Employee reported eye irritation after roof bolting and the following week they sought additional medical attention, resulting in a small piece of metal removed from their right eye.

A roof bolter was putting glue and bolt in hole they had drilled in Unit 2, Entry 6 @ 24+60 when a piece of rock fell. This resulted in a small laceration between lip and nose that received stitches.

When the employee was tramming the roof bolter past the intersection in Unit 2, Entry 4 @ 8+40 to pick up hog panel, the employee bumped EE's right arm on the ATRS.

While roof bolter was hanging cable in Entry 10 XC in Unit 3, a cable reel from 416 RB struck the employee, causing employee to fall down.

Employee had just finished drilling top steel, and inserted bottom steel into the top steel, started bringing the pod (pot) to the bottom steel and missed inserting into the pod, causing the steel to flex away and pop back to their hand. This resulted in a small break at base of thumb.

Operator smashed finger between long arm boom and canopy post. Their right hand was on the drill boom while pushing the bolt up. This resulted in stitches on the tip of their right middle finger.

While backing the roof bolter out in Unit 2, the employee's safety glasses were struck with the roof bolter cable reel ball. The lens of their safety glasses lacerated their upper eyelid resulted in needing a stitch.

Employee roof bolting in Unit when they put their bottom steel in and raising pod up, the steel was not all the way in and it bent. This popped and struck their hand resulting in a sprain/bruise.

In Unit 1, an employee (roof bolter) was putting up the second bolt. The 5' stick pin was halfway in the hole when a bur on the bolt caught their glove. This resulted in a wrist strain.

The opposite operator's foot was struck when the operator advanced the side controls on the roof bolter, resulting in a deep tissue bruise.

Employee had their hand on the drill steel/wrench during the installation of a 5' roof bolt. Employee then activated the rotation of the drill pot to start bolt rotation. The rotation caught their glove. This resulted in a fractured thumb.

Employee experienced pain in their hands as they perform their daily tasks as a roof bolter. They continued to work until they were taken off for medical treatment on 1/25/2023.

Employee was doing repair work on the drill pot of a roof bolter. During this process the other operator tilted the front of the machine causing the employee to lose balance and fall forward into the bolt tray hitting head. Employee suffered a slight concussion and was taken off work by advisement of a doctor.

Employee was in the process of drilling a 6 foot hole for a roof bolt. Employee had completed drilling the length of the first drill steel and was in the process of adding the 2nd drill steel when the first steel fell out the drill hole and contacted employee's left pinky finger resulting in a fracture.

Employee was in the process of installing a roof bolt in the mine roof. Employee was guiding the bolt with hand as EE raised the drill pot. EE activated the rotation in this process and torqued EE's hand resulting in a fractured thumb.

Employee was installing hog panel wire mesh to the mine roof. During the process of installing this mesh to the mine roof, the edge of the wire mesh contacted the employee's arm causing a laceration that required stitches.

Employee was roof bolting when the drill steel became stuck in the drill chuck. EE took the drill wrench and hit the stuck drill steel. The wrench bounced back hitting the employee in the right eye brow causing a laceration requiring stitches.

Employee was in the process of installing a roof bolt. The roof bolt did not seat right in the drill chuck, while they had their hand on the roof bolt they activated the drill rotation to set the bolt in the chuck. The rotation of the bolt grabbed a glove and torqued their hand fracturing the left thumb.

Employee on 05/07/2021 was struck on the head and continued work until June when EE sought medical treatment.

The employee caught their hand on a moving roof bolter drill steel. The employee continued to work, but was taken out of work on 01/09/2020 due to medical advice and further treatment.

Cutting chain link with a grinder - grinder caught causing it to jump forward and slice employees thumb.

The employee ran out of hog rings in the clip gun. When the employee sat the gun down, the clip gun went off and severed the tip of employee's L index finger.

We are submitting this as an occupational illness because any condition is due to cumulative, repetitive trauma. The EE sustained a back injury at home and had previous back pain. Subsequently, claimed to experience back pain while roof bolting. EE was examined twice and released to work with no restriction. EE was examined a third time and issued a lifting restriction.

While roof bolting the employee felt pain in their left shoulder and arm. This employee was injured on 03/15/2017 and went out due to the injury on 04/12/2017.

The employee was getting some roof plates off the roof bolter when the roof bolter operator backed up the bolter and ran on the employees left foot.

The employee was grinding with a die grinder on a miner head lift jack when a piece of the metal hit the employee in the eye. One small piece of metal had to be removed from the employee's eye.

The employee was installing a belt splice pin when a piece of string got tangled with the employee's finger and the spinning pin.

Employee was using a cutting torch he touched his glove with the tip of the torch and the inner glove caught fire. The employee received 2nd degree burns to his left hand.

The employee was roof bolting when the drill steel bend and hit his finger.

The employee was installing mesh to the roof when his hand got caught between the roof and the ATRS causing a fracture and stitches to his right hand.

Employee was using a cutting torch on a miner cover when a piece of slag went down their boot causing 2nd degree burns to their lower leg.

The employee was getting off the ground, putting his hand on the pan of the miner when the miner head was lowered striking the employees hand.

The employee was installing a roof bolt and using his boom to push the bolt into the roof. The employee mashed the tip of his left ring finger between the side of the roof bolt wrench and the bolt being installed. The employee received stitches in the finger on 08/23/2013.

Employee got his hand between the roof bolt plate and the auger clamp.

Employee was cutting with a cutting torch on a Hyd. Cylinder on the ram car when the pin exploded, burning knee, left arm, left wrist.

While putting 2nd drill steel in the drill box the top steel fell out of the roof and hit him in the left hand, it did require sutures.

Employee was bolting and installing roof bolt into drilled hole when his thumb was caught between roof plate and wrench.

Employee saw a rock that he thought was loose. He tried to pry it down, then decided to spot bolt it up. As he was drilling, the rock broke and fell, landed on the protective canopy over his head, and then bounced off, striking the employee on his left shoulder. He fell down and injured his left knee also. The area had been bolted and meshed prior to the incident.

Employee broke his left thumb when it was struck by a drill steel.

Employee had his right hand on the miner and was looking at the Jana screen. As someone else attempted to start the cutting motors, the employee said that the miner "bit" him. Despite the uncertainty as to whether this was a mine related injury, we are reporting it as a precautionary measure.

Employee lowered drill pot onto right foot.

While moving the cable, a rock slid off the top of the machine and struck his finger. His finger required sutures. He returned to work the following day.

A bundle of Hico beams were lowered onto a concrete slab. The load was set on the rigging and raised to remove the rigging. The EE's finger was pinched between the material.
