Employee was adjusting lifting shackle and overhead crane hook during lifting operations. The employee's right index finger was pinched between lifting shackle and overhead crane hook. The employee suffered partial traumatic transphalangeal amputation of right index finger with nailbed involvement.

Employee was removing panels from screen media with Air Tugger, when employee was holding Air Tugger chain with EE's left hand. Employees left hand became lodged in the Air Tugger Block causing laceration to EE's left hand.

While loading Haul Truck 36, a shovel operator trainee made contact with the back of HT36 with the dipper. After dumping the operator continued onto an additional run at Shovel 3. After another full load/dump cycle, the operator stated they felt neck and back pain.

Employee received first bucket from shovel 2 and noted to the operator and Dispatch/Supervisor that the load was rough. They stated at the time they were OK, needed no medical attention, and incident was initially classified as first aid. On 10/24 upon follow up with Occ Med Provider, employee received restrictions.

Employee was grinding overhead, when they removed their face shield, they felt eye irritation, foreign body was discovered that required removal techniques beyond first aid.

Haul Truck 43 was being loaded on the left side of Shovel 2 and had two or three dippers already loaded in the haul truck. Shovel 2 was going to load another dipper full when the operator swung left and made contact with the left corner of the dovetail causing haul truck 43 to pick up and move three to four feet, jarring the haul truck operator inside the cab.

Employee was operating haul truck, backing to be loaded at shovel when the shovel lost power and loaded bucket hit haul truck bed. Shaking up the employee.

While a rebar wall was being lifted by the crane, the wall pieces started to fall apart and a piece of rebar fell and struck our employee on the arm and another piece fell and hit EE's hard hat. EE ended up being taken to the hospital.

As the employee lifted the drill steel with the drills jib the steel got caught on the drill head, causing the bottom of the drill steel to kick towards the drill rig. When the drill steel kicked towards the drill the employee put EE's arms out to redirect the drill steel into place and from hitting the drill. The pipe hit EE's forearm against the metal frame of the safety gate.

A boulder fell out of the dipper teeth into Haul Truck bed, causing a rough load, resulting in neck and shoulder pain.

While engaging the bank to get a dipper full of material the teeth encountered a hard bottom jarring the shovel. This caused the foot latch on the operator seat to release, and the operator seat to slide back and hit the stops. When the seat hit the stops the operator aggravated an old back injury.

Employee was using the handle of the hammer to hold the bolt's head in place while another employee was running the impact. The bolt spun and the employee's hand slipped, coming in contact with the spinning bolt, causing a laceration that required stitches.

Employee was signaling crane to lower load. The load was being placed in front of crane and light plant was in EE's line of sight. Injured employee bent over to place cribbing on the ground. The load was stopped by the operator but shifted, striking the employee in the side of the head. This caused a laceration, requiring 8 stitches.

Employee was torqueing lug nuts on right front wheel of LD02, using a pneumatic torque gun. While positioning the torque gun and socket on a lug nut, EE did not move hand before pulling the trigger, smashing EE's right hand between the reaction arm of the torque gun and the loader wheel.

Employee (Employee 1) was working at the hoist drum with another employee (Employee 2) during hoist rope change. Employee 2 was running the hoist tuggers and Employee1 was working to guide the end of the hoist ropes into the holders on the hoist drum. While Lifting and pushing the end of the hoist rope into place, there was a pop sound from Employee 1 right arm.

Driller was attempting to slide the casing up the rod slide from the rod table and lost control of the casing when EE#'s right hand slipped off, causing it to drop down the slide unexpectedly. This resulted in the individuals left ring finger being caught between the casing and rods on the rod table. Injury resulted in amputation of the ring finger.

Employee complained of neck and back pain after receiving the first dipper of material from Shovel 6.

Employee was getting loaded at Shovel 6 with left arm resting on the window. Resulting in shoulder pain.

An operator and trainee on Haul Truck 22 were rough loaded at Shovel 4. The operator proceeded to dump load, tied down and contacted supervisor to report event. This resulted in the operator to experience lower back pain.

Employee was cleaning Shovel 6 pit area when employee struck a rock, jarring employee and causing lower back pain.

Employee was backing up the Haul Truck to be loaded at the Shovel. The employee turned to roll down the window to hear the horn of the Shovel. While the body was still turned, the shovel operator started loading the Haul Truck causing the employee to feel pain in back.

Employee was rough loaded while operating a Haul Truck. Shovel lost power causing dipper to drop and make contact with Haul Truck bed.

According to an independent medical review and MRI imaging received 10/6/20, the operator returned to work too early from a personal surgical repair, with the movement of the motor grader controls on 9/3/20 aggravating the shoulder and causing the need for a new surgery.

While employee was operating a grader, the employee got what EE believed to be dust in EE's eye.

Employee was installing a ground insulator onto a pole at the lineman yard (Clifton Training Center) when tightening a nut with an impact wrench, employee's left pointer finger was cut by a cotter key. Employee was wearing leather gloves that were torn during the incident.

As the operator pushed down, EE was backing up on uneven ground/boulder and the dozer slammed down which jarred the operator in the cab.

Employee was extracting singles from 4" pipe using an 18-volt cordless drill and spear. While operating drill employee's hand got wrapped around extracting spear that was attached to drill causing the employee's hand to twist resulting in injured fingers.

Rubber Tire Dozer 3 was cleaning up shovel 1 when EE hit a bump in the pit which resulted in the operator getting jolted around in the cab causing injury to EE's lower back.

The employee ran over a rock with a rubber tire dozer while cleaning up a shovel pit, causing a twinge in EE's back. The employee continued to operate the equipment and experienced pain in EE's back and neck after driving over a pothole.

Employee reported lower back pain after being loaded by a shovel in a haul truck.

Haul truck operator experienced neck pain from a possible loading event at the beginning of the shift and reported neck pain to supervision at the end of the shift. Shovel operator states a rock rolled out of the shovel dipper striking the truck bed. Haul truck operator states feeling a warm sensation in the side of the neck, and some stiffness by the end of the shift.

Employee was driving Haul Truck 28 when rough loaded by shovel 5. EE felt pain in left shoulder and neck after loading.

Employee was passenger in Haul Truck 28 when rough loaded by Shovel 5. EE felt pain in back after loading.

Two employees were moving sand line sheave to correct location. EE was pushing on the pin with fingers, while co-worker was pulling on the pin. The pin moved suddenly causing EE's fingers to follow the pin into the sheave. The sheave dropped damaging EE's right middle finger and fracturing the ring finger. Three stitches to the ring finger.

Employee was drilling a hole in pipe when the drill bit hung up causing employee's hand to twist.

Employee was replacing piping on the heat exchangers. Doing this required that employee lay under the piping in order to weld the flanges to the new piping. The employee returned to work on 5/13/2016 and reported pain in the lower back.

Employee was cleaning the Shovel pit using the rubber tire dozer. Employee had made an initial pass and was backing up when she struck a boulder.

Grinding, accumulated debris on face shield fell into eye.

While moving the J-hook out of the line of fire, the hook was between the upper and lower rails. Employee was trying to relocate the hook. The hook caught a hydraulic line while it was moving in the down position. Causing fingers to get smashed between the railing and the j-hook.

Employee suffered chipped tooth while driving T-post with post driver. Post driver caught tooth on T-post striking employee's hard hat and chipping employee's tooth. Employee seen by dentist and repaired with resin and returned to work.

Employee stated while being loaded by the #2 Shovel a rock fell off the dipper causing haul truck #24 to bounce. She began to get a headache, reporting the event to her supervisor later in the shift.

Employee was assisting in changing out the drill bit on Drill 2. Employee was signaling to the operator in the cab and took his eyes off his hands. As the drill bit and basket bottomed out on the drill pad, the basket and bit slammed together.

EE was using the service crane to left a copper cathode out of the wash tank when the clamp that was connected to the cathode slipped off and struck the EE in the center of the mouth.

Employee was assisting driller to change drill steel bit. The employee placed a small steel plate for a spacer between drill steel and tool wrench. When the wrench closed, the employee pinched left index finger.

Employee states that he finished cutting and welding on the tripper (latch bar) & dutchman on Shovel 4 and shortly after felt discomfort in his left eye.

Employee got dust in his eye from another employee grinding behind him. Injured employee was wearing his welding hood and safety glasses. Dust entered from the rear.

Employee was operating a Haul Truck and was in position, getting loaded by Shovel 1. Shovel 1 lost power and the dipper fell into bed of Haul Truck. Incident did not become a reportable incident until 8/6/2010 when employee was placed on restricted duty.

Employee was assisting in doing repairs to the drop chute on the 2140. The employee was positioned inside the chute using an Allen wrench to remove bolts on the liners. The employee positioned outside was using 3/4" drive impact. Mis-communication resulted in the employee inside the chute positioning his hand over a bolt that was being loosened by the impact wrench.

When Shovel 1 experienced a fault while loading Haul Truck 17, the dipper lowered into the bed of the haul truck. The haul truck driver was jostled in her seat while waiting to be loaded.
