Employee had a gloved hand on a lift chain when employee gave the command to raise the load. Employee's glove was pinched in the chain pulling employee's finger tip in the pulley before employee pulled hand out of the glove. Resulting in a fractured finger tip.

Employee was sitting with body at a compound angle while shooting clinker in the preheater. Employee finished the task and stood up when employee felt a sharp pain in lower back.

The dozer operator was moving material on the yellow layer when the operator backed off a highwall approximately 20 feet tall.

An excavator operator was swinging to load haul truck when they struck the sideboard of the haul truck they were loading jarring the truck. The operator of the haul truck later reported a pain/discomfort in their back and sought treatment for back spasms on 9/18/2018 resulting in them being prescribed muscle relaxers.

I was cutting a "C" channel while in a man basket. At the end of the cut the material folded and caused the grinder wheel to backlash and cut my finger. My helper witnessed the cut. I saw the laceration and knew I needed stitches. I drove myself to the emergency room and received three stitches on my right hand index finger.

EE reported lower back/leg pain on 6/6/16 after operating dozer to remove overburden. EE originally declined to see a physician to see if condition improved. 6/24/16 EE was seen by phys. and placed on restricted duty pending follow-up. EE is on temp. leave as no work is available to accommodate medical restrictions.

EE was chipping out the Ducon Scrubber above the hydrator unit using an air chisel. A piece lime or scale entered his left eye behind his safety glasses and full faced mask he was wearing. EE was seen by a physician the following day who removed the particle and prescribed medications to treated corneal abrasion. No other restrictions were put into place.

EE was loading material bags manually onto the bagging machine. As he was putting a new bag onto the spout adjacent to a bag being filled, the bag being filled ruptured sending lime into his face and eyes. EE was seen by physician and provided Rx for medicated eye drops. Follow-up scheduled for 9/12/2014.

EE was injured while retrieving a part no. from the steam fan sheave wheel removed the guard as the fan was winding down - he inadvertently contacted the belt which drew his right hand into the sheave wheel and cut the tip of his thumb off. EE was treated by a physician who sutured the wound and placed him on restricted duty. Follow-up Appt. scheduled in 10 days.

EE was operating a drill press to drill a piece of steel plate, the drill bit seized causing the plate to spin which struck the EE on the left ring finger. His finger was lacerated requiring sutures - follow-up appt. scheduled for 5/2/14 for re-check.

EE was performing maintenance on a waste tank @ Hydrate Plant with an air wand to remove material.Lime blew back and entered his right eye around his safety glasses. EE was not wearing company provided and mandated secondary eye protection(goggles and/or face shields)while performing this task.EE received samples of eye medication for treatment making this a reportable injury

EE was loading a customer truck when the chute's actuator froze in position. Material backed up into the chute and was forced out of the sample port - lime fines blew around his safety glasses and into his eyes. Attending physician prescribed medication for eyes - follow-up visit on 12/17/13 cleared EE to return to full duty.

Miner was cutting plastic sheeting on a table saw and using a piece of lumber to push and guide sheeting through the saw. The board the Miner was using slipped which caused his hand to contact the moving saw blade. Miner received lacerations to his middle and index fingers.

While lifting the motor guard on HP400 cone with a crane, the picking bolt pulled out of the threads and the guard came down from the crane hook. As the guard came down, the corner of the guard hit ee in the face on the left cheek. EE was taken to the hospital where he received five stitches.

Our employee was using a circular saw to cut dry wood. The saw kicked back and the blade caught his coveralls causing the blade to make a cut into the fatty tissue of his abdominal area. He was treated at the hospital and released the same day. He was not given any work restrictions or time off duty by the attending physician.

Employee was on conveyor cutting belt with a chop saw when he slipped and the chop saw fell on his left hand. Employee received surgery to repair tendons in hand.

After a long period of shoveling on the "Big Unit" employee noticed a pain in his lower back which eventually spread to his groin area. Employee went to doctor on 5/4/2010 where he was placed on restricted duty due to a back sprain. Employee was put on restricted duty until next doctor visit.

Angle iron 3" x 3" x 1/4"x 3 ft. long slipped out of chocker and hit worker on right shoulder causing a bruised shoulder.

Employee sustained a whiplash type injury while driving a backhoe. He drove through a trench in the haul road that caused the machine to buck up and threw his head back and forth.

Employee was using a plasma cutter to cut a piece of plate steel. He was holding a straight edge with his left hand as a guide. As he passed by his left hand a piece of slag entered his left glove at the finger seam, burning his left middle finger at the tip. The gloves were in good repair and properly used.

EE was operating the dozer when it started bouncing over the rocks

Flash burns of the eyes after working around welding during shift.

cutting with the air arc when a piece of slag blew back and went into his ear. No treatment at time of injury. Went to doctor on 9/23/06. Treatment was ear drops and antibiotic, hole burnt in left eardrum, no lost time or modified duty.

Foreign object entered eye while welding/grinding.

Using jackhammer on refractory in vertical kiln.

EE was blowing dirt off clothes with the nozzle pointed up, when a sliver of metal blew up under safety glasses. He did not notify employer until 6/13/05. Went to doctor on 6/12/05.

EE WAS WELDING AND GRINDING INSIDE CRUSHER WHEN A SMALL PIECE OF METAL GOT IN HIS RIGHT EYE.

WAS LINING UP BOLTS TO LEVEL AUGER WHEN A FAN WAS TURNED ON AND DUST BLOW AROUND AND OBJECT WENTINTO LEFT EYE.

THE INJURY OCCURED WHEN THE EE S HAND WAS CRUSHED BETWEEN SCREEEN RAIL AND THE SCREEN ITSELF WHEN ANOTHER EE USING A IMPACT WRENCH TIGHTENED THE SCREEN INTO POSITION. THE FINGERS WERE NOT SEVE RED AND THE INJURY IN NO BROKEN BONES, ONLY SEVEERED BLOOD BLISTERS FROM THE PINCHING.

ONE EMPLOYEE WAS STEAM CLEANING WHILE ANOTHER WAS WORKING ON THE CAT 972 LOADER, UNAWARE WHEN THE FIRST EMPLOYEE TURNED AROUND. HE WAS HIT IN THE RIGHT SIDE OF HIS CHEST WITH THE HIGH PRESSURE WATER. THIS FORCED WATER UNDER THE SKIN.

EMPLOYEE WAS WELDING IN THE TUNNEL SECTIONS WHEN ONE OF THE PIECES CAME LOOSE AND STRUCK HIM IN UPPER LIP.

MYSELF AND ANOTHER EMPLOYEE WERE CUTTING STEEL UPON THE HILL WHEN MY RIGHT GLOVE & HAND CAUGHT ON FIRE BURNING MY RIGHT HAND. OTHER THAN THE BURN AND INFECTION I DON'T KNOW OF ANY IMPAIRMENT.

ON NOV. 28TH, EE WAS AT WORK USING A GRINDER ON IRON RAILINGS HE WAS BUILDING. THAT EVENING WHENHE SHOWERED A METAL FLAKE FROM THE DAY PRIOR WORKED ITS WAY INTO HIS L. EYE, CAUSING HIM TO REPORT THE IRRITATION AND TENDING TO IN ON 11/30

HELPED ANOTHER EE SET AND SECURE JIB ON BOOM. ILL FITTING SIDE MOUNT BRACKET FOR JIB. OTHER EE SAID PULLING HINGE PINS WOULD LOOSEN IT UP ENOUGHTO PULL JIB INTO MOUNT BRACKET, THEN WE LOOSENED MOUNT BRACKET SPACER BOLT, PULLED MOUNT BRACKET RETAINING PIN TO GET PLAY SPACER, SHOOK THE JIB MANUALLY AND THEN IT FELL. I RODE IT DOWN.

EMPLOYEE WAS CUTTING AND DRIVING OUT OLD BOLTS OFF STRUCTURE. NEXT DAY EYE WAS SORE AND EMPLOYEE WENT TO RAPID CARE AND THEN SENT HIM TO EYE CLINIC WHERE THEY REMOVED A LITTLE PIECE OF SLAG OU T OF THE EYE.

EMPLOYEE WAS CHECKING PISTON ON PAINT GUN WHEN PISTON DROPPED ON HIS RIGHT INDEX FINGER.

EE WAS WORKING ON THE ROOF OF A WAREHOUSE AT THE LIME PLANT. HE STARTED TO SLIDE OFF OF THE ROOF AND REACHED UP TO CATCH HIMSELF WITH ONE HAND. THE CUTTING TORCH SLIPPED OUT OF HIS OTHER HAND AND FELL ON HIS BOOT--RESULTING IN A BURN TO HIS RIGHT ANKLE.

EE WAS DRILLING 15/16" HOLES IN A PIECE OF PLATE. SHE WAS USING A DRILL PRESS WITH A QUICK RELEASE PLATE CLAMP. THE DRILL GOT CAUGHT IN THE HOLE AND PULLED THE PLATE OUT OF THE CLAMP SMASHING HER LEFT RING FINGER BETWEEN THE DRILL BASE AND THE PIECE OF PLATE.

EE HAD THE DOOR OF THE DRILL OPEN WHILE TRAMMINGTHE MACHINE OUT TO THE QUARRY. THE DRILL ROLLED OVER A LEDGE WHICH MADE THE DOOR SLAM SHUT EE'S FINGERS WERE IN THE DOOR SEAL WHEN THE DOOR SHUT

EE WAS ATTEMPTING TO FREE JAMMED AUGER. WHILE REACHING FOR A SHOVEL HE PUT HIS HAND ON A V-BELT DRIVE. THE DRIVE WAS STARTED REMOTELY AND HIS FINGER GOT CAUGHT IN THE DRIVE.

THE DRILL STEEL WAS STUCK. EE WAS NOT ABLE TO REMOVE THE STEEL WITH THE DRILLING EQUIPMENT, SO HE MANUALLY TRIED TO REMOVE THE STEEL. HE WAS DIRECTING ANOTHER PERSON TO MOVE THE ROLLERS INWARD AND HIS THUMB WAS PINCHED IN BETWEEN THE ROLLER EQUIPMENT AND THE STEEL.

EMPPLOYEE WAS WASHING HIS TRUCK WITH THE STEAM-WASHER AND ACCIDENTLY POINTED IT AT HIS FOOT. HOT WATER ENTERED THROUGHA SEAM IN THE BOOT AND CAUSED 2ND AND 3RD DEGREE BURNS ON THE TWO OUTER TOES OF HIS LEFT FOOT.

WELDING WITH A WIRE FEED WELDER ON KILN TIRE (TOP) SPLATTERING SLAG GOT INSIDE HELMET & SAFETY GLASSES.

EMPLOYEE WAS OPERATING JACK HAMMER IN THE KILN TO CLEAN OUT CLINKERS. A LARGE PIECE CAME LOOSE AND FELL ON EMPLOYEE'S LEFT WRIST. RECEIVED SUTURES AND SPLINT.

EE WAS UNPLUGGING THE DRYER DUST COLLECTOR. WHEN IT WAS EMPTIED OUT, EE LOOKED UP IN THE BAGHOUSE AND THE PUFFERS BLEW DUST IN BOTH EYES.

WELDING ON PIN & THEN FILING STEEL OFF PIN - STEEL FILING GOT INTO EYE.

EE WAS PREPARING A SCREEN DECK FOR 9" SIDE GRINDER. THE GRINDER DISK MADE CONTACT IN A 90 DEGREECORNER & SKIPPED FROM THE MATERIAL'S SURFACE CAUSING THE EE'S WRIST TO BE TWISTED. THE GRINDER K DISK CAME IN CONTACT WITH EE'S WRIST. ABRASION REQUIRED SUTURES.

CO-WORKER WAS SHOWING INJURED HOW TO BAG. INJURED HAD HIS LEFT HAND RESTING ON THE BAGGER SET. WHEN CO-WORKER PUT A BAG ON & PUSHED THE START BUTTON. INJURED'S LEFT THUMB & TOP OF THE LEFT HAN D WAS JAMMED BETWEEN THE BAGGER SEAT & THE CYLINDER BRACKET.

PREPARING TO PATCH HOLE IN AUGER HOUSING, REMOVING TAPE ON AUGER HOUSING TO PATCH HOLE, TAPE CAUGHT ON AUGER & PULEED IN LEFT THUMB.

EE WAS HOLDING A FLATHEAD BOLT WHILE NUT WAS BEING REMOVED. BOLT SPUN CUTTING HIS FINGER.
