Employee stated EE was in the process of bolting the first cut out of 3L on the 007 MMU. After EE finished the first row, draw rock feel from the roof in by the ATRS. Resulting in a piece of draw rock falling into EE's drilling station and striking EE on the right leg.

Employee was finishing EE's inside bolt when EE went to drop EE's canopy. The canopy post collar dropped down striking EE's right pointer finger. This resulted in a small distal fracture of the right pointer finger.

Employee pulled bolting machine up in #1 heading and spotted a roof bolt, then sounded the roof and stated it sounded drummy and then tried to pull down a piece of rock but was unable to. EE then drilled a hole to bolt it up and was making up a bolt to installed when a piece of rock fell striking EE on the left forearm. This resulted in 5 stitches.

Employee states EE was bolting in #4, in 4R entry, first turn. EE had just completed installing EE's first bolt on EE's 3rd row and went to swing in to install EE's 2nd bolt. When EE swung in, a piece of draw rock fell from the mine roof striking EE's right foot resulting in a fracture to EE's 1st metatarsal.

Employee was assisting their bolting partner tram their roof bolt machine from #4 right to #4 left. While tramming the roof bolt machine, the operator did not see the employee at the rear of the machine while getting into position in #4 Left and caught the injured employee between the roof bolt machine and the coal rib.

Employee was drilling the outside bolt on EE's 5th row. EE's right hand was resting on EE's right knee when a piece of rock fell from the roof striking EE's hand. This caused a laceration between EE's thumb and pointer finger resulting in 7 stitches.

Employee was holding cable over for drill in #5 while drill partner trammed drill into #5 behind drill bolting #5 to get drill out of miner's path of travel. Employee tramming drill not did see employee holding cable over and caught EE between the drill and coal rib. Left abdomen and left hand were the injured.

Employee stated employee was pushing a bolt up. Employee had ahold of the pan and it caught the edge of a brow pushing the plate into employee's right hand. This resulted in a laceration to employee's right index finger requiring stitches.

Drill steel was bent, rotation was hit causing steel to break. It spun around and cut employee's right forearm. This resulted in a laceration requiring stitches.

Employee stated while putting bolt into strap, it got in a bind. The bolt bent, hit rotation and bolt struck left arm causing a laceration resulting in stitches.

Employee was starting to drill center bolt on first row of cut. Employee started drilling hole and stated EE drilled approx. 1.5" in roof. This caused a piece of rock measuring 48" long x 20" wide x 0-1.75" thick to start spinning its way down drill steel striking EE in the side of the nose and below left eye.

Employee was roof bolting when a piece of rock fell hitting the t-bar and then hitting them on right ear causing a laceration requiring stitches.

Employee stated while EE was removing the drill steel after drilling a rib bolt, EE's bolt partner applied pressure to the drill head causing the drill to shift, smashing EE's right middle finger between the drill steel coupling. This resulted in a fracture and stitches.

Employee was in the process of drilling a 16' hole for a cable bolt. Employee was attempting to line the steel up with the chuck. The employee bumped the rotation, at which time employee's glove got caught between the drill steel and chuck resulting in a dislocation to the middle finger and fracture to the ring finger on the left hand.

Employee was welding on excavator bucket. The bucket had been removed from the machine. While welding the shank onto the bucket, the bucket rolled toward EE pinning both feet. This resulted in a fracture of the 5th metatarsal on EE's left foot.

Employee was getting ready to start a cable bolt hole. They had drill steel pressed against the top and when they applied pressure a piece of draw rock fell from the top smashing their left middle finger between the drill pot and draw rock. This resulted in a fracture and 6 stitches.

Employee stated was in the process of starting the fourth row in #5 heading. When EE swung the drill station out to put up the outside bolt, EE's left middle finger came in contact with the rib. This resulted in a fracture and stitches.

Employee was installing roof bolt in the hole after drilling. Bolt was bent. While using both hands straightening bolt out to put in drill chuck, the bolt turned which resulted in a pop felt in left shoulder. EE continued to work until 7/4/2023.

Employee was putting grease on jack eye when jack slipped and pinched EE's finger in between jack and pin base.

Employee states was pushing bolt to top when rotation hose got caught under motor at worm gear and pulled hose loose from fitting causing hose to strike operators hand resulting in 2 fractures to middle finger.

Employee was holding steel in hand while bolting. Came down with the drill pot and pushed the steel into top of boot causing steel to puncture left foot. Resulted in fracture and stitches.

Employee stated that they were starting on first bolt when a piece of draw rock fell striking their right hand pushing it down against the drill pot. This resulted in 5 stitches.

Employee was preparing to install roof bolt, when EE turned to install it the roof bolt pan struck the canopy post and recoiled and hit EE in the mouth chipping a tooth.

Contractor was bolting #2 heading and got steel hung up and ran head up to get steel and pinched middle finger on chuck. This resulted in a laceration requiring stitches.

Employee was bolting in the #4 heading on the operator side of FB73 on the 3rd row on the outside bolt, had drill hole and push bolt top had drill steel in left hand and drop drill head and it caught drill pot and pushed drill steel into left thigh. This resulted in a bruise. Employee continued to work until 4/21/2022 when doctor took employee off work.

Employee was drilling the 2nd bolt in 2nd row of cut in #2 entry. EE finished drilling hole and brought steel down and it wouldn't separate at coupling. EE held starter in left hand and flipped pusher with right hand. The steel separated and pusher struck EE in the forehead above left eyebrow. This resulted in a laceration and 6 sutures.

Employee went to pin curtain back up when an undetected rock fell striking them. The rock pushed head down chin against chest and pushed them back.

Employee stated that EE was installing bolt. EE was going to straighten the bolt to put back in drill pot when the bolt rolled sideways and EE felt pain in EE's left shoulder. EE continued to work up until EE had surgery on 1/6/2022.

EE stated EE was spotting a roof bolt. The EE placed the new bolt in the hole. EE was holding the new bolt, plate and pan against the bottom of the damaged plate and pan to attempt to push them both to the roof with the new bolt. When the drill chuck contacted the new bolt, The EE's hand was pinched resulting in a laceration to EE's right hand and 4 sutures.

Employee states: was bringing steel out of hole when employee got down to the last two pieces employee pushed up and jerked down when the steel came out and caught employee's right pinky finger between two pieces of steel. This resulted in a fracture and sutures.

Employee was installing rib bolt and had left hand resting on the bottom of the outer mast. EE dropped the mast down and left middle finger got pinched between the inner and outer mast at the bottom. This resulted in a fracture and sutures.

Employee was putting last piece of steel together and pushed up on the chuck. The steel became fouled against lip on chuck causing it to bow, dislodge and strike them on the web of left hand resulting in a fracture.

Technician was using a 30 ton hydraulic ram to break the inside tire bead on the RRI of a 785 rigid frame haul truck. When the pressure was released from the ram it fell to the ground then bouncing crushing technician's thumb on left hand against the hydraulic pump.

Employee thought that had lost suction to the drill head and EE placed hand on the drill chuck to check for suction. When EE placed hand over the chuck, EE's drill steel that was left hanging in the hole broke free and fell hitting EE's finger against the drill chuck. This resulted in a displaced fracture to EE's left ring finger.

Employee was standing on EE's platform of the drill, swung out to put up EE's middle bolt. The platform hit a rock and pushed EE up into EE's canopy, causing pain in EE's upper back.

Employee stated as EE was drilling EE's hole, EE was throwing out EE's bolt and caught EE's finger between the bolt plate and the push plate on the side of the drill head. This resulted in a fracture and stitches to EE's right index finger.

Employee was swinging out to install first bolt in the third row of 4 right. EE stated: had hand on the handle of the canopy and as EE swung out EE pinched the left index finger between the handle and rock that was protruding from the rib.

Employee was operating right side of face bolter #24. EE had head rolled over to install rib bolt. Employee then rested left hand on the drill while EE retrieved steel from hole sliding the drill mast back pinching the left middle finger. This resulted in a fracture and 5 stitches.

Employee was in the 4 left break loading out shot up rock and trimming rib line when a piece of unknown debris was thrown from the cutter head of the miner striking EE in the face causing a laceration below the left side lower lip requiring stitches.

Contractor was removing steel from the coal rib. While EE was bringing the steel out, EE pulled the mast back towards EE to reconnect the steel and felt sharp pain in the middle finger on the left hand. The bottom of the mast hit EE's finger lacerating it against the back of the frame of the slide. This resulted in 10 stitches to close the laceration.

Employee was in the process of squaring up a rock brow in order to drill for a shot after the coal had been undercut. As EE started to shear down in the rock, a piece of metal (carbon) projected back down the entry towards EE. It struck EE approx. 2" above the left eye above EE's eyebrow resulting in a laceration requiring 3 stitches.

Employee was retrieving steel out of a cable bolt hole when the steel hung up in the hole. While trying to retrieve the hung steel from the hole, the employee came in contact with the drill steel connection and became entangled in the steel while it was rotating which resulted in the amputation of left arm below the elbow.

Employee states ee was backing out of 4 heading when drill cable quit taking up. EE states ee turned reel off and reached to grab cable to throw on back bumper. The cable tightened catching fingers between bumper and the cable. This caused an avulsion to the end of ring finger on right hand.

Employee was drilling a hole for a rope bolt. EE was on third piece of steel and was going to change bit and pulled out steel. The finisher piece got stuck and when ee reached back around, the finisher fell out striking left hand on middle finger. This resulted in a fracture and 4 stitches.

Operator backed dozer up, it jarred on pit floor causing the seat belt to tighten up on lower belly. This resulted in a bruised abdomen.

P&P Construction was installing inside cable bolt. Drill steel extension & finisher were stuck together when withdrawing drill steel from cable bolt hole. Operator proceeded to flip extension to break it loose from finisher. Didn't realize it had already come apart, ee pulled the extension toward ee and struck self in the mouth, chipping two front teeth.

Field technician was cutting a bushing from a 993K frame using a torch. While cutting, a piece of hot metal dropped onto the test port hose for the brake accumulator. The hot metal burned the hose which discharged the oil. The oil sprayed onto the cutting torch which immediately flamed up, causing a burn to the side of the technician's left face and ear
