EE was welding dragline bucket chains. At some point, a small metal fragment entered EE's right eye. In the initial consultation, a doctor removed the foreign object. At a follow-up visit, antibiotic drops were prescribed for an infection that had developed.

While working on the Concrete Metso Hammers, a small piece of metal shaving impacted their left forearm causing a very small laceration.

EE was working on M33 crusher when a single rock that was stuck on M26 conveyor head pulley, fell striking EE on EE's hard hat.

EE was cutting a wear plate from a 4'x8' steel sheet plate with a track torch. As EE reached the end of the final cut the plate became unstable causing it to fall impacting the floor first and then EE's right foot.

When tightening a bolt on a liner with a pneumatic impact gun a piece of microscopic debris went under EE's safety glasses, lodging in the eye.

Using an air impact gun to adjust V-Belts, employee sprained the tendons in his forearm.

Employee was cutting a round bar using his left hand to hold the bar and the right hand to operate the welding torch. The heat from the torch burned his left wrist.

A steel plate which had been put on the rear platform of his mechanic's truck fell onto employee's boot just above the steel toe cap. The torch hose caught the corner of the plate causing it to fall and injure his foot.

A welder was working on the bucket of a dragline. It was a hot day so the welder decided to set up a blower motor aimed at him to cool off. While using the air chipping hammer a small flake of metal was blown under his welding hood and into his right eye by the fan. The employee was taken to workers comp where a doctor removed an embedded metal fragment from his eye.

Employee could not control the water hose when the valve to the pump was opened causing him to fall, injuring his head and right shoulder.

EE first came to us on 3/16/2011 saying he was experiencing back pain. He did state that the pain was caused at work. On May 2, 2011 he saw a doctor who indicated that his operating of a bulldozer was partially responsible for his back pain. The doctor prescribed medication.

Employee was welding and felt something go into his eye.

Drill helper put his hand between kelly bar and the steel tube cap which is used to hold the tube down while the outer kelly bar is being pulled out of the ground. His arm was caught and pulled down iunto the borehole. The drill bar slipped on top of driller's hand. EE had surgery and lost right index finger, other broken fingers and lacerations to forearm.

He burned his forearm when he dropped the cutting torch he was using to cut plates in the plant.

The welder was grinding metal with disk grinder. He was wearing proper PPE consisting of safety glasses & welding hood. A piece of metal entered his right eye. The metal was imbedded in the cornea.

IW received lacerations to left ear and face & head contusion after he was struck by a 20-Ton bottle jack that slipped off while it was being used to raise a screen shaker. Failure to restrain the jack with chains to prevent ejection in case of slip off was the main cause of accident.

IW was on ground ~ 4 feet from basket of manlift. Operator placed manlift in reverse motion, wheels turned right. Boom/basket abruptly swerved left and struck IW on right side of body. Left hand of IW got caught between midrail of basket and conveyor column causing L-hand laceration, left index finger fracture and partially severed L-thumb. Cause: untrained operator for equipment.

EE WAS CUTTING ROUND BAR ON THE HAMMERS WHEN A PIECE OF HOT SLAG FELL ON THE THE TONGUE OF BOOT. THE SLAG BURN THROUGH THE BOOT AND BURNED THE TOP OF EE FOOT

WHILE OPERATING AN AIR IMPACT WRENCH, USING HIS LEFT HAND TO GUIDE THE SOCKET, HE CUT HIS FINGER.

STRAINED BACK WHILE ENGAGING CLUTCH OF THE ROAD ROCK CRUSHER.

AFTER WELDING EE'S EYE BOTHERED HIM

EE WAS CUTTING THE FLANGE ON THE PIPE THE PIPE BROKE LOOSE AND STRUCK HIM ON THE RIGHT LEG.

GREASE PRESSURE HOSE/COUPLING DEVELOPED A LEAK THAT INJECTED GREASE INTO EE'S HAND.

WHILE WELDING ON SHAKER SCREEN A PIECE OF SLAG WENT INTO HIS EAR.

WHILE REPAIRING THE M-54 CONVEYOR HE WAS CUTTING APIECE OF METAL. A SLAG WENT INTO HIS SHOE ADN BURNED HIS FOOT.

WHILE REMOVING BOLTS FROM SCREEN PANEL. THE IMPACT SWIVAL BROKE. CUTTING INJURED LEFT HAND.

EMPLOYEE WAS TARTING UP LIGHT PLANT WHEN SOMETHING CAME OUT OF EXHAUST & WENT INTO EYE.

DRAGLINE WENT INTO LAKE.

WHILE TRIMMING A PIECE OF METAL WITH A TORCH A PIECE F THE METAL FLEW UP BREAKING DIOSDADO'S SAFETY GLASSES AND A SMALL PIECE WENT INTO HIS EYE.

WHILE WELDING IN THE CRUSHER CHAMBER.HE RAISED HIS WELDING HOOD TURNED MY HEAD AND A PIECE OF SLAG POPPED OFF GOING UNDER HIS GLASSES AND ENTERED THE RIGHT CORNER OF HIS RIGHT EYE.

WHILE MOUNTING HIS DRAGLINE TO BEGIN WORK, HE WALKED BY THE RADIATOR. HE BELIEVES THE RADIATOR FAN BLEW PARTICLES INTO HIS EYE.

WHILE WELDING IN THE CRUSHER HE SHIFTED POSITION AND FAN HE HAD BLEW DUST INTO HIS EYE.

USING STEAM CLEANING GUN,A WIRE FROM PROTECTIVE COVER LODGED IN RIGHT INDEX FINGER.REMOVED THE WIRE,DIDNT INFORM ANYONE OR SEEK FIRST AID.THE FINGER BECAME INFECTED AND HE REPORTED THE INCIDEN T ON 8-20-91.

WHILE STANDING IN THE VICINITY OF THE M-14 CRUSHER A ROCK FELL AND HIT EE IN THE FOOT.

WHLE WELDING AND DOING SOME GRINDING ON THE TEREX TRUCK, INJURED THINKS SOME DUST OR METAL GOT INTO HIS RIGHT EYE. IT BOTHERED HIM THROUGH THE NEXT DAY WHEN HE REPORTED.

AFTER FINISHING A WELD ON THE PORTABLE CRUSHERS SKIRTBOARD FRAME, I.NAME HIT HIS HEAD ON THE CATWALK.

INJURED WAS FEEDING THE WIRE INTO THE WIRE FEED MACHINE WHEN HIS FINGER FOLLWED THE WIRE INTO THE GEARS INJURING HIS FINGER.

WHILE I NAME WAS CUTTING CABLE WITH A TORCH A PIECE OF STAG WENT INTO HIS EYE

WHILE WORKING ON A CONVEYOR TAKING GLAZE OFF OF A VULCANIZED HEAD PULLEY, THE GRINDER BOUNDED AND KICKED BACK CUTTING THE SKIN AROUND THE KNUCKLE OF THE FINGER.

EMPL WELDING ON DRILL-GOT FLASH BURNS IN BOTH EYES.EMPL WAS WEARING SAFETY GLASSES.

WHILE WORKING ON CRUSHER I NAME HIT HIS MIDDLE RT FINGER WITH THE AIR IMPACE GUN WHEN HE TURNED IT ON IT SPUN AROUND & INCHED HIS FINGER BETWEEN THE HANDLE & THE FLOOR

WHILE USING A SMALL GRINDER IT SLIPPED AND CUT I.NAMES FINGER THE FINGER REQUIRED 4 STITCHES
