The employee was removing a sample from the pellet press machine when the sample cap contacted the control arm during extraction and was ejected from the machine striking the employee.

Employee was using air lance to dree frozen rock from a chute for several hours. Injured left shoulder due to repetitive work.

Employee was welding and got photokeratitis in both eyes when they lifted their welding hood. Employee received medical treatment beyond first aid.

Employee using hand grinder with cutting disc to trim off steel patch, disc caught on edge of steel and shattered. Piece of disc struck left forearm lodging under skin causing laceration. Stitches required to close wound.

Employee attempted to help another employee cut a board. Just as the employee cutting the board was finishing the cut, the injured employee got his hand too close to the blade of the saw and the blade nicked his finger on his right hand. He received two small cuts which required a total of 3 stitches. Released for regular duty.

Employee was using a torch to cut down some old pipes when one of the pipes fell and knocked him off his step ladder causing a laceration to his left ear.

EE was using pressurized air to clear a pipe when the air hit a restriction in the pipe and dust blew back in his eyes.

EE was using a jackhammer through an access port in #6 rock feeder when a chunk fell hitting the jackhammer bit and forcing the other end of the jackhammer against his head. He suffered a laceration to the left side of his head requiring 6 stitches.

Employee was jackhammering when a piece of hot material fell from above and hit him on the neck and head. He suffered burns to his left hand and neck area as well as a broken right thumb.

EE was using the high pressure water blaster to clean in the preheater tower. He pulled the gun out of one port & went to another. The trigger engaged and the high pressure water was aimed at EE's foot. The water cut through his boot and into his right foot, causing a deep puncture wound.

Employee was welding patches on the walls inside the bottom part of the dust collector when over a period of time his right arm became inflamed.

EE was attempting to see if a feeder was plugged so he had the feeder started. When it started, the edge of the pile gave way and EE slid in with the material.

Employee was using an airlance for an extended time to help the flow of iron ore. The result was a strained left shoulder.

Employee was attempting to use compressed air to bow down some sticky material. He forgot to turn off the air cannons and opened up a 1" hole to put the air lance into. Just as he was inserting the air lance, the air cannon fired and blew hot dust out the hole knocking his face shield and glasses off his head. He got hot dust into both eyes. He has scratches on both eyes.

Ee was pulling on a slack cable and got his hand too close to a pulley. The pulley caught his glove and pulled his fingers in.

WHILE DOING ROUTINE MAINTENANCE HAND BECAME PINCHED BETWEEN CLEVIS AND HEAD OF RETRACTING PNEUMATIC RAM.

EMPLOYEE OPENDED DUST COLLECTOR FAN MOTOR GUARD. WHILE INSPECTING MOTOR CAUGHT RIGHT HAND ON MOTOR FAN AMPUTATING FIRST FINGER AT FIRST PHALANX.

RECEIVED FLASH BURNS TO EYES WHILE ASSISTING A WELDER FABRICATING A HOPPER.

LOST BALANCE WHIEL JACKHAMMERING ONTO CLINKER PILE.

PIECE OF METAL SNAGGED WHILE GRINDING, CUT RIGHT INDEX FINGER.

WHLE CUTTING 2 X 4 BRACES FOR HYDRANT BOXES, LEFT HAND SLID OVER 2 X 4 AND INTO BANDSAW BLADE.

PINCHED FINGERS WHILE USING JACKHAMMER.

EMPLOYEE OXYGEN LANCING BOLTS ON RAW MILL WHEN HOT SLAG BLEW BACK INTO HIS FACE.

WHILE EMPLOYEE WAS WORKING ON COAL MILL SYSTEM, THE BAR HE WAS USING GOT CAUGHT IN COAL SCREW AND HE HIT HIS LEFT INDEX FINGER, RESULTING IN A 3" LACERATION REQUIRING STITCHES.

EMPLOYEE ALLEGES HE GOT SOMETHING IN HIS RIGHT EYE WHILE CLEANING BEAM IN ORDER TO WELD.

COAL DUST/GAS EXPLOSION OCCURRED WHEN COAL MILL REJECTS CLEAN-OUT DOOR OPENED TO CLEAN COAL REJECTS FROM DUCT WORK

WHILE CUTTING SOFTWOOD WEDGES WITH A TABLE SAW EE MADE CONTACT WITH SAW BLADE CAUSING LACERATIONS TO TOP OF INDEX FINGER AND SOME CONTACT WITH MIDDLE FINGER OF LEFT HAND REQUIRED STITCHES.

EMPLOYEE ALLEGES THAT WHILE BURNING SOME GUSSETTS, A PIECE OF SLAG WENT INTO HIS EAR.

EMPL HAS NO IDEA WHEN OR WHERE IT HAPPENED BUT HAD RUST RINGS AND IMBEDDED METAL IN BOTH EYES.

HE WAS DRILLING A HOLE IN CONCRETE SLAB WHEN THE TE-52 DRILL HE WAS USING CAUGHT UP ON A REBAR. IT DIDN'T CLUTCH OUT AND SPUN BREAKING HIS HAND.

EE WAS USING A PELS PRESS TO SHEAR WASHERS. THE PIECE OF WORK SHIFTED AND EE STUCK HIS L 2ND DIGIT IN THE MACHINE AND CAUGHT IT BETWEEN HIS WORK AND MACHINE.

EMPLOYEE PREFORMING BENCH MAINTENANCE ON SILO AERATION PAD. UPON APPLYING COMPRESSED AIR TO AERATION UNIT, CERMIC PLATE RUPTURED. RUPTURED SECTION OF PLATE STRUCK EMPLOYEE IN FACE. FOREHEAD LACERATION REQUIRING SUTURES.
