Two employees were performing maintenance on a conveyor belt. One employee was on top of the conveyor removing a flexco while the other began drilling a punch hole. During this process, the drill made contact with the first employee, causing a laceration to their right middle finger.

Mostrando los 40Employee was removing the end of bolts on a flexco fastener. The grinder came back towards the employee lacerating the side of EE's right knee.
Maintenance worker was working on the 1313 portable crusher changing blow bars. EE was operating an air chisel to break up dirt and rock material and the chisel dropped down causing EE's right index finger to get pinched between the tool and the metal bar.
Employee was cleaning rust/debris from floor in the engine room. The generator kicked on and exhaust air stirred up dust, causing irritation to left eye. Employee sought medical attention on 5-27-23 after irritation persisted overnight; no foreign objects were found to be in the eye: however, the DR prescribed an Rx antibiotic eye drop as a precautionary measure.
Employee was welding and felt a foreign particle get into right eye from behind welding hood. PPE-welding hood and prescription safety glasses. employee flushed their eyes, Later that afternoon they reported the injury and medical treatment was provided at the clinic.
Employee received a foreign body in the left eye when a piece of slag got behind welding hood. The employee did not report the injury until 2/11/20 because they did not realize anything had entered the eye resulting in the foreign body having to be removed by a ophthalmic burr.
While performing maintenance on the primary crusher with the hood open the employee received an FBE in left eye. The employee did not report the incident until 9/4/2019 resulting in the FBE being embedded in the eye. The employee was wearing safety glasses and a face shield while grinding.
Drill operator overturned equipment. This resulted in a laceration to the back of head requiring medical treatment beyond first aid.
Employee was wearing eye protection while welding and grinding. At some point during the day metal dust fell into employee's eye in which he began to rub it due to irritation causing metal to imbed. Employee was taken to medical clinic to have metal removed from eye. He returned to work that same day without restrictions.
Employee was tightening bolts on a screen bearing and cut his right index finger on a wire holding the socket on a 1" impact wrench.
Employee was in the vacinity of a welding operation and obtained flashburn to eyes, in which the employee was treated and released w/ perscription medication. No restrictive duty that affect normal job routine.
EE was working from an aerial lift and as he activated the basket, it moved and he lost balance, falling next to basket rails. He strained his lower back. No time lost due to this incident. He returned to work the next day.
EE was performing maintenance on a crusher with an impact wrench. The bolt broke on jaw crusher and he caught his right hand between the wrench/I-beam causing a laceration between the thumb/index finger. He went to aohc and received 8 stitches and was bandaged. He was also given a tentanus shot and 2 prescriptions. He returned to work the nexd day.
Employee was operating a Bobcat skid steer loader, cleaning out under a Simplicity screen, when a large rock came out of the screen chute and fell inside the safety cab of the Bobcat loader and struck him on the inside of his left shin. Result : a deep bruise on left shin. The rock was approx: 12 inches by 4 inches.
Employee and a co-worker were dismantling/cutting up an old metal catwalk which was supported by a crane with chain slings and an additional chain to prevent excessive swing. The employee cut the last piece of metal; it swung free and hit him in the back of his head and knocked him down. He was assisted immediately and didn't think he needed a doctor.
EMPLOYEE WAS PREPARING TO DO MAINTENANCE ON JAW CRUSHER AND GOT ARM CAUGHT BETWEEN BELT AND SHEAVE WHEN COUNTERWEIGHT SHIFTED UNEXPECTEDLY.
EE WAS GRINDING A SCRATCH AWL TO SHARPEN THE POINT AND AS HE WAS SHARPENING THE TOOL, IT WAS CAUGHT BY THE GRINDING WHEEL AND THIS PULLED HIS HAND INTO THE WHEEL. HIS RIGHT INDEX FINGER CAME I NTO CONTACT WITH THE WHEEL AND CAUSED THE LACERATION.
EMPLOYEE HOOKED THE CABLE FROM THE WINCH TO THE CABLE AND TIGHTENED THE CABLE WHEN THE CABLE TIGHTENED THE WINCH BASE WELD BROKE AND THE BASEHIT THE EMPLOYEES LEFT FOOT.
EMPLOYEE WAS USING HAND GRINDER ON TEH CURSHER, WHILE WEARING PRESCRIPTION GLASSES ONLY. A PIECE OF METAL LODGED IN HIS EYE.
EMPLOYEE TRIED TO GET IN TEH MAIN BUCKET ON THE BOOM CRANE WHILE IT WAS IN THE TRAVEL POSITION. HE WENT FROM THE TRUCK BED TO THE BUCKET AND THE BUCKET SWUNG OUT CAUSING HIM TO FALL AND HURT H IS BACK.

EE WAS AIR ARCING PLATE OFF LOADER BUCKET WHEN SLAG GOT IN HIS LEFT EYE.
EMPLOYEE WAS STANDING BY THE TWO SLIDING DOORS AT THE BACK OF THE SHOP ANOTHER EMPLOYEE WAS GRINDING ABOUT TEN FEET AWAY & A PIECE OF SLAG GOT IN HIS LEFT EYE EMPLOYEE NOT POSITIVE THAT HE GOT SLAG AT THIS TIME BUT THINKS IT WAS THE PROBABLE PLACE

WHILE BENT OVER WELDING ON IMPACT HOT SLAG POPPED UP AND LODGED IN RIGHT EAR CANAL.
EMPLOYEE WAS GRINDING OVER HEAD WHEN SOMETHING GOT IN HIS EYE. HE THOUGHT HE RUBBED IT OUT, BUT LATER HIS EYE SWELLED AND BECAME INFECTED. HE WENT TO THE DOCTOR 07-27-93.
HE WAS CHANGING DRILL STEEL FOR I.NAME. I.NAME HAD NOTICED THAT THE STEEL HAD BROKEN IN THE COLLAR INSTEAD OF THE SHANK. I.NAME REACHED FOR THE STEEL WITH THE STEEL HOLDER TOOL AT THE SAME TIM E INJURED WAS REACHING FOR THE STEEL. THE STEEL TOOL STRUCK HIM ON BOTH HANDS. HE LOST HIS LITTLE FINGER ON RIGHT HAND AT 1ST JOINT.
HE WAS WELDING UNDER CANOPY OF SHOP WITH FANS ON. THE FANS BLEW DIRT IN HIS LEFT EYE.
JAW CRUSHER WAS STOPPED UP WITHA ROOT AND AFTER HE REMOVED IT AND TURNED TO WALK AWAY A ROCK WAS THROWN OUT OF BALANCE WHEEL AND STRUCK HIM IN THE MOUTH.
INJURED WAS USING HAND GRINDER AND A PIECE OF METAL LODGED IN HIS EYE. DID NOT REPORT IT TO SUPERVISOR UNTIL 5-29-92.

EMPLOYEE WAS SHOVELING EXCESS MATERIAL AND A LARGE ROCK FELL FROM SCREEN STRIKING EMPLOYEE ON RIGHT SIDE OF HARD HAT AND RIGHT SHOULDER, THEN STRUCK RIGHT STEEL TOED BOOT. EMPLOYEE LOST CONSCI OUSNESS FOR ABOUT A MINUTE.

EMPLOYEE WAS CU7TING TREES WITH CHAINSAW FOR ROAD. AREA WAS GROWN UP. AFTER MAKING A CUT EMPLOYEE STEPPED BACK, TRIPPED AND CHAINSAW CUT LEG.
