Employee was cutting with a torch. Hose was caught and when they pulled it free, the torch went into their glove burning the left hand.

Employee was patching a pipe when the trackhoe bucket swung around crushing EE's left arm between the bucket and pipe.

Employee was cutting a handrail off a barge when it fell and struck him on the back.

Employee went to start the outboard motor and it was in gear. To prevent cranking the motor in gear, the rope will not come out when the motor is in gear. He pulled hard on the rope and pulled a muscle in his lower back because the rope did not come out.

Employee was putting a pipeline back together when the pipe smashed his left knee against the hydraulic ram on the work barge.

When finishing up a weld, the shield was raised and the rod left over would strike the metal sometimes causing an arc exposing the eyes to it. The employee burned his eyes.

EMPLOYEE WAS UNSTOPPING A SAND LINE AND A RUBBER HOSE HIT HIM ON THE LEFT LEG. HIS UPPER LEG WAS BROKEN.

Employee's left shoulder began burning and hurting while operating the dredge.

Employee was assisting in dumping 10" pieces of plastic pipe. The trackhoe was moving a piece of the pipe when the chain came unhooked and the pipe fell on him hitting him on the left side of the head and on his left shoulder.

Employee was welding on a stacker. When he stood up, he cut his head by hitting it on the stacker.

A BACKHOE WAS PULLING SLACK IN THE TAIL LINE OF A DREDGE. THE ROPE SLIPPED AND POPPED THE EE ON TOP OF HIS LEFT SHOULDER.

EMPLOYEE WAS WORKING WITH DRILL RIG OBTAINING SAND AND GRAVEL SAMPLES. SAND OR RUST BLEW OFF PIPE INTO HIS RIGHT EYE.

EMPLOYEE WAS WELDING INSIDE LOG WASHER. HE BURNED HIS EYES.

EE WERE PUTTING A PIPELINE BACK TOGETHER, LIFTING THE PIPE AT AN ANGLE. THE HOIST FOOK THAT WAS HOOKED INTO THE CHAIN WELDED ON THE PIPE CAME LOOSE AND STRUCK HIM IN THE LEFT SIDE OF THE FACE.

EE WAS WELDING ON GRAVEL PLANT. HE RAISED HIS SHIELD TO LOOK AT THE WELD AND A PIECE OF SLAG POPPED OFF AND INTO HIS RIGHT EYE.

EMPLOYEE WAS WELDING ON THE CUTTER SYSTEM FOR A DREDGE AND BURNED HIS EYES.

EE WAS ASSISTING ANOTHER EE WITH SAND BLASTING. EE WAS MOVING THE HOSE WHEN THE CONNECTION BETWEEN THE HOSE & THE SAND POT BROKE LOOSE. EE'S RIGHT HAND WAS SAND BLASTED.

EE WAS WELDING INSIDE PONTOONS. SMOKE AND/OR FUMES MADE HIM ILL. SYMPTOMS INCLUDED DIFFICULTY BREATHING, SORE THROAT, COUGHING, ETC.

WELDING ON A PIECE OF PIPE-HE STOPPED TO CHIP OFF SLAG FROM THE WELD WITH A HAMMER. A PIECE OF HOT SLAG WENT DOWN IN HIS BOOT AND BURNED HIS ANKLE.

EMP WAS CHANGING OUT A 16" PIPE. PIPE WAS INSERTED IN ONE SLEEVE. HE WAS MOVING PIPE FORWARD TOWARD NEXT SLEEVE USING A BACKHOE TO SWING PIPE. PIPE BUMPED HIM ON LEFT KNEE AND SHIN AREA. PIPE SWUNG AROUND PINNING HIM BETWEEN PIPE AND TRACK ON BACKHOE.

BUILDING A COUNTERWEIGHT TO CHECK SCALES. WAS WELDING A LID ON TOP. LID DID NOT FIT FLUSH WITH EDGE OF BOX. EE WAS HOLDING LID, KIND OF SQUATTING, BALANCING WITH LEFT HAND & WELDING WITH RIGHT HAND. HE WAS TWISTING AS HE WELDED ALONG OPENING & HE TWISTED HIS LEFT SHOULDER & PULLED MUSCLE.

EMP WAS REPINNING A SPUD ON A DREDGE. ACCIDENTALLY STUCK HIS FINGER IN THE SPUD CYLINDER HOLDER.CYLINDER MASHED HIS RIGHT INDEX FINGER CAUSING AFRACTURE TO THE FINGER.

EE WAS TYPING UP A PADDLE WHEEL DREDGE USING A CABLE WITH A WINCH. HE APPARENTLY DID NOT HOOK THE CABLE SECURELY IT CAME LOOSE AN DHIT HIM IN THE SIDE OF THE FACE. HE REFUSED TO GO TO DR. WORK ED ALL DAY WITHOUT COMPLAINT. CALLED IN 8-18-95.AND WENT TO DR.
