Employee was working on cutting and grinding metal for the fines chute on the LT330GP cone plant, EE at noon time felt something in EE's left eye. Went to urgent care where EE's left eye was flushed and drops added. They said EE's eye had a scratch/abrasion.

Employee was using cutting torch and grinder to repair a rock crusher main frame liner, when a piece of metal went up under the face shield and safety glasses into EE's eye.

The miner was grinding a steel plate when the grinding wheel broke apart. The piece that came off cut their right leg requiring going to the emergency room for stitches.

The miner was grinding a steel plate and a small metal chip got into EE's eye. EE was wearing eye protection.

Mechanic had just completed brake job on company 3/4 ton GM van. He had removed the jack stands from the rear axle. He had the hydraulic jack under the center of the axle. He was lying on a creeper and turned the handle to release the jack. The jack came down fast and the handle smashed his wrist onto the shop floor.

EE was performing repair work on an excavator bucket. The grinder he was operating kicked back and cut his wrist.

While working with an air tool to tighten a bolt the employee cut his hand.

While performing maintenance work on a jaw crusher, the employee activated a 1/2" drive air gun which blew dust into his face and eye. The employee was wearing safety glasses at the time.

While using a weed whacker employee, who was wearing safety glasses, got a piece of metal in his eye. Abrasion to the eye.

Employee was fixing a belt splice and his finger got struck by the grinding wheel at the knuckle. His knuckle was cut and he required one stitch.

Removing bolt using impact gun. Finger crushed between truck and gun.

Working with wrench on worn out bolt. Bolt had sharp edges. Spun around and cut finger.

EE WAS STRADDLING OVER A PULL CABLE, WHEN THE SLACK WAS TAKEN UP ON CABLE. THE EE WAS LIFTED AND FELL TO GROUND.

WORKING IN JAW FEEDER WHEN HE WAS SITTING DOWN - THE GRINDER KICKED DOWN AT HIS LEG.

WORKING ON PORTABLE PALNT. HIT HEAD UNDER PLANT. CUT HIS EAR REQUIRING 10 STITCHES. EE WAS WEARING A HARDHAT.

OBSERVING WELDER WORKING ON IRON BAR-DID NOT HAVE SAFETY GLASSES ON FLASH BURN OF EYES.

AFTER WELDING SCREEN, LEANED OVER TO GET NEW ELECTRODES, HAND SLIPPED ONTO HOT WELD.
