Employee was shoveling on EE's knees under the belt. The area under the belt/roller here was 12 inches. When pushing the shovel under the belt, the shovel came in contact with the bottom roller pulling the shovel and the employees left hand into the end of the roller.

EE was opening the elevator doors outside to go underground. EE opened the first elevator door. When opening the second elevator door, EE hit elbow on the first elevator door that was open jamming EE's right wrist in the handle of the door. EE's wrist continued to hurt and went to get examined. On 9/18/24 an x-ray revealed a small fracture and EE was placed in a wrist brace.

Employee was removing structure and smashed right hand ring finger between a bottom roller and the coal rib when removing the roller. Employee turned in the accident 3/19/24. Employee had an x-ray on 3/20/24 showing a fracture of the finger tip.

Employee was standing on the beltline running the Longwall Monorail push/pull unit. A hose busted on the unit. Employee changed the hose. While doing so, employees boots got muddy. When they got back up on the beltline, employee slipped and their left-hand ring finger got smashed by the push/pull unit.

Employee was putting solid blocks back up to seal a hole in the wall. The air pressure and grip on the block caused the block to fall out of EE's hands and off the wall striking the employee on the end of the nose. Employee received 6 stitches on 2/2.

Employee was using a sledge hammer to drive a pin in the bracket for the cat pad on a mule. A piece of the bracket broke off and hit the employee in the left wrist embedding in EE's arm.

Employee was examining the offside of the belt and slipped on a piece of belt lying on the ground. When the employee went to fall, employee grabbed a hold of the belt rail and felt a pain in right shoulder. Employee continued to work until 4/22/2024 when employee was taken off work pending further examination.

Employee was operating the #1 MUV lube truck when EE was attempting to pull into a crosscut. When pulling in, the MUV went toward the rib. Employee stuck EE's arm out of the operators compartment touching EE's palm to the rib. In doing so, the MUV cab contacted the back of EE's left tricep, above elbow, causing a contusion.

Employee was working on the conveyor chain on the continuous miner. Employee hit wrist with a hammer while working on the chain. Employee continued to work until 11/27/2023 when employee was taken off work until employee can see an orthopedic.

The employee was installing a 6' bolt in the top and it bent. The employee then tried to pull the bent bolt out of the top and EE's hand slipped, catching the left forearm on the plate, causing a laceration, requiring 3 stitches.

Employee was following up with the cables from the belt move. As EE was walking toward the feeder/tail area, the employee stepped down on a top roller laying on the ground, twisting their right knee.

Employee went to climb up the crossover just outby 44BK on 38-Butt beltline. When the employee stepped up on the first step to grab a hold of the handles, the crossover slid off the belt rail and dropped down on one side. The employee felt a sharp pain in the right shoulder. The EE continued to work until 9/29/23 when the doctor took EE off work until an MRI could be scheduled.

The employee was on their knees on the head of the miner cleaning sprays in the # 1 entry on 39-butt. While EE was on their knees, the employee slipped and fell forward onto the drum of the miner jamming EE's left shoulder. The employee continued to work until EE was taken off work on 9/26/2023 due to pain in the shoulder. An MRI has been scheduled.

Team Member misplaced EE's right foot under ram cylinder allowing cylinder to come down onto right foot while helping with support in outside yard area.

Employee was operating the fork truck in the outside yard. As the employee was raising the forks up, with no load, the keeper and one of the mast chains broke causing the forks to drop to the ground. In doing so, something knocked the front windshield out, striking the employee in the head. What struck the employee is unknown. Employee received a laceration requiring 8 staples.

Employee was helping secure a ladder and fell back on block when blocks accidentally fell.

Employee was on a 6 foot step ladder getting ready to wedge the wall which was pyramided up in the middle. As employee was attempting to wedge the top of the wall, the wall started to fall toward them. The employee jumped from the ladder, landing on the pavement, injuring left ankle.

Employee was walking around the lube truck at 114 break F-Mains when employee slipped and fell. When falling, something cut employee's leg above right knee, causing a laceration. Employee received 4 stitches.

Employee was a passenger in the MUV mantrip when it accidentally bumped a rib causing injury to employees back area. Employee had the weekend off and released to full duty.

Employee was in the process of repositioning a drill box, spinning it to get in the 913 scoop bucket. The employee's right ring finger got caught between the chain and the metal of the drill box.

An unplanned inundation occurred when the continuous mining machine intersected abandoned mine workings. Employee was found unconscious outby the tail boom of the continuous miner.

Employee was b-bonding the wood on top of the newly built belt wall. Employee had a hand full of b-bond and was slapping it on the wood from a ladder. While doing this, some of the b-bond got behind EE's safety glasses and into EE's right eye.

Employee was driving the #2 MUT (Mine Utility Truck). At 117 break F-Mains, the employee hit a hole in the mine roadway bouncing EE up off the seat, causing EE to hit head off the roof, jamming their neck. The employee continued to work until 2/8/2023 when EE went to the doctor and was taken off.

Employee was getting ready to fill #3 913 scoop with fuel. When employee was pulling the hose around the parked scoop, employee walked into the 16 foot cable bolts that were sticking 13-14 inches off the rear of the scoop, striking mouth on the cable bolts, chipping the right front top tooth. Employee got tooth repaired on March 29th, 2023.

Employee was walking F-Mains intake from the shaft inby, cleaning and rehanging lifeline. Employee started at 115 BK and made it to 158 BK when some rock from the coal rib began to fall out. Employee's reaction was to jump back to avoid it, and in doing so, twisted right knee.

Employee was walking down the outside slope hill toward the belt slope airlock entry into the mine. The employee slipped on the icy/frozen ground causing EE to fall and strike left knee on the ground. Employee continued to work until noticing a laceration on their knee.

Employee was bolting on K-Mains section and had just moved into the cut and started bolting. When the employee was pushing a 6' bolt into the roof, the roof bolt bent causing the bearing plate to hit the employee's right hand between the index finger and thumb, lacerating the employee's hand at the start of the webbing.

Employee was carrying a bag of concrete and stepped in a hole causing injury to their knee.

Employee was cutting 48 inch mining belt to be used as skirting flaps on the engineered tailpiece. Employee had the belt laying on the shop floor using a straight edge and a razor knife to cut it. The knife slipped and struck the employee on EE's left hand behind EE's thumb causing a laceration.

Drive crew employee was working and accidentally bumped hardhat against a roller that was above the employee. The roller dislodged and fell out and hit employee's hand causing injury.

Employee was helping reload an arch top onto the scoop. The scoop operator misunderstood the direction the operator was given, causing the arch top to slide off, hitting the employee on the upper front part of left thigh.

While a beltman was cleaning debris from belt structure with a block hammer, the hammer slipped and was caught between the top belt and the belt roller. As the miner attempted to hold on to the hammer, their left arm was pulled into the belt/roller, resulting in an abrasion/contusion with 2 fractured finger tips.

Employee was sitting in MUV when it backed up and hit a rib board and then a plate hit employee on the bridge of nose causing a small injury.

EE was attempting to throw a track switch handle in the mine yard. The switch handle was stuck and when the EE pulled on the handle, EE felt a pain in their back.

EE just finished loading empty metal block skids on a rail flatcar and had placed a half of a hollow block on the car to get the forks out from under. EE brought the scoop back in to raise the skids to remove the blocks. EE reached in to remove the block and the skids slid off the forks smashing EE's rt. middle finger between the skid and block.

Employee was working on belt move and setting the tailpiece against the rib and caused injury to a finger.

EE was working monorail slack using the push/pull unit. EE rested hand on top of the monorail and the push/pull dolly pinched right middle finger between the monorail and the monorail dolly wheel.

EE was helping to load a trailing cable on a mule. EE stepped over the tongue of a lowboy & EE's foot got caught in the safety cable, causing the EE to fall forward landing on the left shoulder. EE continued to work w/o limitations until placed off work by an orthopedic surgeon on 2/9/2022.

EE was resetting the 995 volt breaker on the section power car. When the breaker reset, it arced and burnt the top of EE's left hand and fingers.

Employee stepped up on ledge to sign date board. As employee was leaving, lost footing and fell, twisting right knee.

EE was installing a dywidag into the mine roof, lost balance causing EE to fall from the elevated bolter platform to the mine floor hurting left knee. EE continued to work without restrictions until placed off from work by a Dr on 5/12/21

Employee was assisting in the retrieval of 7200 cable and got left hand caught between the edge of a diesel scoop bucket and a floor to roof jack. Employee received a laceration and a fracture to left index finger as a result of the accident.

Employee felt a pain in right shoulder while assisting another employee carry a dewatering pump. Employee continued to work without restrictions. Employee sought medical attention and on 3/18/21 was taken off work due to restrictions.

Employee lost footing while stepping down from a belt discharge and fell, injuring right shoulder. Employee continued to work with no restrictions until taken off work for surgery on 2/26/2021.

EE was standing too close to a shuttle car trailing cable and was struck by the cable when the operator started the pump motor. EE received a laceration under bottom lip.

Employee tripped and fell over a broken cement block that was laying in the crosscut. As a result, the employee received a laceration to the palm of the left hand.

Employee was struck in the head by a bottom stand that was lying in the haul road at 14-15 bk on 33 Butt intake. The stand flipped up and into the operator's deck of the scoop the employee was operating.

Employee received fractures to right pelvic area when the employee was caught between two pieces of rubber tired diesel equipment.

Employee received a laceration to the left ring finger while removing a bolt from the end of a hydraulic take-up jack. Once the bolt was removed, the jack fell and caught EE's finger between the jack and the storage unit framework.

An Unplanned and Reportable Roof Fall was discovered on 8/18/19 between 15-16 bk in the tailgate entry (#1 entry 30 butt)for the F-9 Longwall panel. A continuation of above mentioned roof fall occurred on 8/19/19 in the 15 bk 2-1 x-cut and intersection. There were no injuries, no equipment damage and the vent. system was not disrupted. Both occurrences were immediately reported.

Employee received a laceration to the face and dislocated left pinky finger when the employee slipped and fell from an elevated roof bolter platform to the mine floor.

Employee received a laceration to left index finger while attempting to release the coupler between two track mounted flat cars. The releasing mechanism slipped and pinched the employee's finger between the releasing mechanism and the spring.

An unplanned and reportable roof fall occurred at the 14 bk, #6 entry intersection on the D-Mains #1 additional intake that leads to the D-Mains working section (MMU 001-0 & MMU 005-0). Crew was using a sledgehammer to cut a block and during the process employee was hit by hammer causing injury to the finger.

Employee was struck by a rib board and received a contusion to the left side of the chest, when the Rubber tired diesel mantrip that the employee was a passenger in turned too sharply while backing out of a crosscut causing the rib support (rib board) to come into the mantrip.

Employee was removing spillage from under the belt (roller) with a pick and injured hand.

An unplanned and reportable roof fall occurred inby the section tailpiece in the #7 entry between the 25 Block and 26 Block on the D-Mains Working Section (MMU005-0). There was no equipment damage, no injuries and the ventilation system was not disrupted. This occurrence was immediately reported.

Employee was walking up entry and slipped in a hole causing injury to ankle.

Employee was installing bolt and was struck by bolt plate causing injury to elbow.

Employee received a laceration from a sharp edge of face chain while assisting an employee remove a broken section of AFC Face Conveyor chain from the Longwall panline.

Employee received a laceration and a fracture to the left thumb when it got caught between the latch mechanism and the release mechanism of a cable come-along while attempting to remove tension from a belt clamp.

Employee smashed right ring finger causing a fracture to the tip of the bone and an avulsion when EE's hand got caught between a diwydag sling and the cable come-along handle while lowering centenary belt structure.

An unplanned roof fall occurred inby the section tailpiece at the 36 Bk intersection in the #4 entry on the D-Mains (MMU 001-0) section. There were no injuries or disruption to the ventilation system, but the remote controlled continuous miner is under the fall. This occurrence was immediately reported.

Employee hit self with a pick hammer while building a wall causing injury.

Employee lost balance while on a belt crossover and fell on the moving belt underneath the crossover. Employee was pulled under the bottom of the crossover by the moving belt.

Employee was securing a jack with a sledgehammer and hit finger causing injury.

Employee was struck by a 48" belt rail that had slipped out of its stand and received a laceration to the back of right hand.

Employee stepped off the bottom step of a 6' step ladder and hurt right ankle while hanging 7200 cable behind the D-Mains section power car.

While working in the top floor of Grizzly Building, employee received a laceration to left ring finger and had a piece of metal embedded into right hand while attempting to remove a bearing using a 2lb. shop hammer.

An unplanned roof fall occurred inby the section loading point at the 15 bk intersection in the #4 entry on D-Mains (MMU 001-0) section. There were no injuries, equipment damage or disruption to the ventilation system. This occurrence was immediately reported.

Employee hurt lower back (right side)and right leg while assisting a co-worker load 60" top belt structure.

Employee was using a come-a-long on longwall face and was cut by a metal burr on a shield while ratcheting the come-a-long

Employee received a laceration to left forearm when a piece of rock fell from between two Longwall supports while moving inter-chock hoses.

An unplanned roof fall was discovered in the #1 entry, 119 Brk of the F-Mains return. There was no equipment involved, no injuries and the ventilation system was not disrupted. This occurrence was immediately reported.

Employee was attepting to re-hook a chain to the duster and smashed big toe causing injury.

Employee received a laceration to right eyelid when the sharp end of a piece of hanging plastic coated metal tie wire got in behind the employee's safety glasses while exiting the 913 scoop the employee was operating.

Employee stepped off of a working platform onto a wooden pallet. Right foot broke through a pallet broad, this caused EE's body to twist. EE's right foot was lodged in the pallet, causing the ankle injury. The pallet was used as a step.

Employee received a laceration to left middle and ring fingers and a fracture to left middle finger when the employee got hand caught between the canopy of the 913 scoop the employee was operating and a piece of vent tubing that was hanging low on one end.

Employee was advancing supports while the shearer was mining towards the tailgate. A piece of roof rock fell from between the supports and struck the employee on the left boot.

Employee was traveling through a 30 inch man door going from the belt entry over to the track (intake) entry. As the employee was going through the door ee noticed a concrete block in the walkway. The employee extended left leg to avoid the block and when ee went to stand up ee twisted left leg and felt a "pop" in left knee.

Employee received a contusion to the head when EE lost balance and fell from the second step of a 6 ft. step ladder at the F-Portal office building.

Driver was going out loaded on the Private Road and met another truck coming in at turn #6 moved over to the right to far and got into slush on side of road and it pulled truck into guard rail and over the bank. Truck came to rest on right side.

Employee was attempting to pressurize a hydraulic jack (duke), to re position a piece of long wall mining system hardware, and failed to adequately secure the pressure connector to the jack, which came off as ee pressurized the system, and struck ee on the right cheek.

Employee was assisting a co-worker lift a de-watering pump (ABS 50) that was stuck and needed unclogged. As they were lifting, employee felt a sharp pain in the left shoulder.

Employee was readying a plunger for drill steel using a utility knife. While attempting to remove the material from the center of the plunger, employee's razor knife slipped causing a laceration to left hand.

An intersection fell in immediately after being mined through, in/by the section loading point. There were no injuries, or equipment damage, or entrapment.

Employee was positioning a section rock duster machine (Bantam type duster), to load into a scoop bucket, at which time a piece of coal block rib fell out striking the employee, and knocking the employee down.

An unplanned roof fall was discovered at # 9 block of the # 2 entry on # 27 butt section track entry. There was no equipment involved, and no injuries, and the ventilation system was not disrupted. This occurrence was immediately reported.

Employee was walking in the belt entry and slipped/tripped/and fell, injuring right hand index finger.

An unplanned roof fall was discovered at #5 block on the East Mains section, in #2 return entry. There were no injuries, and no equipment damage, and the ventilation was not disrupted. This event was immediately reported.

Employee was assisting to make hydraulic/mechanical repairs on a roof bolting machine drill head/mast, at which time another employee functioned a hydraulic control lever which caused movement of the machine and injured the employee's left thumb.

Dismounted from the operators deck of an underground mining machine, and stepped down onto the mine floor, at which time employee twisted/rolled left ankle.

Employee was positioned at the left rear corner of the roof bolter, troubleshooting/looking for a hyd. oil leak, when a piece of cap rock dislodged from the mine roof and struck the employee on the head.

Employee was dismounting from the center walkway at the rear of a roof bolter, at which time the employee stepped on the mine floor and twisted left ankle, and felt pain.

An accidental/unplanned roof fall was discovered in an out/by area of the mine at # 114 A block F-Mains, in the track haulage entry. This roof fall area was subjected to constant temperature/ weather changes, and the immediate roof displayed water leakage in the fall area. There was no disruption to ventilation, no injuries, or equipment damage. Event was immediately reported.

Employee was repositioning a mine de-watering pump, by lifting on the steel handle which had a steel burr along it's edge, and lacerated the employee's finger

Employee was helping hang trailing cable from the mine roof, at which time an un-noticed piece of loose roof rock came down and struck employee in the lower lip.

Employee was retrieving ventilation straps from the mine roof, and while walking and looking up, the employee slipped/tripped and fell, causing pain in ankle.

Employee was attempting to cut a manilla rope, when employee inadvertently cut employee's hand

Surface area maintenance shop building was discovered on fire. Local EMS was summoned and extinguished the fire. The building and vehicles inside were destroyed. No additional surrounding property damage occurred. No injuries were incurred

Employee was working in an elevated position on a step ladder, when ee over extended lateral reach, causing the ladder to kick out. Employee then fell to the mine floor and felt pain in left ankle.

An unplanned roof fall occurred in an out/by area of the mine at # 2 block on 24 butt section, during an idle shift. There were no injuries and no equipment damage. There was no disruption to ventilation. The event was immediately reported to MSHA.

Employee was pulling on water line. Employee's foot got stuck on a track rail and employee fell and injured leg.

Employee was hand loading 1 inch I.D. X 7 foot long pieces of pipe into a Scoop bucket, at which time the employee overexerted themselves, and felt pain in right shoulder. Employee continued to work until 1/11/2017, at which time they sought medical attention, and was removed from work.

An unplanned roof fall occurred, with no interruption in ventilation, no injuries or equipment damage, and no escape ways were compromised.

Employee was troubleshooting a CM machine at which time employee came into contact with energized electrical conductors within the machine controller.

Employee stumbled/slipped/tripped while navigating in/through the center walk way on a roof bolter. The employee attempted to catch self, and incurred pain in right wrist.

Employee was assisting to unload a bundle of supplemental roof support (steel posts), out of a scoop bucket. The injured employee and the scoop operator un-chained the load from the bucket, at which time a loose steel post slid off the top of the bundle and struck the employee on the top of the left foot.

An unplanned roof fall occurred in an out by area of the mine at # 102 to # 103 block haulage road. There were no injuries. There was no equipment damage. There were no escape ways compromised. There was no disruption of mine ventilation. This event was immediately reported to MSHA.

While mining on a working section a water inundation occurred at the face. No injuries or equipment damage occurred. No escape ways were compromised. The section equipment was moved out by. Dewatering pumps were installed to remove the accumulation of water and control the inundation.

Employee was repairing a CM machine crawler track, and while exerting self attempting to align a retainer pin, slipped and contacted the steel edge of the crawler frame, causing injury (laceration),to right inside wrist.

Employee suffered a laceration on right thumb, while cleaning a ceiling light fixture in the mine portal building.

Employee was roof bolting and accidently trapped hand in the drill steel guide

Employee placed right hand on rib drill steel to retract steel from rib. EE inadvertently started rotation instead of retract operation. Glove caught on steel dislocating little finger of right hand. Dislocated finger was reset and place in splint to immobilize.

Employee was attempting to re-set a mine timber(Post), when EE stumbled and fell, with the post then striking EE on the head.

Employee was making a hydraulic repair to a longwall support, during which time high pressure emulsion fluid was released and contacted the employee's hand.

Employee was on top of a continuous mining machine cleaning foreign material (Rock) off the machine. When EE attempted to dismount from the machine, EE slipped and overexerted ankles and lower legs. The following morning the employee felt pain in lower legs when EE started walking. Employee continued working until 08/08/2016, at which time EE sought medical attention.

Employee struck himself on the mouth, while using a hand tool.

Employee was tramming a battery operated floor scrubber in reverse when employee got left thumb caught between control and a door causing a laceration on both side of thumb

Employee was removing extraneous materials from the belt entry and placing them in the track entry, and while doing this the employee over-exerted self, and incurred pain and swelling in employee's groin area.

Employee was operating a hydraulic impact wrench to install supplemental roof support when the wrench socket slipped off the apparatus and struck the employee's teeth.

Employee was assisting another person carry a post when employee stumbled and fell, at which time the post fell on employee's ankle

Employee was delivering mine supplies and attempted to re-enter the scoop machine operators compartment, at which time the employee stumbled and fell, striking employee's head/face on the steel structure of the operators compartment.

Employee was struck on the lower leg by a mine maintenance repair part when he attempted to assist another employee position/carry this part.

Employee was positioning a hydraulic ram jack and pinched his finger between the end of the hyd. ram jack and the AFC pan line, causing injury to his finger.

Employee was pushing pan line on the long wall when a roof fall occurred behind the supports, producing enough force to cause the employee to be jolted enough to fall.

An accidental roof fall was discovered by mine personnel, with no equipment damage, no injuries, and no interruption of the ventilation system occurring. The roof fall measured apprx. 35 feet long, 16 feet wide, and 8 feet high, falling approx. two feet above the anchorage point. The roof fall was immediately dangered off.

Employee was working in an elevated position on a step ladder, utilizing a pipe wrench to tighten fire suppression plumbing, at which time he incurred pain in his left shoulder.

Employee was positioned on a long/wall support pontoon to remove a relay bar bolt while the AFC panline was being dismantled/removed outby him. As the outby section of pan line was removed, it created a jarring motion of the pan line where the employee was working and struck the employees knee, causing pain/injury.

Employee was unloading ventilation tubing from shuttle car and felt pain in right shoulder. Employee was examiner at ER and was released to return to work. Employee worked his next scheduled shift and then followed up with his family physician and taken off work pending MRI diagnosis. First day missed for this injury is 10/29/2015

Employee was installing concrete blocks into a ventilation regulator wall, when one of the blocks fell out of wall and mashed the employees finger.

Employee was helping lay rail. While letting the rail down with a road jack, the jack handle slipped out of employee's hands and struck him on right eyebrow

A unintentional roof fall was noticed by mine personnel. No injury or equipment damage occurred, with no interruption to the ventilation system. The roof fall measured approx 25 feet long and 17 feet wide, falling aaprox 2 feet above the anchorage point. The roof fall area was immediately dangered off and supplemental roof support set around the perimeter of the fall area.

Employee was taking the DC shuttle cable off anchor and received a shock.

Employee was attempting to close the operators compartment door on a locomotive at which time the door came closed on his finger.

Employee was attempting to reposition a removed section of shearer rack bar on the AFC pan line at which time the employee's finger got pinched between the rack bar and the armored face conveyor pan.

Employee was wrapping ventilation tubing, and as the roll of tubing wrap was being tossed over top of the tubing, the employee missed catching the roll and it struck him on the face, causing a laceration to his upper lip.

Employee was attempting to retrieve a section of ventilation tubing that was hung with nylon straps to the mine roof. Employee utilized a knife to cut the nylon strap, at which time he also cut his finger.

Employee was removing couplings off 8 inch victaulic water pipe that was hung from the mine roof, when the pipe position shifted and pinched his finger between the pipe and the mine roof, causing injury.

Mountain View Mine incurred elevated water levels and inundation in our bleeder ventilation entries due to snow thaw and large amounts of rainfall, which disrupted the mine ventilation system.

Employee was performing maintenance on a roof bolting machine and placed his hand on a swing out type side guard, when a piece of mine brow rock came down and struck his left middle finger, causing injury.

Employee was working on a belt line and fell, as a result of not noticing the belt cross over he attempted to utilize was not yet secured to the belt structure.

A unintentional roof fall was discovered by mine personnel. No injury or equipment damage occurred. No interruption to the ventilation system and no escape way was blocked. Fall measured 16 ft wide X 24 ft long. This are had been supported to plan. The fall area was immediately dangered off and supplemental support set around the fall area.

Roof Bolter had just finished bolting a place, and was preparing to move. The employees foot was on the trailing cable as the cable reel was turned on, allowing the cable movement to jolt and injure his foot.

An unintentional roof fall was discovered. No injuries or equipment damage occurred. Fall area measured 16 feet high x 16 feet long x 16 feet wide. This fall area was roof and rib bolted according to plan.

A continuation of a previously reported unintentional roof fall on 9/2/2014 occurred on 9/8/2014. There were no injuries, entrapment, or equipment damage. This area has been dangered off, and additional supplemental roof support has been installed.

An unintentional roof fall occurred and was discovered by mine personnel. No injuries or equipment damage occurred. Fall area measured 13 feet wide x 16 feet long x 6 to 7 feet high. This fall area had previously been roof and rib bolted to plan, with the addition of supplemental roof support in the form of wire screen,& tensionable slings.

An unintentional roof fall occurred and was discovered by mine personnel. No injuries or equipment damage occurred. Fall area measured 8 feet high x 15 feet wide x 20 feet long. This fall area had previously been roof and rib bolted to plan, with roof screen installed. This fall material consists of "stack rock", clay seams, with water present.

Employee was using a wire rope and sheave block assembly to pull a piece of belt structure into an elevated installation position, when he got his hand caught in the moving cable and sheave block wheel, causing injury to his fingers.
