Employee was cutting a hole in a steel plate sitting on top of a 4' pipe under awning of maintenance shop. EE stopped momentarily to answer EE's phone, then proceeded to attempt to re-ignite Oxygen/Acetylene torch using heat from plate. Gas that had built up in 4' Pipe then ignited causing a loud pop. Employee was not wearing hearing protection at the time of the incident.

EE was bagging on BGI when the air line for the pinch tube began to leak air. EE turned off the air line and leaned over to check the air hose & attempt to repair. The employee put EE's left hand near the pinch tube clamp and EE's right hand on the spout which activated the pinch tube clamp which closed on EE's left thumb

EE was welding a piece of grating in on the 73 turbine platform which required EE to climb into a tight spot below the grating. When pulling self back up EE heard EE's shoulder pop.

The clean out screw auger door fell open on the 66" mill dust collector. EE went to close it while the screw was still running. EE's finger got in the screw, resulting in the tip of EE's finger being cut off.

MAINTENANCE OPERATOR NOT FOLLOWING WORK RESTRICTIONS GIVEN BY PHYSICIAN AND SUPERVISOR. MAINTENANCE OPERATOR WAS ASSISTING WITH JOB ON VALVE FOR MILL 2 FEEDER. RIGHT HAND PUNCTURED THROUGH GLOVE WITH A BESSY CLAMP OPERATED BY ANOTHER MAINTENANCE OPERATOR USING THE CLAMP WITH AN IMPACT WRENCH.
