Employee was working on haul truck and received burns

IE was welding in a haul truck bed installing new bed liners. EE sustained corneal flash burn because the digital welding shield's batteries died, which went unnoticed until the next morning when EE woke up in pain. EE's wife took EE to the emergency room, where EE was prescribed Cortisporin ophthalmic drops and released after receiving medical treatment beyond first aid.

A miner was looking down inside the Jaw to guide the operator of the rock hammer down onto the rock that was stuck. The rock breaker slipped off of the rock causing it to strike the right hand of the miner in between the back of the jaw crusher and the hammer while guiding the hammer into place. A severe hand injury occurred resulting in the loss of 3 fingers.

A miner got a piece of slag/metal stuck in left eye and it had to be removed by the eye doctor. 0n 4/8/2023. The miner was wearing a proper welding helmet and safety glasses while welding during the day on 4/7/2023.

A miner was attempting to light the natural gas burner at the Fine Grind Plant when the burner lit with higher intensity than in the past which EE was not expecting. That caused burns on right arm and right hand. EE was transported to the hospital with non life threatening burn injuries. EE was wearing a hooded sweatshirt and gloves at the time of the accident/injury.

Welding overhead in a chute, a piece of weld slag burned through EE's pants & into boot, causing burn to foot. Initial care was first aid, but EE was later restricted from work.

EE was installing water pipe 18" to get water to the canal by the plant. While bolting the flanges together ee right middle finger got pinched between the two flanges that connects the two pipes together. Fracture of the right middle finger tip.

Drilling holes to bolt guards into place, steel piece got into right eye, employee said was wearing safety glasses, no witnesses.

Welding braces to a stand, metal flaked off, getting into left eye.

Employee was pulling a screen down at the secondary plant to position into place and felt his back tighten up. Employee didn't think any thing would come from it. The next morning he couldn't move his neck. He went to a chiropractor and the company clinic. Employee has a Thoracic back sprain. Was off work 1 day and restricted for 2 days.

EE was operating the Hitachi Shovel & digging in tight rocks all day. EE was jarred around while runing the shovel all day & felt pain in his right shoulder. Pain got worse when he sat down to rest at home that evening. He went to work on Tuesday & was sent to the clinic. Diagnosed as cervical strain; EE was put on restricted duties, but no work was available.

Employee was inside the shaker replacing main support beams. Employee was welding, grinding and cutting to replace the beams. Foreign debris got into his left eye. He had his welding shield on. Later that evening his eye started to hurt and the next morning he woke up with a lot of pain and matting in his eye. He received a severe abrasion.

EMPLOYEE WAS WATCHING AUTOMATIC WELDER BUILD UP ROLLS ON CRUSHER. WHILE EE WAS LOOKING AT THE WORK, HE RECEIVED WELDING FLASH BURNS TO BOTH EYES. HE WAS GIVEN SALVE FOR EYES AND PAIN MEDICATIO N PRESCRIPTIONS.

EMPLOYEE WAS WATCHING THE AUTOMATIC WELDER BUILD-UP ROLLS ON CRUSHER. WHILE LOOKING AT THE WORK HE RECEIVED RADIATION BURNS TO BOTH EYES. HE DID NOT SEEK MEDICAL TREATMENT BUT COULD NOT COME T O WORK THE FOLLOWING DAY.

EMPLOYEE WAS HOLDING THE GRINDER IN LEFT HAND AND HOLDING THE ROD IN THE RIGHT. GRINDER SLIPPED AND CONTACTED EMPLOYEE'S RIGHT INDEX FINGER CAUSING A LACERATION REQUIRING STITCHES. EMPLOYEE IS NOT TO PUSH, PULL OR LIFT OVER 15 LBS FOR 4 DAYS.

EMPLOYEE WAS WELDING THE FEED HOPPER. HOT SLAG GOT ON EMPLOYEE AND IT WENT THROUGH HIS COVERALLS & BURNED HIS EYE. HE USED OINTMENT FROM HOME. LATER IT BECAME INFECTED, HE WENT TO DOCTOR ON 2-23-2000.

EE WAS USING A PRESSURE WASHER TO CLEAN THE RADIATOR, THE TIP BECAME CLOGGED & EE PUT HIS FINGER OVER THE DISCHARGE END OF THE HIGH PRESSURE WASHER WAND AND THE FORCE OF THE WATER PENETRATED H IS LEFT INDEX FINGER CAUSING A NAIL AVULSION.

EE WAS DRILLING HOLES IN RUBBER WHEN THE DRILL BIT GOT CAUGHT. THE DRILL BIT STOPPED AND THE DRILL KEPT ON TURNING AND TWISTED EE'S WRIST.

EMPLOYEE WAS HELPING TO PUT A DRILL BIT INTO THE GROUND. THE BIT WOULD NOT GO INTO THE GROUND. THE BIT WAS RETRACTED AND BECAME CAUGHT ON THE TRUCK. THE PRESSURE CAUSED THE BIT TO SWING OUT. A S THE BIT SWUNG OUT, EMPLOYEE WAS HIT ON THE FOREHEAD. EMPLOYEE RECEIVED STITCHES.

EMPLOYEE WAS WORKING IN THE SAME AREA AS A WELDER. AND EMPLOYEE'S EYES WERE BURNED FROM THE WELDING FLASHES.

WHILE EE WAS USING AN IMPACT WRENCH THE SOCKET DISLODGED HITTING EE IN HIS MOUTH. TOOTH WAS CHIPPED AND HIS TONGUE WAS CUT.

EMPLOYEE WAS INSTALLING ENGINE IN HT 110. THE IMPACT WRENCH SLIPPED AND HIT EMPLOYEE IN THE MOUTH CHIPPING HIS FRONT TOOTH.

WHILE OPERATING THE DRAGLINE, EE CAUGHT HIS FINGER IN THE FAN THAT COOLS THE CAB FOR THE OPERATOR. EE HAD REACHED UP TO ADJUST THE FAN.

EMPLOYEE WAS USING A GRINDER. THE GRINDER KICKED BACK AND EMPLOYEES FINGER WAS CUT. EMPLOYEE HAD TO HAVE STITCHES.

EMPLOYEE WAS USING A CUTTING TORCH. HOT METAL FELL INTO EMPLOYEE'S BOOT & BURNED HIS FOOT. IT WAS DETERMINED LATER TO BE 3RD DEGREE BURN.

EMPL CHANGING THE LIP ON A BIG LOADER USING AIR IMPACT WRENCH HOLDING BOLT HEAD. AS THE BOLT TURNED EMPL CUT HAND.
