Employee was standing on the beltline running the Longwall Monorail push/pull unit. A hose busted on the unit. Employee changed the hose. While doing so, employees boots got muddy. When they got back up on the beltline, employee slipped and their left-hand ring finger got smashed by the push/pull unit.

Employee was bolting on K-Mains section and had just moved into the cut and started bolting. When the employee was pushing a 6' bolt into the roof, the roof bolt bent causing the bearing plate to hit the employee's right hand between the index finger and thumb, lacerating the employee's hand at the start of the webbing.

EE was installing a dywidag into the mine roof, lost balance causing EE to fall from the elevated bolter platform to the mine floor hurting left knee. EE continued to work without restrictions until placed off from work by a Dr on 5/12/21

Employee received a laceration to the left ring finger while removing a bolt from the end of a hydraulic take-up jack. Once the bolt was removed, the jack fell and caught EE's finger between the jack and the storage unit framework.

Employee was installing bolt and was struck by bolt plate causing injury to elbow.

The drill steel slipped out of jaws striking employee on left hand smashing finger

employee was caught between a long wall roof support and the AFC panline as EE was attempting to position the roof support into place on the face, during a long wall move

Employee was attempting to pressurize a hydraulic jack (duke), to re position a piece of long wall mining system hardware, and failed to adequately secure the pressure connector to the jack, which came off as ee pressurized the system, and struck ee on the right cheek.

Employee was assisting to make hydraulic/mechanical repairs on a roof bolting machine drill head/mast, at which time another employee functioned a hydraulic control lever which caused movement of the machine and injured the employee's left thumb.

Employee was roof bolting and accidently trapped hand in the drill steel guide

Employee placed right hand on rib drill steel to retract steel from rib. EE inadvertently started rotation instead of retract operation. Glove caught on steel dislocating little finger of right hand. Dislocated finger was reset and place in splint to immobilize.

Employee was making a hydraulic repair to a longwall support, during which time high pressure emulsion fluid was released and contacted the employee's hand.

Employee was operating a hydraulic impact wrench to install supplemental roof support when the wrench socket slipped off the apparatus and struck the employee's teeth.

Employee was positioning a hydraulic ram jack and pinched his finger between the end of the hyd. ram jack and the AFC pan line, causing injury to his finger.

Roof Bolter had just finished bolting a place, and was preparing to move. The employees foot was on the trailing cable as the cable reel was turned on, allowing the cable movement to jolt and injure his foot.

Employee was helping make a belt splice and using a power rivit driver installation tool, and got smoke/residue in his eye. Report submitted to terminate citation # 7040533

Employee was installing tensionable sling system in the roof and as he was installing the connecting hardware in place the bolter canopy was inadvertently raised pinching the employees left index finger between the roof and canopy.

Employee was replacing leg transducer on #92 longwall face support. When he finished repairs he reached up and pulled the spool valve handle to set the leg jack. The spool valve pulled hard so the employee jerked the spool valve handle and when he did so he felt a pain in the middle of his back and right shoulder.

EE was struck in the mouth while installing a roof bolt when the wrench he was using broke. A piece broke off of the wrench and struck the EE in the upper lip resulting in a laceration which required sutures to close.

Employee was standing on longwall support drum to mark and pull adjacent support when a piece of rock deflected off the shear cutter drum landing on top of right foot striking foot behind metatarsal gaurd causing injury.

While pulling longwall supports at shear a rock fell off shear body and struck on top of left foot fracturing big toe. Person was wearing metatarsal boots at the time of the accident.

While performing duties as a miner helper he was struck in the mouth by an unknown object. The result of this was a broken tooth. Person was 10-ft behind and to the side of the discharge boom of the continuous miner while loading a shuttle car when struck.

While operating the shear in sandstone conditions, a small piece of sandstone struck the operator in the neck, causing a laceration which required 2 sutures to close. As a result of this accident, it is now mandatory to wear neck protection when operating the shear in sandstone conditions.

Longwall foreman was conducting his examination of the face, when he was struck in the neck by a piece of steel that had splinted off of a cutter bit on the shear. The shear was operating in sandstone conditions and he was approx 20' from the shear at the time of the accident. Steel was removed, sutured to close the wound.

Shear was cutting out at headgate, in sandstone roof conditions. One of the cutting bits shattered on the drum and when doing so a piece of carbide from the bit struck the operators right hand causing a puncture wound requiring one suture to close. Metal was removed from wound by operator.

Operating roof bolter on West Mains section, #49 cross-cut 2 to 3. Operator got his ring finger of left hand caught between the drill steel and bolter head crushing end of his finger. Was holding steel and running the head up to connect steel into drill chuck. Was wearing meta carpal gloves at the time of accident.

EE was operating roof bolter on the #1 section face. While placing drill steel in bolter chuck the drill bit caught on a piece of wire roof mat knocking bit from steel falling and striking EE in the mouth breaking upper front tooth.

Person was chipping off slag from bit lug he had welded. A piece of slag went under his safety glasses and into his right eye. Person was examined by doctor - abrasions to right eye and given prescription eye drops. No time missed from work.

Person was installing dywidag slings while operating roof bolter, felt pain in lower back. Accident date - 11/27, doctor visit 11/29 - first day off work.

A 5' drill steel fell out of the roof and caught the left hand between the steel and steel extension. The operator thought the steel was clamped in the guide jaws but the insert had come out prior to the operator closing the jaws onto the steel. The belt securing the insert into the jaws had come out allowing the insert to come out. Sutures to close wound.

While cutting a metal plate with a wafer wheel, a piece of metal debris (shaving) entered the injured employee's right eye.
