Operator was assisting with metal cutting and handling. Afterward, EE felt irritation in EE's eye. EE attempted to flush EE's eye, but EE was unable to dislodge the object and was taken to the emergency room for treatment. The object work itself out on its own, but prescription antibiotics were prescribed (precautionary)

A maintenance welder was cutting plates from a haul truck bed in the rebuild process. While cutting away a section, there was stored energy that released and the plate struck the welder in the left arm and face. The upper part of the left arm received soft tissue damage including bruising and swelling.

A maintenance welder's left foot was injured requiring stitches. The welder was removing worn metal from a mining shovel bucket backplate. The backplate had been removed from the bucket and was laying on cribbing on the shop floor. While removing worn metal with a torch, the backplate slid slightly pinning EE's left foot to the ground causing injury.

During maintenance on the front-end loader bucket an employee (EE) placed their left hand under the plow bolt and while attempting the removal of the bolt, the sharp edges of the bolt head spun and lacerated the middle and index finger of the hand. EE was taken to onsite medic where stitches were determined.

While using a pressure washer, an employee was splashed with lime causing second degree chemical burns to the face and hands.

Tight work space, fell against wall after bolt unscrewed by the rattle gun

Conducting maintenance on a front end loader and was changing the cutting edge. EE was using an impact gun to remove a nut off a bolt and the bolt slipped causing a laceration to EE's hand.

Employee was removing Liners with the Liner handler. The bolts holding the seat to the liner handler sheared causing the seat, with the employee in it, to fall 88 inches to the charge, this broke employee's knee cap. Employee had seatbelt on.

Changing a bit. Operator was attempting to thread the bit on with the rotation on. Operator's sleeve caught in the chuck and pinched causing the material to rip and pull the operator around the steel. Operator was able to rip the sweat shirt to escape.

Injured worker was moving pipe in Millzone Pit, pipe slipped off excavator bucket and crushed left hand between pipe and rock.

Employee was using an air arc on an excavator bucket. Upon completing the task, the employee felt something in right eye. A physician later removed a small piece of metal from eye.

Temp employee was using a hole saw to cut out some metal and was using the blade to remove the piece of metal ee had just cut. EE failed to unplug the saw and the blade cut hand

While operating dozer with ripper, ripper slipped off rock, causing jarring motion to dozer. Employee bit down on teeth, causing fracture of left #19 tooth. Tooth requires extraction and implant.

Welder was using a wire brush on a mini-grinder to clean up a weld and the brush inadvertently ran across the back of the welder's right hand

A Cat EX1 excavator was loading a rock into the bed of a Cat 777 when the rock dropped out of the bucket and struck the truck bed. This action caused the bed to raise slightly and then came down jarring the operator in the cab. The truck driver complained 4 hours later of a pain in their back. The employee was examined by a physician and diagnosed with a low back strain.

Body placement- the employee face was too close to the where EE was operating the grinder, and EE was on the third rung of a ladder compromising EE's ability to handle the recoil of the grinder when it kicked back. The employee was only using one hand on the grinder.

Employee was installing a guard of expanded metal and drilling holes. When the hole was finished, left wrist was bleeding, the employee did not feel it and was unaware had been injured. During the investigation if was assumed the employee was cut on the expanded metal. There were no other sharp edges on the agitator. Employee received 3 stitches.

Employee was working on a exploration drill rig. He was steadying the lower end of a piece of drill casing as it was being threaded to casing that was already installed in drill hole. As it was being lifted into place, the employees right hand got mashed between two pieces of casing. He sustained a laceration, fracture and partial amputation to his right pinky finger.
