Employee was splitting a rock in the holder. The rock split in two vertically, and the outside piece fell on their foot.
Employee was trimming slate chips using Trimmer #1. She had a chip in the trimmer when the blade got stuck on the left side of the machine in the rock. She tried pushing the rock in, and the blade let loose and cut her finger. She had her fingers on top of the slate which is an unsafe act. Finger are always to be underneath the slate chip.
