Employee tripped going up stairs, fell, and wedged right thumb in step grating. The fall resulted in a fractured thumb.

Employee opening cover on railcar and sprained right knee.

Employee was assisting with the installation of a conveyor belt and cut a finger.

Employee was installing a port cover and crushed EE's left thumb between the cover and the riser section.

Employee was working on the F780 clinker/slag conveying system. Employee looked up into the discharge chute of the F780 dust collection system to evaluate the condition of the chute and observe the condition of the dust collector bags. Employee knocked loose cement/slag dust which fell into employee's left eye. The employee was wearing safety glasses.

Employee was working in the No. 1 Finish Mill on a ladder lifting a liner above employee's head. The legs of the ladder were on top of mill balls and as the employee lifted the liner the ladder sank into the mill balls. The employee reacted by pushing harder on the liner and injured right shoulder.

While trying to open #8 feeder in the reclaim tunnel with a metal bar; the bar slipped out of the employees hand and contacted left thumb causing a fracture.

While installing the lower portion of a chain guard, the employee placed fingers between the guard and floor grating. The guard slipped pinching employee's left ring finger causing a laceration requiring 16 stitches.

Two maintenance mechanics were disassembling a bucket elevator. One mechanic was holding a punch in place with a tool holder while the other mechanic struck it with a sledgehammer. A piece of metal that had mushroomed around the end of the punch sheared off and cut the left thumb of the mechanic holding the tool holder. The EE received three stitches to close the wound.

Employee was in the breakroom and used a utility knife to remove a flash holder from safety vest that had been secured with a zip tie when employee accidentally cut left thumb. Employee received 3 stitches to close the laceration.

Employee was welding and got photokeratitis in both eyes when they lifted their welding hood. Employee received medical treatment beyond first aid.

EE was installing an air knife on the alt fuels belt when the air knife shifted approximately 4" and struck EE's left eyebrow causing a 1" laceration. The air knife structure has no sharp edges and weighs approximately 3 pounds. The employee received 6 stitches and returned to work immediately with no restrictions.

EE jumped down from a storage trailer that was approximately 4 feet in height. When EE landed, left knee gave out. After visiting the doctor following the incident, EE was returned to work with restrictions that still allowed EE to perform EE's job duties. On August 11th, EE had an MRI conducted and it was discovered that EE had a torn ACL and meniscus in left knee.

Employee was injured while installing a sprocket on a drive shaft in the maintenance shop. EE was striking the sprocket with a hammer when a piece of metal approximately 1/4" in size sheared from the sprocket and stuck into EE's right arm. EE received one stitch to close the wound. EE was wearing gloves, safety glasses and a hard hat.

EE was injured while opening the hatch doors on the truck sander. EE tried to stop the hatch door from falling and hitting another employee and in doing so pinched the pinky finger on right hand between the hatch door and the door jam. EE was wearing work gloves.

Employee was working on the outside of a bucket elevator attempting to bend a stud using a piece of pipe as leverage. The stud snapped, causing the employee to step backwards off balance and twisting their right knee.

While vacuum cleaning interior of the refractory line duct an employee's hand became lodged in vacuum hose. The truck was shut down and the employee evaluated. Corrective Action: Vacuum breaker and hose handle to be installed on all vacuum operations involving personnel handling the flex hose.

EE was connecting a trailer to the Maintenance pickup truck. EE pulled the tongue of the trailer towards the ball-hitch and pinched EE's finger between the trailer and the ball-hitch, lacerating the ringer finger on EE's left hand. EE returned to work shortly after receiving 7 stitches.

Fuel leaked from a #2 Fuel Oil pump and came in contact with hot machinery causing a fire

EE was chipping hardened cement off a conduit and a large chunk broke free and hit EE in the mouth. Medical treatment from dentist resulted in extraction of two cracked upper left teeth.

EE was attempting to move the tongue of the kiln ramp when ee felt discomfort in shoulder and back. The incident resulted in a torn bicep and rotator cuff.

Employee was carrying chain fall in their arms and over their shoulder. Due to unsafe ice conditions on the walking surface of the normal route, employee had to step over pipe and caught their foot in the chain, falling onto their arm and resulting in a compound fracture of the left radius.

Employee was placing red tape up to protect workers below an area to be worked. The employee slipped and tore right quadriceps muscle.

Employee positioned a steel frame on a sawhorse so they could paint it. Frame became unstable and tipped over. Employee grabbed it and the edge of the frame lacerated the left forearm. Laceration required 16 stitches to close wound. Employee came back to work after being stitched.

Employee went directly to coal screw area at start of shift to ensure no blockages. Within a couple minutes of arriving at coal screw, employee radioed that ee needed help. Supervisor found ee laying on the grating stating that ee had injured hand. Employee had lost the tip of one finger and lacerated two others. Employee stated ee didn't know how it happened.

Employee was using a sledgehammer to remove a jackhammer bit that was stuck. Employee missed the target and left hand struck the target. The result was a broken index finger on the left hand.

Employee was climbing the ladder onto the quarry haul truck and when they reached the third step. they felt their left hamstring pop. Employee suffered a strained hamstring. Employee was on modified duty for two days.

Employee was tightening a bolt when the wrench slipped causing employee's hand to strike a vertical beam. Employee has a fracture on the right hand.

Employee was closing the freight elevator door and accidentally got one hand between the doors and closed the door on fingers causing a laceration to right pointer finger.

Employee using hand grinder with cutting disc to trim off steel patch, disc caught on edge of steel and shattered. Piece of disc struck left forearm lodging under skin causing laceration. Stitches required to close wound.

Employee was using a knife to open packaging when EE nicked right thumb. The laceration required two stitches and EE was released to full duty.

Tightening a nut and bolt with a spud wrench and the wrench slipped and hit EE in the nose. Doctors are saying that it is suspected to be broken. The practitioner report says that this person is on 'modified duty'. Modified to include limited lifting, minimize bending and no heavy pulling or pushing. This person has been off of work since the injury happened.

Employee had just completed a cleaning task and sat down to wipe sweat from his face. He had taken his safety glasses off and a gust of wind blew some dust in his eyes. We sent him to the emergency room to get his eyes flushed and the Dr. requested that the employee remain out of work until Monday morning. scheduled shift on 11/11/15.

Employee was using a bar to dislodge another bar at a slag feeder. When attempting to dislodge the bar, he pinched his finger against the bar that was stuck suffering a laceration to his right index finger.

Employee was shoveling up a spill with 3 other workers when another worker's shovel struck him on the left wrist. He suffered a contusion to the left wrist.

Employee was working in a dusty area when sweat from his forehead dripped down his face carrying dust into his left eye. Employee's eye became sore and needed medical attention.

Our employee was handling a beam on our job site at the plant. He got his hand caught briefly between pinch point and as a result received bruising and a cut on the back of his hand that required stitches. He went to the nearest emergency facility, was treated and returned to work.

Employee was using a come-along to move over a screw auger trough with a wire rope sling wrapped around it. Whilst pulling the lever the wire sling broke and the employee fell backwards and hit his upper back on the edge of a steel beam. This contact caused a contusion on his back. The doctor's note stated "ee was seen today in the ER. He should not work tomorrow".

Large rocks were being tossed out of the Pit. One rolled back in, landing on employees foot, just behind the steel portion of his boot.

Employee attempted to help another employee cut a board. Just as the employee cutting the board was finishing the cut, the injured employee got his hand too close to the blade of the saw and the blade nicked his finger on his right hand. He received two small cuts which required a total of 3 stitches. Released for regular duty.

Employee was walking to a job when he slipped on a patch of ice and fell injuring (contusion) his left shoulder.

Employee was performing his pre-shift workplace exam and slipped on a patch of ice. The area was sanded and he stepped on the sanded ice but his right leg slipped. He strained his right knee.

EE was working with another EE to remove the headwheel assembly of the 781 drag conveyor using 2 chainfalls. While the load was partially lifted, EE reached around the headwheel assembly to retrieve a wedge and the headwheel assembly shifted. When it shifted, it pinned his arm against a beam causing bruising to his right arm.

EE was using a jackhammer to clean inside a hopper when he decided to sit and take a break to wipe sweat from his head. He removed his hard hat and gloves and wiped sweat off. When he stood up, he slipped. When he slipped, he grabbed onto a piece of metal in the hopper to prevent the fall and the metal cut his left hand. The laceration required 5 stitches to close.

Employee was loading a 100 lb. propane cylinder into the back of a truck when the cylinder slipped and pinched his finger on his right hand against the side of the truck body. He received 7 stitches to close the laceration.

Employee was plowing snow and he had to get off the loader to speak with an employee. While returning to his loader, he slipped on the ice breaking his right ankle.

EE was attemting to free up some material in the Gypsum material by using a pipe to stick into the access hole. While ramming the pipe in the hole, EE's hand struck the frame of the weigh feeder which resulted in a laceration to the right thumb.

EE was carrying cooler blocks for repairs to the Clinker Cooler. He suffered a lower back strain.

EE was removing electrical cabinets when he came into contact with electrical current. He received an electrical shock.

Employee was operating a front end loader and he drove the loader into an uncovered reclaim feeder. The loader dropped quickly and the employee strained his lower back.

EE cleared a hopper of rock and sand and his hand was hit by a rock from the belt feeding that hopper. He suffered a broken right thumb.

Employee was lifting a rotary feeder onto a manbasket when it slipped and jammed his hand against the handrail causing puncture wounds to his right hand.

Employee was using a torch to cut down some old pipes when one of the pipes fell and knocked him off his step ladder causing a laceration to his left ear.

EE was closing a door by pulling with his left hand. While pulling the door, he felt his left middle finger pop. He has a strained finger.

EE was cleaning in the preheater tower when dust fell onto his hard hat. When he took the hardhat off, dust fell into his left eye causing abrasions to the eye.

EE was using pressurized air to clear a pipe when the air hit a restriction in the pipe and dust blew back in his eyes.

EE was using a jackhammer through an access port in #6 rock feeder when a chunk fell hitting the jackhammer bit and forcing the other end of the jackhammer against his head. He suffered a laceration to the left side of his head requiring 6 stitches.

EE was lifting a counterweight, when he heard a pop in his lower back.

Employee was jackhammering when a piece of hot material fell from above and hit him on the neck and head. He suffered burns to his left hand and neck area as well as a broken right thumb.

EE was working on a dust collector and got dust on his forehead causing burns to his forehead and nose.

Employee was operating a winch when his back become sore.

EE was using the high pressure water blaster to clean in the preheater tower. He pulled the gun out of one port & went to another. The trigger engaged and the high pressure water was aimed at EE's foot. The water cut through his boot and into his right foot, causing a deep puncture wound.

EE was using a prybar to close a door in the preheater tower, when the bar slipped and he fell spraining his knee.

EE tripped over a bolt sticking up from floor. He fell to the floor injuring his shoulder and ribs.

Shutting the freight elevator door, hand slipped off strap striking right hand against lower door.

Employee was cleaning out the clinker feeder when the bar slipped and his wrist hit the steel feeder. He has a contusion to his right wrist.

EE stepped into hot dust and burned his feet.

Employee was attempting to remove a bearing with a bearing puller. While applying pressure to the puller, the bearing housing broke and he fell down straining his lower back.

Employee was working on the primary screen deck. When he went to move from the upper deck to the lower, he felt his shoulder pull.

Employee was in the control room getting ready to go onto the burning floor. When he put on his safety glasses, dust fell into his eye.

Employee was working on the plant roads when he stepped on a nail. The nail caused a puncture wound and he requested a tetanus shot.

Employee was attempting to lift a machine guard back in place when it slipped & fell on his right index finger. His finger is broken.

Employee was welding patches on the walls inside the bottom part of the dust collector when over a period of time his right arm became inflamed.

EE was attempting to see if a feeder was plugged so he had the feeder started. When it started, the edge of the pile gave way and EE slid in with the material.

Employee was attempting to pry a piece of steel from a loader bucket. The steel suddenly came loose and the steel struck him on his lower right leg. The injury required 6 stitches and he is on regular duty.

Employee was using an airlance for an extended time to help the flow of iron ore. The result was a strained left shoulder.

EE was drilling holes for the next shot. When he exited the cab of the drill to check one of his holes, he stepped too close to the edge of the bluff and slid over the edge. Hairline fracture - left foot.

While employee was poking at a plugged hopper, a piece of material fell aand hit the end of the bar. When it hit the end of the bar, the end he was holding went up and jammed his finger against the side of the hopper. He sustained a crushing injury to his ring finger on his left hand.

Employee was attempting to use compressed air to bow down some sticky material. He forgot to turn off the air cannons and opened up a 1" hole to put the air lance into. Just as he was inserting the air lance, the air cannon fired and blew hot dust out the hole knocking his face shield and glasses off his head. He got hot dust into both eyes. He has scratches on both eyes.

THE TANKER FROM WHICH AMMONIUM HYDROXIDE WAS TO BE EXTRACTED DID NOT LOOK RIGHT TO AN EE AND EE PLACED HIS HAND ON EXTRACTION VALVE ASSEMBLY IN THE AREA OF THE WING NUT ON THE CLAMP THAT HOLDS BONNET PORTION OF VALVE IN PLACE. AT THIS POINT CLAMP RELEASED SHOT OUT UNDER PRESSURE FOLLOWED BY A STREAM OF AMMONIUM HYDROXIDE. EFFORTS TO STOP THE FLOW FROM TANKER WAS UNSUCCESSFUL

EMPLOYEE WAS SCRAPING CEMENT FROM FORMS OUTDOORS. SNOW IN FORMS CAUSED WORKPIECE TO SLIP FROM HIS HAND AND STRAIN HIS LEFT FOREARM. TENDONITIS DIAGNOSED & Dr ADVISED THAT THE 64 YEAR OLD EMPLOYEE SHOULD COMPLETE 2 WEEK COURSE OF PT/STRETCHING WHILE WORKING AT REGULAR JOB WITH CARE NOT TO STRAIN FOREARM. TREATMENT COMPLETED & EMPLOYEE RETURNED TO FULL DUTY 2/2/05.

Ee was pulling on a slack cable and got his hand too close to a pulley. The pulley caught his glove and pulled his fingers in.

Employee was cleaning in the riser duct and wearing a face shield and safety glasses while working. He was poking in a hole in the riser duct when he looked up. There was some dust that had settled on top of his face shield and when he looked up, the dust fell into his left eye. He has a chemical burn in his left eye. Estimated date to return to work is Sept. 14th.

Employee developed rash on skin. Initial diagnosis by doctor was that it was due to protective outer wear employee was wearing. Has stopped wearing protective suit and rash still developing on face and neck. Also has some rash on arms.

EE DRIVING LOADER ACROSS RAILROAD TRACKS, THE LOADER MOVEMENT CAUSED TWISTING, INJURING EE'S BACK.

EE was in the process of gathering misc. steel for other crews. He was squatted down and as he slid the piece to the side he felt a pop in his back.

INSTALLING HEAD BOLTS OVER #9 INTERSTICE SILO. CREW MEMBER STANDING ON HOLE COVER WORKING CHAINFALL. AS HEAD BOX CLEARED TANK FLANGE LOAD SHIFTED AND SLID PLYWOOD HOLE COVER WITH EMPLOYEE ON I T PAST LIP OF FLANGE. PLYWOOD TIPPED UP CAUSING EE AND PLYWOOD HOLE COVER TO FALL INTO TANK. HIS CREW MEMBER HEARD CHAINFALL TRAVEL APPROX 20' WITH EE HOLDING ON, THEN HE FELL TOO.

EE WAS STEPPING OVER STATIC-RATLINE IN FINISH MILL WHEN HE STEPPED ON A PIECE OF 2"X2" ANGLE IRON AND TWISTED HIS LEFT ANKLE.

EE SAID HE WAS WALKING UP STAIRS TO PREHEATER TOWER AFTER MORNING STRETCHES. IT WAS EXTREMLY COLD OUT AND HE FELT A PULL IN HIS RIGHT CALF. WAS TAKEN TO DR AND DIAGNOSED WITH ACHILLES TENDONIT IS.

EE WAS ASSISTING WITH SECURING A 3' BY 4' BY 1/4PLATE ON THE PREHEATER TOWER. WHILE PUSHING PLATE UNDER PIPE TO TACK WELD IT DOWN. EE FELT A TEAR IN HIS RIGHT ARM.

EMPLOYEE SAID HE WAS BUILDING CONCRETE FORMS AND DID NOT FEEL ANY INCIDENT THAT CAUSED PAIN. ON HIS WAY HOME HE STARTED TO FEEL TIGHTNESS IN HIS RIGHT SHOULDER MUSCLE. WAS IN PAIN THROUGH THE NIGHT. BROUGHT TO DOCTOR WHO PLACED HIM ON WORK RESTRICTIONS FOR FIVE DAYS.

EE WAS WORKING ON A CHUNK OF CLINKER IN THE RECLAIM TUNNEL WITH A PRY BAR. BAR SLIPPED AND EE SMASHED HIS HAND BETWEEN PRY BAR AND SIDE OF FEEDER.

EE WAS IN A MANLIFT INSTALLING 6" X 15' PIPE THAT WAS SUSPENDED BY A NYLON SLING ON A PITMAN BOOM TRUCK. TRUCK COULD NOT BOOM OUT BECAUSE OF OVERHEAD INTERFERANCE. EE & FOREMAN WERE PUSHING ON PIPE TO DRIFT INTO PLACE AS THEY BOOMED DOWN LOAD. PIPE MOVED TOWARD EE WHICH PRESSED HIS BACK INTO HANDRAIL OF MANLIFT CAUSING MUSCLE STRAIN MID-BACK.

WHEN STEPPING DOWN FROM FAN LEVEL TO BAGHOUSE DAMPER LEVEL HEEL OF EE'S BOOT CAUGHT CAUSING EE TO FALL FORWARD ONTO PISTON HOUSING FRAME. EE LANDED ON FLANK (LEFT RIBS).

EE WAS ATTEMPTING TO CHANGE A BIT ON A RIVET BUSTER WHEN THE TRIGGER WAS ACCIDENTLY HIT, BLEW DUST INTO EYES.

EE LOAD RESTACKING BAGS OF CEMENT ON A PALLET.

EE WAS CARRYING CHAIN INTO KILN - TRIPPED FALLING ON CHAINS.

EE WAS POUNDING COAL THROUGH THE GRATING OF INCLINE BELT CATWALK.

EE WAS DIGGING OUT THE MATERIAL BUILD-UP UNDER THE RAW MILL PEBBLE SCREEN.

EE WAS TIGHTENING HOSE FITTING WITH WRENCH. TOOLSLIPPED OFF FITTING CAUSING EE TO FALL TO GROUNDLANDING ON ROCKY AREA.

EE APPLYING FORCE TO MACHINERY WITH PRY BAR. BAR SLIPPED WEDGING HAND BETWEEN SUPPORT COLUMN & PRY BAR.

LADDER SLID-EE SUPPORTED SELF BY ARM AND HAND WHILE STRAIGHTENING LADDER.

EE WAS WORKING ON A DE-ENERGIZED, LOCKED OUT COOLER FAN MOTOR. WHILE INSTALLING A NEW V-BELT TIPOF HIS FINGER WAS CAUGHT BETWEEN BELT & SHEAVE.

EE WAS ATTEMPTING TO FREE MATERIAL FROM A SILO. AS HE WAS STRIKING THE CHUTE WITH A HAMMER HE HIT HIS FINGER.

METAL BARS WERE BEING USED TO CLEAR CHUTE BAR SLIPPED OUT OF EE'S HAND FALLING ON HIS FINGER

EMPLOYEE SLIPPED ON LOOSE MATERIAL AS HE ATTEMPTED TO CATCH HIMSELF FROM FALLING, HE STRAINE DHIS SHOULDER.

EE SLIPPED WHILE CLIMBING SCREW CONVEYOR CROSS-OVER LADDER, INJURIES SHOULDER ATTEMPTING TO CATCH HIMSELF.

WHILE DOING ROUTINE MAINTENANCE HAND BECAME PINCHED BETWEEN CLEVIS AND HEAD OF RETRACTING PNEUMATIC RAM.
