Plant operator was using a hand held grinder to cut the bearing housing off the tail pulley shaft. The grinder kicked back and came into contact with the employees left foreman. The employee suffered a laceration that required 16 sutures to close.

Driller was adding addition to auger. When the auger stop rotation the helper reached for the pin as the driller reversed rotation and the head of the bolt pinched his thumb against the outer portion of the yoke. Helper had a fracture to right thumb.

EE was holding pressure on a cutting edge bolt, pushing bolt into place for removal with air wrench using the palm of his hand. The bolt started turning and cut a circular pattern in right palm

Employee was operating a piece of equipment (dozer) when he came to uneven ground which resulted in the jarring of his back.

Employee was using a pneumatic impact wrench to tighten a bolt. He used his left shoulder to lean against the wrench for more leverage. This resulted in pain to his shoulder and chest. After seeing a medical professional this injury was not MSHA recordable; however the employee wished to get a second opinion at which time it became reportable.

Employee was using pressure washer to clean equipment when he decided to use on his boot. The wand slipped and went in his boot resulting in a burn to left foot

Employee burned his eyes due to a welding flash.

Employee was assisting in the maintenance to shaker when the crane malfunctioned when lowering shaker, pinning employee's left hand between the spring and mounting plate.

Employee alleges pain in left arm from shoulder to wrist due to repetitive operation of controls on dozer.

EE was using air ratchet tool, attempting to remove bolt from VSI table on crusher when he hit his hand against table.

EE was operating the dredge when he stood up to close the door, at which time the cutter head hit a log causing the dredge to bounce, slamming the door against employee's right hand.

(3 EMPLOYEES WERE INJURED IN THIS ACCIDENT.) EE WAS ASSISTING WITH INSTALLATION OF A REPLACEMENT DECK ON SHAKER WHEN THE DECK FRAME SUDDENLY SLID INTO PLACE, STRIKING EE'S RIGHT FOREARM (SURGICAL REPAIR REQUIRED).

(3 EMPLOYEES WERE INJURED IN THIS ACCIDENT.) EE WAS ASSISTING WITH INSTALLATION OF A REPLACEMENT DECK ON A SHAKER WHEN THE DECK FRAME SUDDENLY SLID INTO PLACE, STRIKING EE IN THE PELVIC AREA (SURGICAL REPAIR REQUIRED).

EE was assisting co-worker with plant maintenance inside primary shaker when the flash from welding being conducted by co-worker caused burns to his eyes. Given prescription for pain.

EE WAS ASSISTING IN WELDING THE PRIMARY SHAKER WHEN HE SUSTAINED WELDING BURNS OF THE EYES.

THE EE WAS OPERATING THE SURGE LOADER AND WHILE HE WAS BACKING UP HE NOTICED THAT HE WAS ABOUT OTO HIT THE HATCH ON THE EXCAPE TUNNEL. HE TURNED HIS HEAD FAST, AND LATER HIS NECK AND SHOULDER BEGAN TO HURT.

OVER A PERIOD OF TIME, DUE TO OPERATING THE DRAGLINE, EE HAS DEVELOPED STRAIN TO BOTH KNEES.
