for the boom of 004 Miner. The 1 1/2 inch rolled steel had been heated & a crib block against the mine rib & using the swing action of the conveyor boom of the miner to achieve the desired bend in the hook, the crib block struck employee in the thigh of his leg.
Miner was moving the left side miner on the Mains section so he could scoop the 2nd right intersection. While tramming the miner the boom was swung to the right and miner was struck by the boom. Miner continued to work after the incident but decided to go to the hospital due to pain. ER doctors found no injuries but told miner to take off until 8-24 due to soreness.
Miner was pushing a bolt into the mine roof with the bolting machine. The wrench and bolt were mis-aligned, so the miner bumped the controls to rotate the wrench. His glove was pinched between the wrench and bolt, jerking his hand into the spin. His ring & pinky were dislocated.
Employee was trying to use the head wrench on a roof bolter to bend a roof bolt when the head sprang off the roof bolt causing the roof bolt plate to strike his right hand.
Employee was operating a roof bolter with his right leg outside the operator's compartment. The roof bolter came in close proximity with the mine rib. Employee's right knee was pushed into circuit panel in operator's compartment.
EE had his left hand on the drill pot as it was being raised. His left little finger got caught between the leveling arm and feed jack cylinder.
While bolting, employee's glove became entangled by drill steel & twisted his hand.
Operator was pulling a dozer out of a stuck position - got dozer out - hit the tram leavers - was back further that he thought - excavator flipped backwards & rolled - landed on the cab - cab glass was broken - removed broken glass & got ee out
Employee was bolting top when he observed rock dripping from top. He moved to get out of the way when a rock 66" by 28" by 9" thick fell, striking him after it hit the bottom.
EMPLOYEE WAS KNEELING BESIDE THE ROOF BOLTER WHEN PLATES FELL FROM TOP OF ROOF BOLTER AND CUT HIS RIGHT FOREARM.
