While working as a miner operator, employee reports numbness in both right and left hands up to elbows and, at times, in fingertips. Employee continued to work until January 28th.

While working as a ram car driver employee experiences that wrists and elbows hurt/have numbness. They continued to work until 1/30/2026.

A roof fall occurred at this location on 12/18/2025. The fall location was in the 1st panel south off the 3rd main west (Unit 4), entry 9, in the intersection at survey station 42+00 (71xc). The fall measured 22ft wide by 24ft long by 6-8ft high. No one was injured and ventilation was not affected.

A roof fall occurred at this location on 12/12/2025. The fall location was in the 1st panel north off the 3rd main west (Unit 3), entry 4, in the intersection at survey station 18+00 (31xc). The fall measured 18ft wide by 20ft long by 5-7ft high. No one was injured and ventilation was not affected.

While roof bolting in 3L, employee was hitting a clogged drill steel when EE felt pain in EE's right shoulder blade. EE continued to work until EE was taken off work later in November.

While pulling down exhaust curtain with their left hand in Unit 1, Entry 12, 44+40 - 45+00. In doing so, a section of rock fell out, striking the contractor on their head and left side.

While lifting a belt crossover to back on it's legs it slipped. This caused the employee to jam their arm/elbow into the crossover legs/skirt boards on the pony drive in Unit 3.

Employee reported occasional tingling in wrists and hands. EE continued to work until surgery on 1/5/2026.

Employee was unloading a box of SCSR's off the back of a Kubota when it slipped. Employee heard pop in left arm around elbow area and felt pain/numbness. Employee continued to work until surgery on 10/31/2025.

Employee reported intermittent pain with overhead work.

While passenger in mantrip leaving Unit 1, they hit a hole causing employee to pop up and land on hammer in tool belt, striking the lower right side of back.

While carrying belt structure, employee felt pain in right arm. They tripped while carrying a top roller and felt pop in right elbow/forearm area.

An employee was bent over working on suction line when a piece of rock fell out between bolts in Unit 1, Entry 4 at 18+61, striking the employee on the lower back.

When the employee was tramming the roof bolter past the intersection in Unit 2, Entry 4 @ 8+40 to pick up hog panel, the employee bumped EE's right arm on the ATRS.

A roof fall was discovered on July 16, 2025 at approximately 4:00am. The location of the fall was on the 1st North Panel off the 3rd Main West at survey station 32+60 (xc 45) in Entry 9.

Employee felt pain in lower abdomen when loading trash and pallets in the dumpster in Unit 3.

Contractor strained shoulder while pulling down curtain in Unit 3, entry 11.

Employee reported an occupational illness prior to shift start. They continued to work until they went out for surgery the following month.

An unplanned roof fall occurred on Unit #4 between entries 7-9 at survey station 33+00.

After cutting bands on a stack of curtain boards outby U2, xc 108, supply entry 5, some boards fell on the employees leg, causing a strain and small abrasion to the outer side of their left knee.

While pulling down curtain employee felt pop in upper right arm/shoulder.

While stacking a box of miner bits on supply tractor, employee felt a pop in upper arm by right shoulder.

When turning to speak with supervisor, employee bumped their mouth on the canopy of the roof bolter.

Employee changing out link on conveyor chain in Unit 4, Entry 10. While driving link back together a small piece of metal chipped off and imbedded in upper thigh. It was removed and stitched closed.

Ram car driver in Unit 1, #10 entry 29+40 between XC 29+40 - 30+00 reached out of the ram car to get drops out of the way to better see miner operator and caught middle and ring finger of right hand between the rib and canopy of the ram car.

A roof fall was discovered at cross cut #14, 1 North 3 (Unit #3), entry #6, Survey Station 7+80.

Struck in mouth by a steel for boring holes in 2x4. After finishing making a hole in 2x4, when removing steel, employee punched self in the face with steel, resulting in a chipped front tooth.

While walking around maintenance sled employee stated they tripped and tried to catch themself then stepped in a hole at the same time. Felt sharp pain in shoulder.

Employee was shoveling on the 1ME belt at XC35 when they felt a sharp pain in the lower right abdomen.

Employee slipped on ladder while taking down drops in Unit 1 for a belt and power move. EE was using a 4' ladder and stepped on the 3rd step when EE slipped. EE landed on EE's left foot and stated EE rolled EE's foot and twisted EE's left knee.

Employee was pulling down on a piece of curtain attached to hog panel. They fell to the ground on their right shoulder.

While roof bolter was hanging cable in Entry 10 XC in Unit 3, a cable reel from 416 RB struck the employee, causing employee to fall down.

Employee had just finished drilling top steel, and inserted bottom steel into the top steel, started bringing the pod (pot) to the bottom steel and missed inserting into the pod, causing the steel to flex away and pop back to their hand. This resulted in a small break at base of thumb.

While building a stopping, employee pulled a block off the pallet which struck EE's left wrist/forearm resulting in a small abrasion and strain on forearm.

A roof fall was discovered on 11/8/2024 on the 3MW at survey station 45+60, between entries 4 & 5.

Employee walked into staging area after exiting mantrip and felt abdominal pain with pulling sensation. Employee continued to work until March 13, 2025.

Employee felt pain in stomach while lifting belt structure in Unit 1 Pony Drive between XC7-8. After further evaluation it was determined to be a hernia.

Employee reported pain in both elbows and hands. They continued to work until further evaluation determined the need to be taken off in November.

Operator smashed finger between long arm boom and canopy post. Their right hand was on the drill boom while pushing the bolt up. This resulted in stitches on the tip of their right middle finger.

While rolling up curtain in Unit 1 (entry 1, 19+20) a rib popped out causing the employee to fall to ground. This resulted in a fibula fracture.

Employee struck their face on the canopy of #517 scoop when getting into the scoop. This resulted in a laceration of their eyebrow.

Employee was in Unit 1 (13 R, XC-28, 16+20), when they put outside bolt swing in to get the inside bolt, a piece of rock rolled off the canopy, striking the employee and resulting in a lower back strain.

A roof fall was discovered at cross cut #55, 1 North 3 Panel(Unit 3), entry #9, Survey Station (38+60).

Employee strained middle back while pulling pump out of road water truck.

Employee was walking around the corner in entry #12 in the last open XC when a brow area popped out and struck their shoulder when they were walking past.

Operator was installing inside bolt when a rock slid off canopy and slid down. When the employee went to move out of the way they rolled their ankle resulting in a small fracture.

Repetitive motion claim of numbness to hand, elbows, and fingers. Employee was taken off work later for further evaluation.

A roof fall was discovered in Unit #2, entry #7, at Survey Station 9+00(16XC).

While backing the roof bolter out in Unit 2, the employee's safety glasses were struck with the roof bolter cable reel ball. The lens of their safety glasses lacerated their upper eyelid resulted in needing a stitch.

Bolter operator was putting up outside bolt when the weight of a rock bent the hog panel. The employee was holding up a bolt when the hog panel pushed their hand, causing them to jerk their arm back quickly. The jerking motion caused a strain in their upper right arm/shoulder. They were taken off work on June 3rd.

Employee was loading pin boards bundles and when employee threw a bundle it was heavier than they realized and pulled employee's shoulder back. Employee continued to work until 6/6/24 when employee was taken off for additional evaluation.

The employee loaded block and a mandoor in the bucket of their scoop. The door slid and struck the employee on EE's right shin causing a laceration that was treated with stitches.

Employee lost their footing when stepping off ladder in Unit 3 Battery Barn. They fell to ground and strained their knee. They were seen by onsite medical and offsite medical. They continued to work until they were taken off for their January 2025 surgery.

Employee reports numbness in hands/wrists. Continued to work until was taken off for surgery on 6/26/2024.

Employee was walking to mantrip in breezeway. Employee's foot fell in an opening in the grate in the breezeway that was 6" wide.

Employee reported numbness and dull pain. Continued to work until 6/5/2024.

When making the first turn in Unit 4, entry #11 @ 9+60, employee was struck by a rock that fell from between bolts.

Employee pulled on an 8' bolt that broke at its stamp. This motion caused them to stumble backward and roll their left ankle.

While setting a timber, the employee climbed on a piece of coal to cap off the timer and fell on their side and then was struck by the timber. After evaluation and treatment they continued to work until April 15th.

While undoing twists on the tie wire holding one end of the battery jumper, the plug came loose and struck EE in the shoulder.

A roof fall was discovered in Unit 3, entry #14, between Survey Station 80+60 and 81+20.

Employee reported elbow and wrist pain when roof bolting. EE was evaluated and continued to work until taken off for surgery.

Employee reported shooting pain in hands, along with numbness and swelling. After evaluations, continued to work until taken off for treatment.

Employee roof bolting in Unit when they put their bottom steel in and raising pod up, the steel was not all the way in and it bent. This popped and struck their hand resulting in a sprain/bruise.

Employee states that their wrists and elbows were sore from repetitive motion. The employee continued to work until they had surgery.

Employee reports numbness and tingling in hands and wrists. Continued to work until January 10, 2024.

In Unit 1, an employee (roof bolter) was putting up the second bolt. The 5' stick pin was halfway in the hole when a bur on the bolt caught their glove. This resulted in a wrist strain.

The opposite operator's foot was struck when the operator advanced the side controls on the roof bolter, resulting in a deep tissue bruise.

Employee was outside of ram car in Entry 2 (12+60) in Unit 4. The tailgate of the ram car was sticking out into the intersection when it was struck by a battery scoop cab. This shifted the ram car and the tire rub rail stuck the employees right knee into the rib.

Employee was hanging cable and felt their wrist pop. It started hurting later and was evaluated for repetitive motion. Employee continued working until surgery.

Reported pain while using a hammer and while squeezing items while picking them up. Employee continued to work until was taken off for surgery on 12/11/2023.

A roof fall was discovered in Unit 4 at Survey Station 8+40 (15 XC), Entry 8L.

Employee stated that while pulling bolts off of tray and placing them in roof, they felt a strain in their right shoulder. Treated for muscle ache and taken off work on 9/18/2023.

Repetitive motion of right wrist up to right shoulder. Employee continued to work until surgery on 9/27/2023.

While lifting miner cable up during power move, employee felt a sharp pain in right shoulder. EE continued to work until 7/27/2023.

Employee was working on mantrip in the top shop. When kneeling and removing the u-joint bolts, the axle shifted and rolled off the forks, pinning the employee's arm to the floor. This resulted in injury to EE's right arm, wrist and hand.

A roof fall occurred in the intersection of Entry #5 at crosscut #16 on the 3rd Main West Travelroad. Entry #5 is designated as the Secondary Escapeway.

When loading a roll of conspec wire into the back of a vehicle, shoulder was strained and roll dropped into ride.

Throwing miner cable over and tripped over gob. Fell into rib and caught themselves with their hands. Released to return to work. Began missing work from Dr. note on 4/27/2023.

While on the return side of a partially built stopping the employee was struck by a man door and fell forward onto EE's pallet of material. EE continued to work until EE was taken off on April 4, 2023.

Employee was installing cells in a coal hauler battery, when employee stepped back and tripped backwards, falling to the ground. Employee continued to work until employee was taken off by a medical doctor on 2/27/2023.

While installing a roof bolt, employee felt a pop in their left shoulder. They were physically placing the bolt in the hole at the time and their left arm dropped.

Employee was climbing on the back of the miner for a repair and felt something pop in their left knee. They continued to work until they were taken off by a medical doctor on 2/13/2023.

A roof fall occurred in Entry #5 at #126 crosscut on the Main North Travel way which is the secondary escapeway.

Employee had their hand on the drill steel/wrench during the installation of a 5' roof bolt. Employee then activated the rotation of the drill pot to start bolt rotation. The rotation caught their glove. This resulted in a fractured thumb.

A roof fall occurred in the crosscut and partial intersection of 55 crosscut on the 3rd Main West Belt. The roof fall was approximately 8 feet high, 18 feet wide, and 18 feet long.

Employee reported hands and elbows have been going numb and hurting. Employee continued to work until 2/15/2023 when EE was taken off for medical treatment.

While scaling down loose top, a piece approximately 3 x 3 x approx. 3.5 to 5 thick came down, striking the employee's right lower leg, resulting in a fracture.

Employee was carrying a set of wooden pie pans to bolter. Felt pain in shoulder. The employee was carrying one bundle with both hands.

Employee experienced pain in their hands as they perform their daily tasks as a roof bolter. They continued to work until they were taken off for medical treatment on 1/25/2023.

Employee is experiencing pain in shoulders, arms, and wrists while performing regular duties as a mine tech. Employee continued to work until 12/27/2022 when employee started leave.

A roof fall was found in the intersection of #13 entry at 84 crosscut in the 3rd Main West.

Employee was recovering fire suppression pipe, this one piece of pipe was in a bind in the mounting bracket. Employee loosened the bracket holding the pipe causing the pipe to move abruptly. It contacted their hand resulting in two fractured fingers and a laceration to the tip of one of the fractured fingers.

Employee was doing repair work on the drill pot of a roof bolter. During this process the other operator tilted the front of the machine causing the employee to lose balance and fall forward into the bolt tray hitting head. Employee suffered a slight concussion and was taken off work by advisement of a doctor.

Employee was in the surface shop working on a piece of equipment. EE was on EE's knees driving a pin out with a sledge hammer. EE was swinging to hit the pin and missed the pin becoming off balanced falling on EE's right shoulder and arm. Incident occurred on 08/29/22 EE worked until 09/27/22 when EE was taken off work for medical treatment.

Employee felt pain in EE's side and abdomen and was evaluated in August 2022. It was diagnosed as a non-work related strain. EE continued to work until EE was re-evaluated. EE was taken off for medical treatment in May 2023.

Employee had removed the suction plate on the bottom of the roofbolter drill pot and was in the process of re-installing this plate. While installing the plate it slipped out of their grip, falling approximately 16 inches contacting their shoulder. Employee continued to work until until 08/10/2022 when they were placed on restrictive duty by a doctor due to discomfort in shoulder.

Employee was using a pry bar while standing on concrete forms. The pry bar slipped and the employee fell approximately four feet landing on EE's left side.

Employee is experiencing pain and numbness in hands and elbows while performing employee's daily tasks as a maintenance technician. Employee worked until 8/29/2022 when employee was taken off work.

A roof fall was found in the secondary escapeway on the 2nd panel north travel way. The fall was located in Entry 7 between XC4 and XC6. The fall measured approximately 60 feet long x 20 feet wide x 5 feet high.

Employee was getting out of ram car and tripped, causing EE to fall against the rib and to the ground. EE continued to work until EE's injury was aggravated and was taken off for medical care.

Employee was picking up a belt roller cradle when they felt a pop in their shoulder. This employee continued to worked until 08/29/2022 when they were taken off work by a medical doctor.

While performing service work on a continuous miner, the EE raised a hinged door and rested it on their shoulder. The EE had slight body movement, causing the door to slip and fall onto their left hand. This resulted in a fracture of the left hand.

Employee states: walked into a roof bolt plywood square and jammed their neck. They worked until June when they decided they needed medical treatment.

Employee was in the process of drilling a 6 foot hole for a roof bolt. Employee had completed drilling the length of the first drill steel and was in the process of adding the 2nd drill steel when the first steel fell out the drill hole and contacted employee's left pinky finger resulting in a fracture.

EE tripped and fell while walking. Continued to work until July when EE was taken off for further treatment.

Employee is experiencing numbness and pain in EE's hands and wrists while performing EE's daily task as a ramcar operator. EE continued to work until 05/25/2022 when EE was taken off work for medical treatment.

A roof fall occurred in the 2nd Panel North off the 1st Main East in Entry #4 at 29+40. This location is inby the section loading point. The dimensions of the fall are approximately 19' wide by 25' long by 8' high.

Employee was in the process of installing a roof bolt in the mine roof. Employee was guiding the bolt with hand as EE raised the drill pot. EE activated the rotation in this process and torqued EE's hand resulting in a fractured thumb.

Employee stats that they are feeling pain and numbness in both wrists and hands. Employee continued to work until they were taken off for medical treatment.

Employee was installing hog panel wire mesh to the mine roof. During the process of installing this mesh to the mine roof, the edge of the wire mesh contacted the employee's arm causing a laceration that required stitches.

A roof fall was found in the primary escapeway on the 1st Main East Intake. The fall was located in Entry #8 between xc31 and xc32. The fall measured approximately 15 feet long, 20 feet wide, and 8 feet high.

Contractor employee was building a stopping using concrete blocks. Contractor picked up a concrete block and put it in place when they felt pain in shoulder

Contractor employee was helping load angle iron metal pieces when one the pieces rolled and contacted EE's right pinky finger resulting in a fracture.

Employee states they are feeling sharp pain shooting through both hands/wrists into both elbows and left shoulder while bolting.

Employee was hand loading a valve bank for a U/G roof bolter. When they loaded the valve bank on to the transportation vehicle it shifted causing the employee to strain their back. The employee continued to work until they were taken off work on 03/10/2022 for medical treatment.

Employee is experiencing pain in the hands as EE performs daily tasks as a U/G mechanic.

Employee was walking in the U/G production unit when EE tripped and fell to the mine floor injuring their shoulder.

Employee was involved with the removal of an articulation jack on a machine. Employee was holding the the pin removal hammer while another employee was hitting it with a sledge hammer. The sledge hammer glanced off the pin hammer striking employee's finger, resulting in a fracture to that finger.

Employee was walking from the working face to the tool bench when EE tripped and fell to the mine floor catching self with EE's hand and arm, jamming EE's shoulder. This incident occurred on 01/10/2022, employee continued working until 01/20/2022 when EE was taken off work by EE's doctor for treatment.

Employee experiences pain and numbness in right hand and elbow while performing tasks as a U/G mechanic.

IW was working next to another EE when the other EE was unhooking chains from material. The chains stored their own energy and as such when the last chain was unhooked from position it swung toward the IW striking the IW in the mouth causing laceration with repair by sutures.

Employee was exiting the operators cab of a ram car when their knee "popped" and they felt pain also.

Rocks were being removed from the continuous miner. Employee stepped up to remove the pulling rope off the rock that been pulled off machine. As EE stepped up, EE stepped on a rock this rock was sitting on causing the rock to shift and wedge EE's foot resulting in a fracture.

Employee was roof bolting when the drill steel became stuck in the drill chuck. EE took the drill wrench and hit the stuck drill steel. The wrench bounced back hitting the employee in the right eye brow causing a laceration requiring stitches.

Employee was using a pry bar to remove a rock off the continuous miner when employee felt pain in right elbow. Employee continued to work until 04/05/2022 when employee's doctor took employee off work for surgical treatment of the elbow.

Employee was installing hog paneling mesh to the mine roof. Employee was attaching it to the ATRS of the roof bolter when a piece of rock fell out of the mine roof contacting them on the head, lacerating left ear that required stitches and the doctor also took them off work for this injury.

Employee is experiencing pain and numbness in their hands and elbows while performing daily work tasks. This was reported on 11/10/2021, employee continued to work until 01/13/2022 when EE was taken off work by a medical doctor.

Employee is having pain and discomfort in hands, elbows and shoulders while performing daily job assignments. This was reported on 11/04/2021, employee continued to work until 12/01/2021 when employee was taken off work for medical treatment.

A roof fall was found in the examiner's weekly return examination route. The fall was located in Entry #1 between #108 and #109 crosscuts of the 2nd Panel North off the 1st Main East. The fall was approximately 8 feet high, 20 feet wide, and 50 feet long.

Employee was walking across the U/G working section when EE stepped in a hole/rut twisting left knee.

After completing a mucking cycle IE was waiting on drill basket to be sent in. IE was standing at shaft bottom when a rock loosened behind the shaft liner and fell from approximately 33' striking IE in the back of the head, neck, and shoulders. The loose rock come from fire clay that was saturated with water causing a piece of black shale approximately 18"x6"x6" to fall.

Employee experienced some pain in their back and right arm as EE was helping hang miner cable. This incident was reported on 09/22/2021, employee continued to work until 10/12/2021 when EE was taken off work by their doctor for treatment.

A roof fall occurred in the 2nd North off the 2nd Main East Panel. The roof fall was located in Entry #9 at 13+80. A hidden slip was evident in the anchorage zone after the fall had occurred.

Employee was replacing cutting bits on the head of the continuous miner. EE went to hit on the bit removal tool with a hammer and missed hitting EE's right pinky finger. This resulted in a crushing injury to that finger requiring a surgical procedure.

Employee was changing out worn miner bits and was using a hammer and a chisel in this process. When hitting the chisel a sliver of metal came off and imbedded in EE's left forearm. EE continued to work until 10/06/2021 when EE had it removed and received 3 stitches.

Employee feels pain and numbness in EE's left arm and hand while performing EE's work assignments. This incident was reported on 08/24/2021 but employee continued to work until 10/27/2021 when EE was taken off work for medical treatment, surgery for carpal tunnel.

Employee was assisting with removing the jack pipes from the unit tail piece during a belt advancement. As employee pulled the pipe out of the rib notch, employee felt pain in groin. After medical evaluation it was determined that employee has a hernia that does not require medical treatment at present. This incident occurred on 8/18/2021.

Employee is experiencing numbness and pain in hands and arms while performing job as a roof bolter. Incident was reported on 08/17/2021, employee continued to work until 10/06/2021 when employee was taken off work for surgery.

While setting a timber, the employee was struck by a piece of rock resulting in a neck strain. EE continued to work until additional treatment was sought.

Employee reported hands are falling asleep and elbows are hurting. EE continued to work until EE was taken off for surgery in June 2023.

Employee was helping move the roof bolter out of a cut and was handling the trailing cable. A piece of rock fell out of between the roof bolts contacting EE's right forearm causing a laceration that required stitches.

Employee is experiencing pain and numbness in hands and elbows while performing job as a belt mechanic. This incident was reported on 6/29/2021 but employee continued to work until 8/27/2021 when taken off work for surgery.

Employee was walking alongside the roof bolter and had EE's hand on the side of the machine. A piece of rock fell out of the roof and contacted EE's hand resulting in a bruised hand. After further medical evaluation EE was taken off work on 06/18/2021.

Employee was in the process of installing a roof bolt. The roof bolt did not seat right in the drill chuck, while they had their hand on the roof bolt they activated the drill rotation to set the bolt in the chuck. The rotation of the bolt grabbed a glove and torqued their hand fracturing the left thumb.

Employee is experiencing numbness and pain in both hands and elbows while performing daily job tasks. This incident was reported on 05/17/2021, employee continued to work until 06/16/2021 when EE went out for surgery.

Employee on 05/07/2021 was struck on the head and continued work until June when EE sought medical treatment.

Employee is experiencing numbness and pain in hands and elbows as EE performs job as a roof bolter. This incident was reported on 4/29/2021. EE continued to work until 6/9/2021 when EE was taken out for surgery.

Employee is experiencing pain and numbness in both hands while performing daily job tasks which involves some repetitive motions. This incident was reported on 4/28/2021, employee continued to work until 6/03/2021 when employee went out for surgery.

A roof fall occurred in the Main North intake entry number 3 at S.S.70+73(90 x cut). This area is in the mine examiners weekly route. The fall was 20 wide x 40 feet long x 10 feet high.

Employee experiences pain in both hands and elbows while performing EE's daily job tasks. This was reported on 04/07/2021, employee continued to work until 05/14/2021 when EE was taken out for surgery for carpel tunnel.

Employee was performing maintenance duties. While EE was getting down off of a roof bolter, EE fell and injured EE's left shoulder. This occurred on 04/03/2021. Due to medical opinion EE was removed from duty 04/03/2021.

Employee was performing maintenance duties. EE was moving a wheel hub and felt a pop in EE's groin area on 03/20/2021. Due to medical opinion EE was removed from work on 04/02/2021.

Employee was doing service work on a piece of equipment, while walking around the machine EE tripped and fell injuring EE's right shoulder. Employee continued to work until 05/14/2021 when EE was taken out for surgery.

In the process of pulling a rock off of the Continuous Miner a piece of the rock came off and contacted the employee; The result of this was a laceration over the left eye requiring stitches.

Employee was beating out a plugged drill steel and accidently hit EE's left index finger.

The employee felt numbness while working in employee's hands. The alleged carpal tunnel syndrome injury occurred 01/12/2021. The employee went out for surgery on 03/19/2021.

Employee was doing gunite work on the East Chevron doors, EE was climbing up a ladder pulling the hose with them. EE's head contacted the mine roof jamming head and neck. This incident was reported on 12/10/2020, employee continued to work until they were taken off work by a doctor on 06/28/2021.

This employee was at the Main East belt drive area when their hand came in contact with the bottom belt, which resulted in their arm contacting the angle iron support bracket for the return idler pulley. This resulted in a fractured left forearm. The employee had not been given any asignment by management at this belt drive area.

Employee was carrying a bag of rock dust and had a slip/trip/fall event. In the process of falling employee damaged right shoulder.
