Employee was removing handrailing from deconstructed conveyor. Employee was cutting support bolts from the top of the handrail using a torch. When the second bolt was cut, the handrail had stored energy and popped free. The handrail then came in contact with employees forearm, causing a laceration.

EE was washing the mast on the drill. He was walking along the mast washing it when he slipped. His legs slipped out from underneath him and as he fell, he sprayed himself with the power washer, which was equipped with a stripping nozzle. Took a chunk of meat out of his leg.
The starter had locked up on an air compressor. Employee grabbed the V-belt to move motor. The engine kicked back and caught his right hand in alternative sheave.
EE was grinding a piece of 1-inch angle, when his hand was pulled into wheel on bench grinder. He suffered a left hand laceration.

Another employees was using a backhoe to put a liner in a crusher bowl at the plant. The backhoe shifted and the outrigger landed on employee's foot. His right foot suffered a contusion.
EE WAS WORKING WITH OTHER EMPLOYEES TO CUT THE BEARING OFF THE SHAFT ON THE #2 CONVEYOR. THEY HAD HUNG A TORCH ON THE I-BEAM OVERHEAD FOR LIGHT. WHILE THEY WERE PUTTING A NEW BEARING ON, THE TORCH FELL AND STRUCK INJURED, KNOCKING OFF HIS HARD HAT AND SAFETY GLASSES. THE TORCH STRUCK HIS RIGHT EYE.
Employee was welding on a loader bucket. A chain hoist was attached to the bucket. The nylon strap broke and the chain host swung down and struck employee's left hip. His hip was bruised. Injured is losing time.
EE WAS CUTTING A PIECE OF 2"AIR HOSE WITH A DIE GRINDER AND A CUTOFF WHEEL. WHILE CUTTING THE HOSE THE DIE GRINDER SLIPPED OFF OF THE HOSE WITH THE CUTOFF WHEEL CUTTING HIS LEFT HAND. CUT REQUIRED STITCHES.
Employee was cutting a bolt on a belt puller. A hot piece of metal fell on his pant leg, burned through his pants, fell inside his boot and burned his right foot. Employee is off work beginning 4/25/04.
Crew was drilling in location where they couldn't quite level the drill. So when making steel connections the drillers helper was having to pull on the drill steel to get it to line up so they could make up. While he was pulling on the steel the driller lowered the steel to make the connection and at that time ee felt something pop in his right elbow.
Two employees were in a manlift at the construction site. The lift was inadvertently raised into a beam. One employee was caught between the manbasket and a metal frame. His chin was cut and a vertabra in his neck/back was injured. He is losing time.

EE HAD HIS PAYCHECK SO HE CLIMBED OVER THE HANDRAIL INTO THE HAUL TRUCK'S BED TO HAND THE DRIVER HIS CHEC K THROUGH THE WINDOW. THE DRIVER NEVER SAW HIM & STARTED TO DRIVE OFF. HE JUMPED UP & GRABBED A WATER PIPE, LEAVING HIM HANGING 14' OFF THE GROUND. HE FELL & SPRAINED BOTH ANKLES.
EE WAS SPLICING A CONVEYOR BELT. WHILE TAKING THE METAL CLAMPS OFF, THE IMPACT WRENCH STRUCK EE IN THE CHIN. NO LOST OR RESTRICTED DUTY BUT STITCHES WERE REQUIRED.
EE WAS USING A TORCH TO DRY SAMPLES. HE TURNED AND FELT A SHARP PAIN BETWEEN HIS SHOULDERS.
EE SUFFERED A WELDING BURN TO HIS RIGHT EYE.
EE WAS DRILLING 2 PIECES OF METAL WHEN THE DRILL BIT BROKE. HE FELL FORWARD AND STURCK HIS HAND ON THE BIT.

EE WAS DRILLING A HOLE. WHEN DONE, WENT TO PICK UP THE ROTARY HEAD WHICH CAME DOWN ON HIS BACK. COWORKER CAME TO HIS RESCUE AND RELEASED HIM FROM HIS TRAPPED POSITION. HE WAS TAKEN TO THE HO SPITAL WHERE IT WAS DETERMINED THAT HE HAD A SEVERE TO HIS BACK AND HIP.
