The operator was using and excavator to lift a load and the material slipped on the employees hand when employee tried to put lifting strap under load.

Mostrando los 11While attempting to reposition the flange holes of a 12" HDPE pump line, the EE removed the last bolt and the pump line shifted and hit EE in the lower left leg. The incident did not involve any equipment or compliance issues. It also did not involve job skill, proficiency, training, PPE or protective devices.
Employee was welding when a piece of slag burned through employee's pant leg and down into right boot, burning the outside of right ankle. Employee continued to work until 7/23/2019 when employee was taken away from work because of the burn for three days and returned to normal duty on 7/26/2019.
Employee reported that metal plate he had just cut, fell on his foot just above the steel toe.
Employee was in vacinity of welding operations. Was notified prior to welder striking electode. Employee closed eyes and shielded eyes with hardhat. Employee completed shift and began having eye pain at home. Employee went to Emergency Room later that evening where he was diagnosed with Ultraviolet Keratitis (welder's burns).
Employee was running a rock crusher, when a rock hit him in the back of head under his hard hat.
DURING RECENT RENOVATIONS EE WAS BENDING OVER CUTTING IRON FOR A FEW HOURS, AND LIFTING AND MOVING IT. AT LUNCH HE COMPLAINED OF PAIN IN NECK, SHOULDER AND ARM. DR SAYS AREA IS INFLAMED FROM S TRAIN.
HE WAS OPERATING DROPBALL THE DROP BALL SLANTED ON PART OF ROCK.IT THREW TIRE INSIDE OF DOOR AND HIT HIS LEFT ARM.
GOT THROUGH WELDING AND HAD TO PUT RODS BACK INTO THE STINGER. HAD TO TURN HEAD PART OF STINGER TO TIGHTEN ROD AND HE WAS STANDING TOO CLOSE.
EMPL STRAINED LOWER BACK MUSCLES WHILE OPERATING CRANE LEVERS.
HE WAS DRILLING AND ROCK FELL ON LOWER LEFT LEG AND KNOCKED HIM OFF OF TRACKS ONTO BACK AND NECK.
