EE was cutting a steel manifold from the side of 10SC001. While getting to the bottom of cutting the brackets off, the manifold broke loose unexpectedly and dropped 3 feet to the catwalk. The EE's foot got tangled in the manifold's valves and dragged EE down the catwalk. This resulted in a twisting of the EE's left ankle.

EE reports hydraulic ladder pinched right leg between the handrail and the folding ladder. EE was assisting a third-party mechanic while the mechanic was troubleshooting the hydraulic ladder. Contacted WorkCare and after a few days of self-care, the EE was scheduled for a CV. EE was diagnosed with a torn ligament in right calf area.
The employee was heating a bolt for extraction when a piece of paint flaked off and entered employee's eye. At the time, employee was looking over the top of employee's prescription safety glasses, which were tinted due to their transition lens feature. The piece was removed with a swab and antibiotic drops were prescribed.
Employee was cutting and grinding metal with a torch and grinder and began to feel irritation in their eye. After employee flushed out with eye wash, irritation continued and resulted in a visit to the doctor. Nothing was removed from the employee's eye but EE was given a prescription. The employee was wearing safety glasses and a face shield while conducting the work.
Employee was cutting a pipe out of an old tank, once the final cut was made the pipe, which was under pressure gave way and struck the employee in the face resulting in a small hairline fracture laceration to the employees nose resulting in 3-4 stitches.
