While positioning a pinion for install for the Pebble Mill, an employee's finger was pinched between the pinion and a liner bolt on the Pebble Mill. The employee sustained a laceration to their left index finger

Shovel operator stated jarring their left shoulder while operating the shovel mining out a ramp, and experienced left shoulder pain on 02/19/2025. Employee has a history of left shoulder injury. Employee continued to work full duty until receiving medical treatment for their left shoulder on 10/08/2025.

Maintenance mechanic was wire wheel grinding a weld when a foreign body entered right eye. Employee stated they were wearing safety glasses and face shield. Foreign body later removed by physician. Employee experienced no eye damage.

Equipment operator reported feeling a burning sensation in left shoulder during normal operation of a Cat 24M Grader. Employee reported no traumatic event leading to the reported pain.

Employee reported left eye irritation after drilling on concrete. Employee reports having been wearing PPE at the time.

A Dozer operator reported (2) chipped teeth following routine dozing activities.

An employee was kneeling down on a foam pad using a 6" handheld grinder to remove welds in the #2 dryer when the grinder kicked back, contacting the employee's left inner thigh above the knee resulting in 15 stitches.

Employee was working on Line 10 for 3 conveyor cutting bolts when a hot bolt fell and burned through the employees pants and into their boot. Employee sustained a full thickness burn to the left foot.

Employee reported to supervisor a sore elbow with no explanation before the shift started on 1/11/23. Employee was given light duty. Employee was let go for erratic behavior on 1/15/23. Employee reported on 1/17/23 that elbow was broken. Employee believes it is from jackhammering the night of 1/10/23.

The employee was removing a bolt from a liner on the #1 Crushed Product Feeder when they received a cut from the bolt on their left index finger resulting in 3 stitches.

Employee was grinding rust off the wall of a filtrate tank to prep for a patch and a foreign body entered eye.

Employee was cleaning inside t-1 #4 vacuum header line, when a plug of iron ore concentrate and water released from the pipe, forcing the material and the employee out of the pipe. The employee was ejected about 15' out of the pipe, while falling 6' vertically to the concrete floor below, resulting in fractures to the left arm, right femur, and pelvis.

While grinding the grinder wheel broke apart and struck the employee in the leg. All proper PPE was being utilized.

A shovel operator was loading a truck when they experienced right wrist pain. Reportedly, the employee experienced pain when EE was operating the joystick.

Repairman cut EE's left thumb with a 6 inch Metabo grinder while cutting the head off a 3/8 inch bolt. EE was holding the bolt with a small pair of channel locks and cutting with the other hand when the grinder kicked back.

Employee was kneeling and using a 9" grinder for an extended amount of time. It kicked back striking the employee in the leg causing a laceration leading to 6 stitches.

Using a grinder, stopped and cradled it on EE's arm. No finger on the trigger but the grinder must have shifted and started up. The grinder wheel cut through EE's welding jacket causing a laceration to EE's left forearm.

While transporting the incher with the overhead crane in the mill, employee was looking up and felt something enter right eye.

Employee was operating the overhead crane via a remote control box with their right hand and maneuvering a discharge liner with their left hand. Tip of left hand ring finger was pinched between the liner being lifted and a stationary liner positioned in the storage rack.

EE lowered the overhead crane block hooking the bridle onto the hook and started to pick it up. Leg of bridle got lodged in grating, snapped free swinging the chain and hook hitting ee in right forearm. Ee was taken to a hospital. The employee sustained a laceration requiring stitches. Employee was returned to work.

Employee was welding a door frame and chipped the slag off the weld when they felt a foreign body enter the left eye.

While removing a rail an end tipped down slightly when hoisted by a crane causing the rail mounted trolley to roll inward toward the mill. The moving trolley struck the hand grasping the rail resulting in two fingers sustaining cuts. Employee was treated by site responder, then taken to hospital where they received stitches.

Employee used a torch to cut a piece of 5/8" mild steel plate in the lower repair bay. The plate employee was cutting was standing up vertically in the plate rack. When employee was finishing the cut, the piece of steel started to fall and employee attempted to catch it but it pinched employee's middle finger as it hit the floor. Employee was taken to a medical facility.

Employee operating metal lathe using an emery cloth to complete finish on a pin when cloth doubled up pulling EE's hand into turning piece resulting in finger laceration. Taken to ER, obtained stitches to finger.

While using a 9 inch grinder the cut off disc broke apart and a piece of the disc fell into the employees right eye.

*****Alleged***** TD-44 operator reported dozing in barn when equipment rolled over a chunk that resulted in jarring of back. Operator was seen by site responder, and then brought to health care facility for further evaluation. Given time off and restrictions.

Welder was in the process of cutting a strip of metal into 4" pieces using a metal shear. While shearing the piece of metal a tip-up occurred allowing a finger to be pinched between the piece and table that resulted in lacerations to finger. Brought to ER. Stitches received and x-ray showed a fracture.

Employee was removing a lauder when the lauder came down pinching the employee's left index finger between the grating and the lauder. Evaluated by nurse and taken to ER. Laceration stitched. Bone fracture identified. EE taken off work. Referral to orthopedics.

While the operator was ripping with a track dozer the operator states experienced some back pain while performing this task. Operator did not want to be seen by EMT and continued working. Employee still had back pain 3 days later and was seen by NP. Diagnosed with back strain. Treated conservatively.

While operating the primary mill overhead crane employee sustained debris in eye. Eye flushed by nurse. Sent to eye doctor for foreign body removal. Prescription for eye drops given.

Employee was grinding on steel and felt something was in employee's right eye. There was debris falling from above. Employee did not report this until the next day.

While an employee was loosening house bolt on S33 with an impact, some debris got into eye. EE tried to flush debris from eye on site. EE was brought to the hospital where a foreign body was remove from eye and employee returned to work. Safety glasses were being worn at the time of the incident. Prescription eye medication ordered.

Alleged: EE stated on next shift that EE got something in eye when needle scaling the previous shift. States self flushed and was wearing safety glasses and face shield at time of event. On 4-20-17, EE was evaluated in the ED and given rx medication for eye.

EE was using a hand held grinder with a cut off wheel to make modifications to a conveyer belt guard when ee lost control of the grinder striking right leg just above the knee causing a laceration. Taken to local emergency room. 6 stitches required to close laceration. Returned to work with restrictions.

Tilden Plant Repairman was torqueing bolts on #2 Dryer with a high torque hydraulic wrench. Employee placed hand on wrench and asked partner to apply hydraulic pressure. At this time his finger became caught in between the wrench and bull gear flange, resulting in 13 stitches. Employee is off work on administrative leave post drug and alcohol testing.

Alleged aggravation of a pre-existing injury from 1-15-07 back strain from slipping on deck of drill while shoveling snow and 12-16-09 neck strain from being jostled in loader. EE continues to state pain in lower back, neck while operating mobile equipment. The employee was evaluated by a Physician & physical therapy was ordered. The EE has completed a series of visits.

Alleged, employee states he is not aware of a specific work related event to substantiate this claim. Employee states he had been welding and grinding in previous days with adequate PPE worn and no eye irritation or event reported during the time of welding or grinding. Days later employee was seen and treated for a foreign body in eye.

Alleged - employee states that when he was being trained on an Electric Shovel his back became sore. Seven days after the alleged incident the employee makes mention to on-site nurse.

Employee alleges that a foreign object entered in his right eye while grinding. He was evaluated by a physician on 10/9/14 and a piece of metal was removed. He was issued antibiotic eye drops and taken off of work for one shift (10/10). He returned to work full duty on 10/11.

On 12/29 employee alleged that during his previous shift (12/28) he was jostled while shifting a dozer from forward to reverse, resulting in back pain. The employee was evaluated by a doctor on 12/29 and issued Rx muscle relaxants & released back with restrictions. He was unable to report for 2 shifts due to the side effects of the muscle relaxant. He returned full duty on 1/1/14.

The employee was holding a pulp lifter in place while another employee was cutting the last bolt. The pulp lifter slipped and struck the employee in the forearm resulting in a laceration. The ee was evaluated by a Physician and sutures closed the wound. He returned to work.

Employee received a cut to his left knee while grinding, grinder kickback.

Employee was cutting floor grating. The cutting saw bound up and when it freed up a piece of the steel gave away and pinched off the tip of his finger. He was evaluated by a physician and was taken off of work.

Employee was drilling hole when bit got stuck. Torque on drill caused a clockwise quick motion of drill, catching his left pinky finger, resulting in a fracture and laceration of finger.

Employee was using a sawzall to cut conduit, the saw blade was pinched by the conduit. The saw then jumped causing the employee's hand to slip into the collect resulting in a fracture and sutures to his finger.

Employee was cutting conduit with a saws all and pinched his finger between the blade and frame of the saw resulting in a fracture to the end of his finger.

The medical was reviewed and it was made known on 1-17-11 that it was work related. Physical Therapy was ordered and a series was completed. The employee was driving his dozer, backed into a feeder and was jostled, resutling in an injury to his shoulder.

Employee complained of a foreign body in his eye during his work activities. He was wearing safety glasses and face shield along with a half mask repirator. He was evaluated by a physician and a small piece of metal was removed from his eye and eye medication was prescribed.

Employee was using a hand grinder when it kicked back cutting his left wrist area resulting in sutures.
